lien Law -- Homeowner's Protection Guide" prepared by the Florida Depal.tment of Agriculture
and Consumer Affairs. If the applicant is someone other that the "owner", I cerify that I
have obtained a copy of the above described document and promise in good faith to deliver
it to the "owner" prior to commencement.

be done in compliance with all applicable laws regul aUng construction, zoning, and land
development.
Application is hereby made to obtaln a permlt to do work and installation as indicated. I
certify that no vwrk or installation has conunenced prior to issuance of a permit and that
all work vlill be performed to meet standards of all laws regulatlng construction, City
codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other governmental agencies Inay apply to
the intended work, and that it is my responsibility to identify what actions I must take to

A permit issued shall be construed to be a license to proceed with the work and not as
authority to vJolate, cancel, alter, or set aside any provisions of the technical codes,
nor allall issuance of a permlt prevent the Building Official from thereafter requiring a
correction of errors in plans, con~truction, or violations of any code. Every pa!mit
iSsUed lOha 11 become invalid unless the \-lark authorized by such permit is cOllunenced within
six Inonths of issuance, or if work authorized by the permi.t is suspellded or abandolled for a
period of sb\ months after the time the work is conunenced. One 90 day extension of tlme
lIlay be allowed for the permit with fee charge of $15.~O. The extensi.on shall be ~equested
ill wri ting to the Building Official. An approved inspection must be logged during each tdK

BOARD OF ADJUSTMENT
The Board of Adjustment meeting was held on Tuesday, April 8, 2003,
at 4:00 p.m. in the City Council Chambers of City Hall. Present
were members Mickey McPhee, Wallace Reeves, and Cecila Graham. Also
present was Bill Burgess, Building Official. Nell Petty and Gary
Hatrick were absent.
Meeting was called to order by Chairman Reeves. Roll call was
taken, followed by the Pledge of Allegiance.
Motion was made by Cecila Graham for approval of minutes from
November 12, 2002 meeting. Seconded by Mickey McPhee and passed
unanimously.
Item 1 - Leroy Willmirth, 5745 Yorkshire Drive, variance request of
six foot for rear setback to build a screen room.
After brief discussion, motion was made by Mickey McPhee to approve
variance request. Seconded by Cecila Graham and motion passed
unanimously.
Meeting adjourned 4:13 p.m.

I hereby certify that I have reviewed the level and flat construction site and
the engineering plans and find that the existing 4" slab is sufficient for the
support and resistance of lateral as well as uplift loads resulting from a 110
mph wind acting on this screen room.
I therefore hereby certify that the existing 4" concrete slab may be used for
the attachement of the aluminum columns and base rails without
an added footer and that no footer is required for additional concrete to be
added. .
Basic wind speed = 11 0 mph

Exposure category=B
Internal pressure coefficient=O.O for open structure
