The payment of inspection fees shall be made before any further permits will be issued to the person owning
same.

CONDITIONS OF PERMIT AFFIDAVIT
A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this permit may be subject to "deed restrictions" which
may be more restrictive than City regulations. The undersigned assumes responsibility for
compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to undertake work, they may be requirf
to be licensed in accordance with state and local regulations. If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law. If the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work, they are advised to contact the
City of Zephyrhills Building Department, 813-788-6611.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contractor(s) sign portions of the "Contractor Sections" of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

D. CONSTRUCTUION LIEN LAW (CHAPTER 713, FLORIDA STATUTES, AS AMENDED)
I certify that I, the applicant, have been provided with a copy of "Florida's Construction
lien Law - Homeowner's Protection Guide" prepared by the Florida Department of Agriculture
and Consumer Affairs. If the applicant is someone other that the "owner", I cerify that I
have obtained a copy of the above described document and promise in good faith to deliver
it to the "owner" prior to commencement.
E. CONTRACTOR'S/OWNER'S AFFIDAVIT
I certify that all the information in this application is accurate and that all work will
be done in compliance with all applicable laws regulating construction, zoning, and land
development.
Application is hereby made to obtain a permit to do work and installation as indicated. I
certify that no work or installation has commenced prior to issuance of a permit and that
all work will be performed to meet standards of all laws regulating construction, City
codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other governmental agencies may apply
the intended work, and that it is my responsibility to identify what actions I must take ~~
be in compliance. Such agencies include but are not limited to: *Department of
Environmental Regulation-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment
*Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas,
Altering Watercourses
*Army Corps of Engineers-Seawalls, Docks, Navigable Waterways
*Department of Health & Rehabilitative Services, Environmental Health Unit-Wells,
Wastewater Treatment, Septic Tanks
*U.S. Environmental Protection Agency-Asbestos abatement
I also certify that, if fill material is to be used in Flood Zone "A" or "A,etc.", it is
understood that a drainage plan addressing a "compensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida prior to permit
issuance.
A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code. Every permit
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is commenced. One 90 day extension of time
may be allowed for the permit with fee charge of $15.00. The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month period, or the project will be considered abandoned.

PAYING TWICE FOR IMPROVEMENTS TO YOUR PROPERTY. IF YOU INTEND TO OBTAIN FINANCING, CONSULT
WITH YOUR LENDER OR AN ATTORNEY BEFORE RECORDING YOUR NOTICE OF COMMENCEMENT. JOBS UNDE'
$2,500 IN VALUE DO NOT NEED TO RECORD AND POST A "NOTICE OF COMMENCEMENT".

APPLICATIONS
Use to contribute to life safety, fire protection, and property
conservation. Photoelectric detectors are recommended in ar-
eas where slow smoldering fires are likely to ignite. In areas
where small combustion particles are usually present from forklift
trucks, cooking stoves, etc., they are less likely than ionization
detectors to produce false alarms.
CONSTRUCTION AND OPERATION
All System Sensor 2400 Series photoelectric smoke detectors
contain a unique optical sensing chamber designed to sense
the presence of smoke particles produced by a wide range of
combustion sources and meet performance criteria designated
by UL 268. A new custom integrated circuit incorporates signal
processing to reduce false alarms and sample/hold circuitry to
provide easy field metering of sensitivity. Additional key fea-
tures of AT/AIT models include available isolation of the ther-
mal for separate monitoring, and a built-in horn.
The built-in piezoelectric horn produces an interrupted 85 dBA
tone. The horn operates when the unit senses smoke, or when
the thermal reaches its alarm point (integral thermal models
only).

INSTALLATION
Model 2400 detectors are designed for systems use with UL
listed control panels. See panel Compatibility Chart to deter-
mine maximum number of detectors per zone. Easy to install
and maintain, this detector is designed for direct surface mount-
ing (mounting bracket included), or mounting to a 4" (101.6 mm)
octagon or smaller box. Easy-to-wire screw terminals allow fast
and simple field wiring of in, out and remote annunciator con-
nections.
Consult control panel specifications for the maximum allowable
loop resistance for the particular control panel to be used.
To prevent wiring mistakes, observe polarities and make cer-
tain that each conductor is identified. A copy of Installation and
Maintenance Instructions is packaged with each detector. For
further information, refer to NFPA 72 and to Local Authority
Having Jurisdiction.

