may be more restrictive than City regulations. The undersigned assumes responsibility for
compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to undertake work, they may be required
to be licensed in accordance with state and local regulations. If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law. If the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work, they are advised to contact the
City of Zephyrhills Building Department, 813-780-0020.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contractor(s) sign portions of the UContractor Sections" of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

Application is hereby made to obtain a permit to do work and installation as indicated. I
certify that no work or installation has commenced prior to issuance of a permit and that
all work will be performed to meet standards of all laws regulating construction, City
codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other governmental agencies may apply to
the intended work, and that it is my responsibility to identify what actions I must take to

I also certify that, if fill material is to be used in Flood Zone "A" or uA,etc.", it is
understood that a drainage plan addressing a ucompensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida prior to permit
issuance.
A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code. Every permit
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is commenced. One 90 day extension of time
may be allowed for the permit with fee charge of $15.00. The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month period, or the project will be considered abandoned.

Addition of a Concrete Handicap Ramp to the Northwest entrance to the church sanctuary
(faces 10th Street).
Project Specifications:
Ramp to be 12 feet long since the existing step is 11.75" high.
Ramp to be 4'8" wide.
4" concrete footer to be underneath the ramp, part of which is the existing landing.
After the ramp there will be a concrete landing 60" beyond the edge of the ramp.
The concrete landing will be 10'9" wide to allow for the walkway to the step side the entrance.
Continuous handrails to be placed on both sides of the ramp the entire length, 12'. Distance
between railings to be 44". Handrails height to be 36".

11-4.7.11 Islands. Any raisd:i islands in crossings shall
be cut through level with the ptreet or have curb ramps at
both sides and a level area at lpast 48 inch (l220 mm) long
between the curb ramps in th~ Dart of the island intersect-
ed by the crossings (See Figu~ ]5(a) and (b)).
11-4.8 Ramps.
11-4.8.1 General. Any part of an accessible mute with a
slope greater than 1;20 shall be considered a ::-amp and
shall comply with 11-4.8.
11-4.3.2 Slope and Rise. The least possible slope shall be
used for any ramp. The maximum slope of a ramp in new
construction shall be 1 to 12. The maximum rise for any
run shall be 30 inches (760 mm) (See Figure 16). Curb
ramps and ramps to be constructed on existing sites or in
existing buildings or facilities may have slopes and rises
as shown as allowed in 11-4.1.6(3)(a) iI- space limitations
prohibit the use of a 1 to 12 slope or less (See 11-4.1.6).
11-4.8.3 Clear Width. The minimum dear width of a

:n -4.8.4 Landings. Ramps shall have level landings at
bottom and top of each ramp and each ramp run. Landings
shall have the following features:
(1) The landing shall be at least as wide as the ramp run
leading to it.
(3) If ramps change direction at landings, the minimum
landing size shall be 60 inches by 60 inches (1525 mm
by 1525 mm).
(4) If a doorway is iocated at a landing, then the area in
front of the doorway shall compi)' with 11413.6.
11-4.8.5 Handrails. If a ramp run aas a rise greater than
6 inches 1150 mm) or a horizontal proJection greater than
7: inches i 1830 mm). then it shall have nanarails on :;,oth

sides. Handrails are not required on curb ramps or adja-
cem to seating in assembiy areas. Handrails shal! comply
with 11-4.26 and shal1 nave the following features:

(3) The dear space between the handrail and the wall
shall be 11/: inches (38 mm).
(4) Gripping surfaces shall be continuous.
(5) Top of handrail gripping surfaces shall be mounted
between 34 inches and 38 inches (865 mm and 965
mm) above ramp surfaces.
(6) Ends or handrails shall be either rounded or
returned smoothly to floor, walL or post.
(7) Handrails shall not rotate within their fittings.

11-4.8.6 Cross Slope and Surfaces. The cross slope of
ramp surfaces shall be no greater than 1:50. Ramp sur-
races shall comply with 11-4-5-
H-4.8.7 Edge Protection. Ramps and landings with
drop-offs shall have curbs, walls, railings, or projecting
surfaces that prevent people from slipping off the ramp.
Curbs shall be a minimum of 2 inches (50 rom) high (See
Figure 17).
11-4.8.8 Outdoor Conditions. Outdoor ramps and their
approaches shall be designed so that water will not accu-
mulate on walking surfaces.
11-4.9 Stairs.
H-4.9.Jl Minimum Nu~ber. Stairs required to be acces-
sible by 11-4.1 shall contPly with i 1-4.9.

11-4.9.2 Treads and RiJrs. fJn any given flight of stairs,
all steps shall have uniiPrm riser heights and uniform
tread widths. Stair treads.\'Shall be no less than 11 inches
(280 mm) wide, measure from riser to ;-iser (See Figure
18(a). Open risers are not ermitted.
11-4.9.3 Nosings. The un~ersides of nosing shal! not be
abrupt. The radius of curv~ure at the leading edge of the
tread shall be no greater than 112 inch 03 mm). Risers
shall be sloped or the underside of the nosing shall have
an angle not less than 60 degrees from the horizontaL

11-4.9.4 Handrails. Stairways shall have handrails at
both sides of all stairs. Handrails shall comply with 11-
4.16 and shall have the fOll~wing features: .-

(1) Handrails shall be c~ntinuous along both sides or
stairs. The inside handrail/on switchback or dogleg stairs
shall always be continuo!ts (See Figure 19(a) and (b)).

least 12 inches (305 rrub) plus the width of one tread

11-4.9.6 Outdoor Conditions. Outdoor stairs and their
approaches shall be/designed so that water will not accu-
mulate on walking surfaces.
11-4.10 ELEVATORS.
11-4.10.1 General. Accessible elevators shall be on an
accessible route and shall comply with 11-4.10 and with
the ASME AI7.1-11990, Safety Code for Elevators and
Escalators. Freight elevators shall not be considered as
meeting the requirements of this section unless the only
elevators provided;are used as combination passenger and
freight elevators for the public and employees.
11-4.10.2 Automatic Operation. Elevator operation
shall be automatic. Each car shall be equipped with a self-
leveling feature that will automatically bring the car to
floor landings within a tolerance of 112 inch (13 mm)
under rated loading to zero loading conditions. This self-
leveling feature shaH be automatic and independent or the
operating device and shall correct the overtravei or under-
traveL
H-4.10.3 Hall Call Buttons. Call buttons in elevator
lobbies and hails shall be centered at 42 inches (1065 mm)
