A permit issued shall be construed to be a license to proceed with the work and not as
authod ty to violate, cancel, alter, or set aside any provisions of tbe technical codee,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, oonstruction, or violations of any oode. Every permit
issued shall become invalid unless the Hork authorized by such permit is oOllunenced Hithin
six months of issuance, or if work authorized by the permit 1s suspended or abandolled t~r a
period of SJ:H months after the tillle the work is conunenced. One 90 day extension of time
may oe allowed for the permit with fee charge of $15.00. The extension shall be ~eques~ed
in writing to the Building Official. An approved inspection must be logged during each aix
month period, or the project Hill be considered abandoned.

THIS LEASE AGREEMENT made and entered into this 1st day ofMay, 2003 by
and between William F. Nye and Andrea R. Nye, 5306 Fox Hunt Drive, Wesley
Chapel, FL 33543, (813) 973-4256, hereinafter called the Landlord and Michael L.
Mann and ~erein after called the Tenants.
WHEREIN, IT IS ~~RSTOOD, STIPULATED, COVENANTED AND AGREED,
By and between the parties as follows:
1. DESCRIPTION OF PREMISES: Landlord hereby leases and demises unto
the tenant the premises known as Northside Plaza, 6305 Gall Blvd, Zephyrhills, Pasco County Florida
33541 being one of two rental wits located in the building, owned by William F. and Andrea R. Nye, the
Landlord, situated at 6305 GaD Blvd and containing approximately 1,200 square feet. The Tenant is also
granted the right in conjunction with other tenants to use the common parking areas, driveway,
hallway, sidewalks and means of ingress and egress subject to the limitations set forth
below. Tenant shall be subject to reasonable rules and regulations for use thereofas
prescribed from time to time by Landlord.
2. TERMS: The terms of this lease shall be for a period of One. Year,
commencing on the~ 0.~~03J~A ~ting on the 1st day of May 2004.
Lease y~fined. tfi~ term "traiJ year" as used in this Lease shall
mean a period of twelve (12) full consecutive calendar months. The fIrst lease year shall
begin on the date of commencement of the term hereof if the date of commencement of
the term hereof shall occur on the first day of a calendar month; if not, then the first lease
year shall commence upon the first day ofthe calendar month next following the date of
commencement upon the anniversary date of the fIrst lease year.
3. RENT: The tenant shall pay to the Landlord, a monthly rental of$I,OOO,
plus applicable state, and/or local sales tax, (now 6%). $1,060 due upon execution of
this lease, for the first month's rent, and N/A for the last month's rent, said monthly
rental being payable in advance. Said monthly rental shall commence of the first day
of the term of this lease as described in the preceding section with rent to be pro-
rated to the first of the month. Applicable state sales tax to be added to the base rental
amounts. Rent to be due and payable on the first day of each month. Any payments not
received by the fifth day of the month wiD accrue a penalty of 5% as liquidated
damages to cover the Landlord's additional charges. This will be added to the
rental amount due. Tenant agrees to pay this penalty if rented amounts are not
paid timely as per lease t~rms.
4. SECURITY DEPOSIT: On execution of this Agreement, Tenant has
deposited with the Landlord $1,000, receipt of which is hereby acknowledged by the
Landlord, as security for the faithful performance by the Tenant of the terms hereof.
Landlord shall hold said deposit without liability for interest. Any damages to the
property, beyond normal wear and tear, and to the equipment located thereon, shall be