MAINTENANCE
The 2400 Series has been designed to seal the sensing cham-
ber from back-pressure air flow, dust, dirt, and insects. The
back of the detector is sealed. The chamber is protected by a
fine mesh screen. Testing is accomplished by insertion of a
0.1" (2.54 mm) max. diameter tool. If cleaning is required, it is
easy to remove the cover and obtain access to the screen and
chamber to perform a thorough cleaning.
ENGINEERING SPECIFICATIONS
Smoke detector shall be a photoelectronic type (model 2400)
or a combination photoelectronic/thermal (model 2400TH) with
thermal sensor rated at 1350F (57"C) as manufactured by Sys-
tem Sensor. Wiring connections shall be made by means of
SEMS screws. The detector will have a visible LED which will
blink in standby and latch on in alarm. The detector shall have
a nominal sensitivity of 3% per foot as measured in the UL smoke
box. The detector screen and cover should be easily remov-
able for cleaning. It shall be possible to perform a sensitivity
and functional test without the need of generating smoke. De-
tector circuitry shall perform a self test on the sensing chamber
and internal electronics every 40 seconds. If circuitry fails, the
detector LED shall stop blinking. The detector shall have a
mounting bracket that allows for mounting to a 3-1/2" (88.9 mm)
or 4" (101.6 mm) octagonal or a 4" square electrical box.

GENERAL
The Fire-Lite FCPS-24F(E) is a compact, cost-effective, 6-
amp remote power supply with battery charger. The FCPS-
24F(E) may be connected to any 12- or 24-volt Fire Alarm
Control Panel (FACP) or may stand alone. Primary applica-
tions include Notification Appliance (bell) Circuit (NAC) ex-
pansion (to support ADA requirements) or auxiliary power to
support 24-volt system accessories. The FCPS provides
regulated and filtered 24 VDC power to four Notification Ap-
pliance Circuits (two Class A [Style Z] and two Class B [Style
Y] or four Class B only). Alternately, the four outputs may be
configured as any combination of resettable/non-resettable
power outputs (optimal for powering four-wire smoke detec-
tors). The FCPS-24F(E) also contains a battery charger ca-
pable of charging up to 7.0 Amp Hour batteries.

GENERAL
System Sensor's MDL Sync Module is designed to work with
the SpectrAlert series of horns, strobes, and horn/strobes to
provide a means of: synchronizing the temporal-coded horns,
synchronizing the one-second flash timing of the strobe, and
silencing the horns of the horn/strobe combination over a two-
wire circuit while leaving the strobes active.
MODULE CONFIGURATION
Each MOL module has the capability of connecting two Style Y
(Class B) circuits or one Style Z (Class A) circuit.
The NAC output(s) from the panel are connected to the zone
inputs of the MOL module and the zone output(s) from the MOL
module are connected to the notification loop(s). Supervision
is accomplished in the module by a direct connection between
the zone input and the zone output of each of the two zone
circuits connected to the normal end-of-line device. The FACP
"sees" the EOL device through the MOL module. When either
or both outputs (zones 1 & 2) from the module are wired to the
SpectrAlert products, the horns and strobes in both zones will
be synchronized.
The MOL module can be configured so that more than two zones
can be synchronized by the interconnection of the slave input
and output (see Application Examples).
SPECIAL CONSIDERATIONS
A latching Form-C contact is provided in case the synchroniz-
ing signal to the notification devices is interrupted. The output
can be wired so that a trouble signal will be annunciated at the
panel. If the synchronization pulse fails in the MDL module, the
strobes will shut off.
NOTE: The MDL Module is factory-set with the trouble contacts in the
open state. These contacts may close during shipping. ApproXimately
two seconds after power-up, these contacts will open.

