REINSPECTION FEES: When extra inspection trips are necessary due to anyone of the following reasons, a
charge of twenty-five and 00/100 Dollars ($25.00) shall be made for each trip for each trade:
a. Wrong Address
b. Condemned work resulting from faulty construction.
C. Repairs or corrections not made when inspection called.
d. Work not ready for inspection when called.
e. Permit not posted on job site.
f. Plans not at job site.
g. Work not accessible.
The payment of inspection fees shall be made before any further permits will be issued to the person owning
same.

A. NOTICE OF DEED RESTRICTIONS
The undersigned understands th,t this permit may be subject to "deed restrictionsU which
may be more restrictive than City regulations. 'l'he undersigned aSSUmes responsibili.ty for
compliance with any applicable deed restrictions.
B. UlnICENSED CONTRAC'I'ORS AND CON'l'RAc'rOR RESPONSIBII,I'I'IES
If the owner has hired a contractor or contractors to undertake work, they may be required
to be licensed in accordance with state and local regulatJons. If the contracl:or is not
licensed as required by law, both the ovmer and contractor may be cited for a misdemeanor
violation under state law. If the owner or intended contractor are uncertaln as to vlhat
licensing requirements may apply for the intended work, they are advlsed to contapt the
city of Zephyrhills Building Department, 813-788-6611.
Furthermol--e, it the owner has hired a contractor or contractors, he is advised to have the
oontractor-(s) sign portions of the "Contractor Sections" of this application for which they
will be ~esponsible. If you, as the owner signs as the contractor, YQY are indicatlng that
you, rather than the contractor, are responsible for the work. If the contracto~ wisl16s
you to slgn as contractor that may be an lndication that he is not properly lioensed and is
lIot enUtled to permitting privileges in the City of Zephyrhil1s.

lien Law - Homeowner's Protectlon Guide" prepared by the Florida Department of Agriculture
and Consumer Affairs. If the applicant is someone other that the "o~ler", I carify that I
have obtained a copy of the above described document and promise in good faith to deliver
it to the \'owner" prior to commencement.

I certify that all the information in this application is accurate and that all work will
be done in compliance wlth all appl1cable laws regulating construction, zoning, and land
developmeIlt.
Application is hereby made to obtain a permj.t to do work and installation as indicated. 1
certify that no work or installation has corrunenced prior to lssuance of a permit and that
all work will be performed to meet standards of all laws regulating oonstruction, City
codes, zoning regulations, and land development regulations ln the jurisdiction. I also
certify that I understand that the regulations of other governmental agencies may apply to
the intended work, and that it is my responsibility to ldentify what actions I IIIUst take to
be in compliance. Such agencies include but are not limited to: 'Department of
Environmental Regulation-Cypress Bayheads, Wetland Areas and Environmentally Sensj.tJve

understood that a drainage plan addressing a "compensating volume" 'will be submitted vlhioh
is prepared by a professional engineer registered in the State of Florida pr10r to permit

A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alte!.-, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requlrlng a
correction of errors in plans, construction, or vio~ations of any code. Every permlt
is.aued shall become invaUd unless the work authoriz.ed by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandol1ed for a
period of siil months after the tlme the work is oorrunenoed. One 90 day extension of tillle
may be allowed for the permit with fee charge of $15.00. The extension shall be reques~ed
in Hriting to the Building Official. An approved inspection must be Jogged during each six
month period, or the project Vlill be considered abandoned.
