contractor(s} sign portions of the "Contractor SectionsH of this application for which they
will be responsible. If you, as the o",ner signs as the contractor, you are 1ndicating that
you, rather than the contractor, are responsible for the work, If the contractor ",ishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not ent1tled to permitting privileges 1n the City of Zephyrhills.

A permit issued shall be construed to be a 11cense to proceed w1 th the vwrk and not as
authority to violate, cancel, alter, or set aside any provi.sions of tlle technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code, Every permit
issued shall become invalid unless the work authorized by such permit is cOlllmenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is conunenced, One 90 day extension of time
may be allowed for the permit with fee charge of $15,00, . The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month per10d, or the project will be considered abandoned,

We have recently inspected the metal roof, at this location, 1 lis roof has been
recently installed and many of the techniques and details userj were incorrect. In
addition, the iIIat roof was not flashed into the metal roof proP':trly.
We propose the following:
1 . Contact local building department to resolve existing mating issues, pull
permit, and satisfy all inspection requirements.
2. Remove and replace all existing, sheet metal screws, ":hat are loose, with
new fasteners of similar type; with a larger head and ~I neoprene washer,
3. Tear-c)ff existing rear, flat roof system, down to the ex sting wood deck
and repair any rotted wood; at an additional cost of $1 ,50 per square foot
and $2.75 per board foclt for dimensional lumber,
4. Fabricate and install new edge metal around flat deck, this was not
previously installed,
5, Furnish and install a fibE~rglass base sheet nailed down to the wood deck
and a torch applied, mo,jified bitumen, roof system,
6, Re-work existing edge metal around the metal roof; this was incorrectly
installed, by the previous installer.
7. Install metal flashing around the dormer/eave, in the I'l:tar of the building,
this was not previously installed.
8, Fabricate and install pit(:h-transition flashing, on front ,)f building, this was
not previously installed. .
9, Re-work rake and ridge flashings that were previously installed,
incorrectly,
10, Caulk all penetrations and seams to insure a watertight installation,
11. Dispose of all roofing related debris from jobsite, and e:lispose of all in a
legal manner,
