A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this permit may be subject to "deed restrictions" which
may be more restrictive than City regulations. The undersigned assumes responsibility for
compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to undertake work, they may be required
to be licensed in accordance with state and local regulations. If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law. If the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work, they are advised to contact the
City of Zephyrhills Building Department, 813-780-0020.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contractor(s) sign portions of the "Contractor Sections" of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

I certify that I, the applicant, have been provided with a copy of "Florida's Construction
lien Law - Homeowner's Protection Guide" prepared by the Florida Department of Agriculture
and Consumer Affairs. If the applicant is someone other that the "owner", I cerify that I
have obtained a copy of the above described document and promise in good faith to deliver
it to the "owner" prior to commencement.
E. CONTRACTOR'S/OWNER'S AFFIDAVIT
I certify that all the information in this application is accurate and that all work will
be done in compliance with all applicable laws regulating construction, zoning, and land
development.
Application is hereby made to obtain a permit to do work and installation as indicated. I
certify that no work or installation has commenced prior to issuance of a permit and that
all work will be performed to meet standards of all laws regulating construction, City
codes, zoning regulations, and land development regulations in the jurisdictl.on. I also
certify that I understand that the regulations of other governmental agencies may apply to
the intended work, and that it is my responsibility to identify what actions I must take to
be in compliance. Such agencies include but are not limited to: *Department of
Environmental Regulation-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment
*Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas,
Altering Watercourses
*Army Corps of Engineers-Seawalls, Docks, Navigable Waterways
*Department of Health & Rehabilitative Services, Environmental Health Unit-Wells,
Wastewater Treatment, Septic Tanks
*U.S. Environmental Protection Agency-Asbestos abatement
I also certify that, if fill material is to be used in Flood Zone "A" or "A, etc.", it is
understood that a drainage plan addressing a "compensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida prior to permit
issuance.
A permit issved shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code. Every permit
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for a
period of six months after the time the work is commenced. One 90 day extension of time
may be allowed for the permit with fee charge of $15.00. The extension shall be requested
in writing to the Building Official. An approved inspection must be logged d'Jring each six
month period, or the project will be considered abandoned.
WARNING TO OWNER: YOUR FAILURE TO RECORD A NOTICE OF COMMENCEMENT MAY RESULT IN YOUR
PAYING TWICE FOR IMPROVEMENTS TO YOUR PROPERTY. IF YOU INTEND TO OBTAIN FINANCING, CONSULT
WITH YOUR LENDER OR AN ATTORNEY BEFORE RECORDING YOUR NOTICE OF COMMENCEMENT. JOBS UNDER

A study has been conducted under the authority of sections 307(A) and
313 (A) of the Federal Aviation Act to determine'whether the existing
structure would be an obstruction to air navigation, and/or whether
it should be marked and lighted to enhance safety in air navigation.
The findings of that study are as follows:

FIRST AMENDMENT TO
SITE LEASE AGREEMENT
This First Amendment (the "Amendment") is attached to and made a part of the Site Lease Agreement (the
"Agreement") dated April 14, 1998, by and between Nextel South Corp., a Georgia corporation, d/b/a NEXTEL
COMMUNICA nONS (herein called "Lessee") and GTE WIRELESS OF THE SOUTII INCORPORATED, a Virginia
corporation (herein called"Lessor").
WIT N E SSE T H:
WHEREAS, Lessee desires to modify its Lessee Facilities on the Premises by replacing its antennas on the
Tower, as more particularly described in the Revised Exhibit E attached hereto; and
WHEREAS, the parties now desire to modify and amend the Agreement;
NOW, THEREFORE, in consideration of the execution of this First Amendment, and the mutual promises,
terms, covenants and conditions contained herein, the parties hereto do hereby agree as follows:
1. The foregoing recitations are true and correct and are hereby incorporated by reference.
2. That Exhibit E of the Agreement is deleted in its entirety and replaced with the attached
Revised Exhibit E.
3. It is the intention of the parties hereto, that in the event of any conflict between the terms of the
Agreement and this First Amendment, this First Amendment shall prevail and any conflicting language,
terms or provisions in the Agreement shall be inoperative. Capitalized terms in this First Amendment
shall have the same meaning as are given in the Agreement, unless otherwise defined in this First
Amendment. Except as set forth herein, all provisions of the Agreement remain unchanged and in full
force and effect.
IN WITNESS WHEREOF, the parties have executed this First Amendment to the Agreement on the date and year
first above written.

