REINSPECTION FEES: When extra inspection trips are necessary due to anyone of the following reasons, a
charge of twenty-five and 00/100 Dollars ($25.00) shall be made for each trip for each trade:
a. Wrong Address
b. Condemned work resulting from faulty construction.
c. Repairs or corrections not made when inspection called.
d. Work not ready for inspection when called.
e. Permit not posted on job site.
f. Plans not at job site.
g. Work not accessible.
The payment of inspection fees shall be made before any further permits will be issued to the person owning
same.

A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this permit may be subject to ndeed restrictionsH which
may be more restrictive than city regulations. The linder-signed assumes responsibility for
compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to underti~ke work, they may be required
to be licensed in accordance with state and local regulations. If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation' under state law. If the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work, they are advised to contact the
City of Zephyrhills Building Department, 813-788-6611.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contr~ctor(sl sign portions of the nContractor SectionsH of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

I certify that I, the applicant, have been provided with a copy of nFlorida's Construction
lien Law - Homeowner's Protection GuideH prepared by the Florida Department of Agriculture
and Consumer Affairs. If the applicant is someone other that the nownerH, I cerify that I
have obtained a coPY of the above described document and promise in good faith to deliver
it to the nownerH prior to commencement.
E. CONTRACTOR'S/OWNER'S AFFIDAVIT
I certify that all the information in this application is accurate and that all work will
be done in compliance with all applicable laws regulating construction, zoning, and land
development.
Application is hereby made to obtain a permit to do work and installation as indicated. I
certify that no work or installation has commenced prior to issuance of a permit and that
all work will be performed to meet standards of all laws regulating construction, City
codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other governmental agencies may apply to
the intended work, and that it is my responsibility to identify what actions I must take to
be in compliance. Such agencies include but are not limited to: *Department of
Environmental Regulation-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment
*Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas,
Altering Watercourses
*Army Corps of Engineers-Seawalls, Docks, Navigable Waterways
*Department of Health & Rehabilitative Services, Environmental Health Unit-Wells,
Wastewater Treatment, Septic Tanks
*U.S. Environmental Protection Agency-Asbestos abatement
I also certify that, if fill material is to be used in Flood Zone "A" or "A, etc.", it is
understood that a drainage plan addressing a "compensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida prior to permit
issuance.
A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code. Every permit
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is commenced. One 90 day extension of time
may be allowed for the permit with fee charge of $15.00. The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month ped.od, or the proj ect will be considered abandoned.
WARNING TO OWNER: YOUR FAILURE TO RECORD A NOTICE OF COMMENCEMENT MAY RESULT IN YOUR
PAYING TWICE FOR IMPROVEMENTS TO YOUR PROPERTY. IF YOU INTEND TO OBTAIN FINANCING, CONSULT
WITH YOUR LENDER OR AN ATTORNEY BEFORE RECORDING YOUR NOTICE OF COMMENCEMENT. JOBS UNDER

210.8(A):
All I 25-volt, single-phase, 15- and 20-ampere receptacles installed in the locations
specified in (1) through (8) shall have ground-fuult circuit-interrupter protection for
personnel.
( I ) Bathrooms
(2) Garages
(3) Outdoors
(6) Kitchens - where the receptacles are installed to serve the countertop surfaces
(7) Wet bar sinks - where the receptacles are installed to serve the countertop surfaces
and are located within 1.8 m (6 ft) of the outside edge of the wet bar sink.
210.11(C)(3):
In addition to the number of branch circuits required by other parts of this section, at least
one 20-ampere branch circuit shall be provided to supply the bathrOOJll receptacle
outlet(s)_ Such circuits shall have no other outlets.
Ex:ception: Where the 20-ampere circuit supplies a single bathroom. outlets for other
equipment within the same bathroom shall be permitted to be supplied in accordance
with 210.23(.4).
210.12(B): All branch circuits that supply 125-volt, single phase, 15- and 20-ampere outlets
(RECEPTACLES, LIGHTS, SMOKE DETECTORS) installed in dwelling unit
bedrooms shall be protected by an arc-fault circuit interrupter listed to provide protection
of the entire branch circuit.
210.52(A)(I): Receptacles shall be installed so that no point measured horizontally along the floor line
in any wall space is more than 1_8 m (6 ft) from a receptacle outlet.
210.52(B)(I): In the kitchen, pantry, breakfast room, dining room, or similar area ofa dwelling unit, the
two or more 20-ampere small-appliance branch circuits required by 2l0.11(C)(1) shall
serve all receptacle outlets covered by 21 0.52(A) and (C) and receptacle outlets for
refrigeration equipment.
210.52(C)(I): A receptacle outlet shall be installed at each wall counter space that is 300 nun (12 in.) or
wider. Receptacle outlets shall be installed so that no point along the wall line is more
than 600 nun (24 in.) measured horizontally from a receptacle outlet in that space.
21O.52(C)(2): At least one receptacle outlet shall be installed at each island counter space with a long
dimension of600 mm (24 in.) or greater and a short dimension of300 nun (12 in.) or
greater.
210.52(C)(3): At least one receptacle outlet shall be installed at each peninsular counter space with a
long dimension of600 nun (24 in.) or greater and a short dimension of300 mm (12 in_)
or greater. A peninsular countertop is measured from the connecting edge.
210.52(D): In dwelling units, at least one receptacle outlet shall be installed in bathrooms 900 nun
(3ft) of the outside edge of the basin. The receptacle outlet shall be located on a wall or
partition that is adjacent to the basin or basin countertop.

In dwelling units, at least one receptacle outlet shall be installed for the laundry.
In dwelling units, hallways of3.0 m (10 ft) or more in length shall have at least one
receptacle outlet.
As used in this sub-section, the hall length shall be considered the length along the
centerline of the hall without passing through a doorway.
The service disconnecting means shall be installed at readily accessible location either
outside of a building or structure or inside nearest the point of entrance of the service
conductors.
For permanently connected appliances rated over 300 volt-amperes or 118 hp, the branch-
circuit switch or circuit breaker shall be permitted to serve as the disconnecting means
were the switch or circuit breaker is within sight from the appliance or is capable of being
locked in the open position.