LIMITATIONS OF HORNS/STROBES
The horn and/or strobe will not work without power. The
horn/strobe gets its power from the fire/security panel monitor-
ing the alarm system. If power is cut off for any reason, the
horn/strobe will not provide the desired audio or visual warn-
ing.
The horn may not be heard. The loudness of the horn meets
(or exceeds) current Underwriters' Laboratories standards.
However, the horn may not alert a sound sleeper or one who
has recently used drugs or has been drinking alcoholic bever-
ages. The horn may not be heard if it is placed on a different
floor from the person in hazard or if placed too far away to be
heard over the ambient noise such as traffic, air conditioners,
machinery or music appliances that may prevent alert persons
from hearing the alarm. The horn may not be heard by persons
who are hearing impaired.
The signal strobe may not be seen. The electronic visual
warning signal uses an extremely reliable xenon flash tube. It
flashes at 1 Hz over its entire operating voltage range, and ex-
ceeds current Underwriters' Laboratories standards for private-
mode viewing. The visual warning signal is suitable for direct
viewing and must be installed within an area where it can be
seen by building occupants. The strobe must not be installed
in direct sunlight or areas of high light intensity (over 60 foot-
candles) where the visual flash might be disregarded or not
seen. The strobes are ADA compliant, flashing at 1 Hz (one
flash per second) over their entire operating voltage range.
The signal strobe may cause seizures. Individuals who have
positive photic response to visual stimuli with seizures, such as
persons with epilepsy, should avoid prolonged exposure to en-
vironments in which strobe signals are activated.
The signal strobe cannot operate from coded power sup-
plies. Coded power supplies produce interrupted power. The
strobe must have an uninterrupted source of DC power in order

The ADT-8G-12L is a cost-effective, feature-packed, non-coded,
dual-action manual fire alarm pull station. It was designed to
meet multiple applications with the installer and end-user in
mind.
The ADT-BG-12L provides ADT Fire Alarm Control Panels
(FACPs), as well as other manufacturers' controls, with a
manual alarm initiating input signal. Its innovative design, du-
rable construction, and multiple mounting options make the

OPERATION
The ADT-8G-12L manual pull station provides a textured fin-
ger-hold area that includes Braille text. In addition to PUSH IN
and PULL DOWN text, there are arrows indicating how to oper-
ate the station, provided for non-English-speaking persons.
Pushing in and then pulling down on the handle activates the
normally~open alarm switch. Once latched in the down posi-
tion, the word "ACTIVATED" appears at the top in bright yellow,
with a portion of the handle protruding at the bottom as a visible
flag. Resetting the station is simple: insert the key, twist one
quarter-turn, then open the station's front cover, causing the
spring-loaded operation handle to return to its original position.
The alarm switch can then be reset to its normal (non-alarm)
position manually (by hand) or by closing the station's front
cover, which automatically resets the switch.

Manual Fire Alarm Stations shall be non-coded, with a key-
operated reset lock in order that they may be tested, and so
designed that after actual Emergency Operation, they cannot
be restored to normal except by use of a key. An operated sta-
tion shall automatically condition itself so as to be visually de-
tected as activated. Manual stations shall be constructed of
red colored LEXAN@ (or polycarbonate equivalent) with clearly
visible operating instructions provided on the cover. The word
FIRE shall appear on the front of the stations in white letters,
1.00 inches (25.4 mm) or larger. Stations shall be suitable for
surface mounting on matching backbox S8-10; or semi-flush
mounting on a standard single-gang, double-gang, or 4" (10.16
em) square electrical box, and shall be installed within the lim-

