The undersigned understands that this permit may be subject to ~deed restrictions" which
may be more restrictive than City regulations. The undersigned assumes responsibility for
compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to undertake work, they may be required
to be licensed in accordance with state. and local regulations. If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law. If the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work, they are advised to contact the
City of Zephyrhills Building Department, 813-788-6611.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contractor(s) sign portions of the ~Contractor Sections" of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

lien Law - Homeowner's Protection Guide" prepared by the Florida Department of Agriculture
and Consumer Affairs. If the applicant is someone other that the ~owner", I cerify that I
have obtained a copy of the above described document and promise in good faith to deliver
it to the ~owner" prior to commencement.
E. CONTRACTOR'S/OWNER'S AFFIDAVIT
I certify that all the information in this application is accurate and that all work will
be done in compliance with all applicable laws regulating construction, zoning, and land
development.
Application is hereby made to obtain a permit to do work and installation as indicated. I
certify that no work or installation has commenced prior to issuance of a permit and that
all work will be performed to meet standards of all laws regulating construction, City
codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other governmental agencies may apply to
the intended work, and that it is my responsibility to identify what actions I must take to
be in compliance. Such agencies include but are not limited to: *Department of
Environmental Regulation-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment
*Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas,
Altering Watercourses
*Army Corps of Engineers-Seawalls, Docks, Navigable Waterways
*Department of Health & Rehabilitative Services, Environmental Health Unit-Wells,
Wastewater Treatment, Septic Tanks
*U.S. Environmental Protection Agency-Asbestos abatement
I also certify that, if fill material is to be used in Flood Zone ~A" or ~A,etc.", it is
understood that a drainage plan addressing a ~compensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida prior to permit
issuance.
A permit issued shall be construed to be a license to proceed with' the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code. Every permit
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is commenced. One 90 day extension of time
may be allowed for the permit with fee charge of $15.00. The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month period, or the project will be considered abandoned.

This permit is issued under the provisions of Chapter 373, Florida Statutes (F.S.), and the Rules contained
in Chapters 40D-4 and 40, Florida Administrative Code (FAC.). The permit authorizes the Permittee to
proceed with the construction of a surface water management system in accordance with the information
outlined herein and shown by the application, approved drawing(s), plans, specifications, and other
documents, attached hereto and kept on file at the Southwest Florida Water Management District (District).
All construction, operation and maintenance of the surface water management system authorized by this
permit shall occur in compliance with Florida Statutes and Administrative Code and the conditions of this
permit.

SPECIFIC CONDITIONS
1. If the ownership of the project area covered by the subject permit is divided, with someone other
than the Permittee becoming the owner of part of the project area, this permit shall terminate,
pursuant to Section 40D-1.61 05, FAC. In such situations, each land owner shall obtain a permit
(which may be a modification of this permit) for the land owned by that person. This condition shall
not apply to the division and sale of lots or units in residential subdivisions or condominiums.
2. The discharges from this system shall meet state water quality standards as set forth in Chapter
62-302 and Section 62-4.242, FAC., for class waters equivalent to the receiving waters.

3. Unless specified otherwise herein, two copies of all information and reports required by this permit
shall be submitted to:

The permit number, title of report or information and event (for recurring report or information
submittal) shall be identified on all information and reports submitted.
4. The Permittee shall retain the design engineer, or other professional engineer registered in Florida,
to conduct on-site observations of construction and assist with the as-built certification requirements
of this project. The Permittee shall inform the District in writing of the name, address and phone
number of the professional engineer so employed. This information shall be submitted prior to
construction.
5. Within 30 days after completion of construction of the permitted activity, the Permittee shall submit
to the Brooksville Service Office a written statement of completion and certification by a registered
professional engineer or other appropriate individual as authorized by law, utilizing the required
Statement of Completion and Request for Transfer to Operation Entity form identified in Chapter
40D-1, FAC., and signed, dated, and sealed as-built drawings. The as-built drawings shall identify
any deviations from the approved construction drawings.
6. The District reserves the right, upon prior notice to the Permittee, to conduct on-site research to
assess the pollutant removal efficiency of the surface water management system. The Permittee
.nay be required to cooperate in this regard by allowing on-site access by District representatives,
by allowing the installation and operation attesting and monitoring equipment, and by allowing other
assistance measures as needed on site.
7. For dry bottom retention systems, the retention areas shall become dry within 72 hours after a
rainfall event. If a retention area is regularly wet, this situation shall be deemed to be a violation
of this permit.
8. The operation and maintenance entity shall submit inspection reports in the form required by the
District, in accordance with the following schedule.
For systems utilizing retention or wet detention, the inspections shall be performed two (2) years
atter operation is authorized and every two (2) years thereafter.
9. If limestone bedrock is encountered during construction of the surface water management
system, the District must be notified and construction in the affected area shall cease.
10. The Permittee shall notify the District of any sinkhole development in the surface water
management system within 48 hours of discovery and must submit a detailed sinkhole
evaluation and repair plan for approval by the District within 30 days of discovery.
11. The District, upon prior notice to the Permittee, may conduct on-site inspections to assess the
effectiveness of the erosion control barriers and other measures employed to prevent violations
of state water quality standards and avoid downstream impacts. Such barriers or other
measures should control discharges, erosion, and sediment transport during construction and
thereafter. The District will also determine any potential env iron mental problems that may
develop as a result of leaving or removing the barriers and other measures during construction
or after construction of the project has been completed. The Permittee must prov ide any
remedial measures that are needed.
