The und'ersigned understands t~at this permit: may be subject ;~to "deed restrictions" which
may be IOOre restrictive than City regulat:ions. The undersigned aSSUlnes responsibility for
compliance with any applicable deed restr:icthms.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contraotors to undertake work, they .ay be required
to be licensed in accordance with state and local regulations. If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law. If the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work, they are advised to contact the
City of Zephyrhills Building Depart~ent, 813-788-6611.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contractor(s) sign portions of the "Contractor Sect!ons" of this application for which they
will be responsible. If you, as the owner signs as the contrilctor, you are indicating that
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting priyileges in the City of Zephyrhills.

it to the "owner" prior to commencement.

Application is hereby made to obtain a permit to do work and installation as indioated. I
certity that no work or installation has commenced prior to issuance of a permit and that
all work will be perfonmed to meet standards of all laws regUlating oonstruction, City
codes, zoning regulations, and land development regulations in the jurisdiction. I also

authority to Violate, cancel, alter, or set aside any prOVisions of the technical codes,
nor shall issuance of a permQt prevent the Building Official from thereafter requiring a

six months of issuance, or if' work authorized by the permit is suspended or abandoned for a

Dear Sirs,
This letter is to certify that I have reviewed the above referenced project to
determine the structural adequacy of the existing structure to receive the attached
structure. The proposed structure is roughly a IOxl2 shed, a 22x20 carport and
a12x20 open patio. The existing mobile home, to be considered an adequate
host structure, should be blocked and anchored in accordance with the
manufacturers suggested and permitted set-up permit.
Therefore, it is my opinion that the marriage of the proposed structure with the
addition will not compromise the structural integrity of the host structure, if the
addition is constructed as per the attached aluminum engineered drawings.