The System Sensor 5pectrAlert@ wall-mount series includes a complete
line of electronic horns, strobes, and horn/strobes. Intended for primary
signaling use, SpectrAlert@ products meet UL 1971, UL 464, and Ameri-
cans with Disabilities Act requirements.
Performance. With its extremely efficient reflector design and
Xenon flash tube. SpectrAlert@ offers current draw reductions as high
as 40% over previous generation designs. With less current consump-
tion, more devices per loop are possible for a lower installed cost.
Installation. SpectrAlert@ products are designed for ease of in-
stallation, which also lowers the installed cost. SpectrAlert@ strobes
and horn/strobes take up no room in the backbox, making wiring con-
nections simpler and faster. Each SpectrAlert@ wall-mount device in-
cludes a universal mounting plate for 4" (10.16 cm) square and single-
gang backbox mounting. Accessory mounting plates are also available
for small-footprint or surface-mount applications.
Flexibility. The SpectrAlert@ wall-mount series offers the flexibility
to meet a broad range of requirements. Horns and horn/strobes fea-
ture a number of field-selectable/reversible horn tones. Strobes and
horn/strobes are available in a wide variety of configurations to address
visibility requirements for non-sleeping areas, sleeping areas, and cor-
ridors. Offerings include 24-volt models at 15. 15/75, 30, 75, and 110
candela; and 12-volt devices at 15 and 15/75 candela.
Aesthetics. To meet the aesthetic requirements of building own-
ers, SpectrAlert@ incorporates a stylish, low-profile design that is con-
sistent across the SpectrAlert@ wall-mount product line.

ENGINEERING SPECIFICATIONS
General - SpectrAlert@ horns, strobes and horn/strobes shall
be capable of mounting to a standard 4" x 4" x 1-1/2" (101.6 x 101.6
x 38.1 mm) backbox or a single-gang 2" x 4" x 1-7/8" (50.8 x 101.6
x 47.625 mm) backbox using the universal mounting plate included
with each SpectrAlert@ product. Also, SpectrAlert@ products, when
used in conjunction with the accessory Sync-Circuit™ Module, shall
be powered from a non-coded power supply and shall operate on
12 or 24 volts. 12-volt rated devices shall have an operating volt-
age range of 10.5 - 17 volts. 24-volt rated devices shall have an
operating voltage range of 20 - 30 volts. SpectrAlert@ products
shall have an indoor operating temperature range of 320F to 1200F
(DOC to 490C) and operate from a regulated DC or full-wave recti-
fied, unfiltered power supply.
Horn - Horn shall be a System Sensor SpectrAlert@ model
capable of operating at 12 and 24 volts. Horn shall be
Listed to UL 464 for fire protective signaling systems. The horn
shall have two tone options, two audibility options (at 24 volts) and
the option to switch between a temporal 3 pattern and a non-tem-
poral continuous pattern. All horn models shall operate on a coded

Listed to UL 1971 and be approved for fire protective
service. The strobe shall be wired as a primary signaling notifica-
tion appliance and comply with the Americans with Disabilities Act
requirements for visible signaling appliances, flashing at 1 Hz over
the strobe's entire operating voltage range. The strobe light shall
consist of a xenon flash tube and associated lens/reflector system.
Horn/Strobe Combination - Hom/strobe shall be a Sys-
tem Sensor SpectrAlert@ model Listed to UL 1971 and
UL 464 and shall be approved for fire protective service. Horn/
strobe shall be wired as a primary signaling notification appliance
and comply with the Americans with Disabilities Act requirements
for visible signaling appliances, flashing at 1 Hz over the strobe's
entire operating voltage range. The strobe light shall consist of a
xenon flash tube and associated lens/reflector system. The horn
shall have two tone options, two audibility options (at 24 volts) and
the option to switch between a temporal 3 pattern and a non-tem-
poral continuous pattern. Strobes shall be powered independently
of the sounder with the removal of factory-installed jumper wires.
The horn on horn/strobe models shall operate on a coded or non-
coded power supply (the strobe must be powered continuously).
Module - Module shall be a System Sensor Sync-CircuiFM model
Listed to UL 464 and shall be approved for fire protec-
tive service. The module shall synchronize SpectrAlert@ strobes
at 1 Hz and horns at temporal 3. Also, the module shall silence the
horns on horn/strobe models. while operating the strobes, over a
single pair of wires. The module shall be capable of mounting to a
4-11/16" (119.063 mm) square x 2-1/8" (53.975 mm) deep backbox
and shall control two Style Y (Class B) or one Style Z (Class A)
circuit. Module shall be capable of multiple zone synchronization
by daisy-chaining multiple modules together and resynchronizing
each other along the chain. The module shall NOT operate on a
coded power supply.
