If the owner has hired a contractor or contractors to undertake work, they may be required
to be licensed in accordance with state and local regulations, If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law, If the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work, they are advised to contact the
City of Zephyrhills Building Department, 613-768-6611,
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contractor(s) sign portions of the ~Contractor Sections" of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, rather than the contractor, are responsible for the work, If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the city of Zephyrhills,

be done in compliance with all applicable laws regulating construction, zoning, and land

Application is hereby made to obtain a permit to do work and installation as indicated. I
certify that no work or installation has commenced prior to issuance of a permit and that
all work will be performed to meet standards of all laws regUlating construction, City
codes, zoning regulations, and land development regulations in the jurisdiction, I also
certify that'I understand that the regulations of other governmental agencies may apply to
the intended work, and that it is my responsibility to identify what actions I must take to
be in compliance, Such agencies include but are not limited to: *Department of

I also certify that, if fill material is to be used in Flood Zone ~AH or ~A,etc,", it is
understood that a drainage plan addressing a ~compensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida prior to permit
issuance.
A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code. Every permit
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is commenced. One 90 day extension of time
may be allowed for the permit with fee charge of $15,00, The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month period, or the project will be considered abandoned.
WARNING TO OWNER: YOUR FAILURE TO RECORD A NOTICE OF COMMENCEMENT MAY RESULT IN YOUR
PAYING TWICE FOR IMPROVEMENTS TO YOUR PROPERTY. IF YOU INTEND TO OBTAIN FINANCING, CONSULT
WITH YOUR LENDER OR AN ATTORNEY BEFORE RECORDING YOUR NOTICE OF COMMENCEMENT, JOBS UNDER

As we have discussed, the City of Zephyr hills does have provisions to
accept a down payment and establish a payment plan for water and

Dear Mr. Patel:
As we discussed earlier today, I have completed an informal
comparison between the City of Zephyrhills and Pasco County,
regarding utility connection fees and transportation impact fees,
Pasco County bases its utility (water and sewer) connection fees on the
size of the meter used for an individual project. I asked them for the
rates of a 2" water meter (for a 72-unit motel). The fees quoted to me
are: $14,730 for water and $34,610 for sewer, The total utility
connection fee is $49,340, The county's transportation impact fee is
$1,239.39 per unit (room) or $89,236, Total fees in Pasco County
would be $138.576,
In Zephyrhills, we base our water and sewer connection fees on the
actual fixtures used (water closet, lavatory and tub/shower), The fees
for the water closet (toilet) per room are $610.50, for the lavatory
(sink), fees are $203,50, and for the tub/shower, fees are $407 per
room. Total utility connection fees per room are $1,221. Total utility
connection fees in the city are $87,912. Transportation impact fees are
$847 per room, for a total of$60,984 for 72 motel rooms. Total fees in
the city are $148.896.

On August 6, 2003, a final walk-through for landscaping was performed on the Zephyrhills
Motel project with Jayantilal Patel, owner and Todd Vande Berg, City of Zephyrhills. The
owner was guaranteed a Certificate of Occupancy for the Quality Inn Motel provided the
following is completed within ten days of the inspection.
1) The 2nd and 4th Oak trees from the northwest corner of the site (near dumpster) are to be
relocated into the adjacent northwest corner interior island.
2) The remaining Oak trees along the north property line are to be relocated to the far west
interior islands.
3) Three CrapelWax Myrtles shall be planted fifteen feet apart east of the remaining oaks
along the north property line near dumpster.
4) Palm clusters shall be planted in all parking islands that do not receive the oak
relocation.
5) Landscaping (hedges and palm clusters) shall be planted along the east side of the
furthest east parking area along HWY 301.
Again, the owner is to complete this punch list before a Certificate of Occupancy is issued.