This Second Amendment ("Amendment") is made by and between Pinnacle Towers LLC, successor in interest to Pinnacle Towers Inc. by
State of Delaware conversion ("Lessor"), and Nextel South Corp.("Lessee"), and shall modify certain terms of Communications Site Sublease
Agreement dated February 18, 1998, by and between Primesite Consulting Group, Inc and Nextel South Corp., as previously amended by
First Amendment dated August 12, 2005 by and between Pinnacle Towers LLC and Nextel South Corp. (the Site Schedule and all
amendments and amendments SUbsequently executed thereto are collectively, the .Schedule").
WHEREAS, Pinnacle Towers LLC is the assignee of Primesite Consulting Group, Inc., by way of assignment dated April 28, 1998; and
WHEREAS, The parties hereto wish to modify certain terms of the Schedule, and now therefore, for good and valuable consideration the
receipt and sufficiency is hereby acknowledged the parties agree:
1. The Effective Date of this Amendment shall be November 1, 2005, prior to which the terms and provisions of this Amendment shall
be inoperative.
2. Rent shall be increased by Five Hundred Dollars and 00/100 Dollars ($500.00) per month, beginning on the Effective Date, and
otherwise payable in accordance with the Schedule, including any escalation provisions set forth therein.
3. Lessor hereby permits additional equipment, ground space, frequencies, or other changes as more specifically set forth on attached
Exhibit "A," which is incorporated herein by reference.
4. Paragraph 19 of the Communications Site Sublease Agreement dated February 18, 1998, Miscellaneous, is amended to reflect the
current notice address of Lessee:

This First Amendment to Site Lease Agreement (this "Amendment") is made this /7tIJ day
of ...7uAltE': ,2005, by and between Crown Castle GT Company LLC, a DelaWare
limited liability company ("Crown"), and Nextel South Corp., a Georgia corporation, d/b/a Nextel
Communications ("User").
WHEREAS, on April 14, 1998, GTE Wireless of the South Incorporated, a Virginia corporation, its
successors and assigns ("GTE") and User entered into a Site Lease Agreement (the "Site Agreement")
whereby User leased from GTE certain space at a telecommunications facility located at 702 Avenue B,
Zephyr Hills, Florida 33540 (the" Tower Facilities''); and,
WHEREAS, GTE granted all of its right, title and interest in the Site Agreement to Crown via a Special
Warranty Deed recorded JlD1e 14,2001, with the Pasco County, Florida Clerk, in Book 4639, Page 280
through Page 282; and,
WHEREAS, Crown and User desire to amend the Site Agreement in order to modify User's centerline,
antennas and ground space, to identify additional sets of frequencies that User will utilize to transmit and
receive, and to adjust the Rent as set forth herein. Additionally, the Site Agreement is amended to clarify
that all references to the site street address should read 702 Avenue B. All instances where a different
address is referenced are hereby amended.
NOW, THEREFORE, for good and valuable consideration, the receipt and sufficiency of which is hereby
acknowledged, the parties hereto agree to be legally boWld to this Amendment as follows:
I. The Site Agreement, including and without limitation to Paragraph 2(a) thereof and Exhibit "B"
and Exhibit "E" thereto, is hereby amended by removing User's existing antennas within its
Tower Space at the Two HWldred Fourteen feet (214') centerline level, and adding those antennas
and equipment consisting of Twelve (12) Decibel / DB878G90A-XY antennas and Twelve (12)
lines of Andrew LDF7-S0A / 1-5/8" coaxial cable (the "Replacement Equipment") at a new One
HWldred Twenty feet (120') centerline level, as fiuther identified on the Engineering Application
attached hereto as Exhibit ''B-1'' and incorporated herein by reference. All of the Replacement
Equipment is hereby and shall be considered part of the Communications Facility for all purposes
under the Site Agreement. All references to Exhibit "B" and Exhibit "E" in the Site Agreement
shall hereinafter mean and refer to Exhibit "B" and Exhibit "E" as amended herein by the
inclusion therein of said Replacement Equipment set forth herein, and in Exhibit "B-1" and
Exhibit "E-l ", which are attached hereto and incorporated herein.

I CORPORA TED, a Virginia Corporation, its successors and assigns (hereinafter referred to as "GTE")
and NEXTEL SOUTH CORP., a Georgia corporation, d/b/a NEXTEL COMMUNICATIONS
(hereinafter referred to as "User").
WHEREAS, the parties are party to the Master Site Agreement dated May 28, 1997 as amended by
that certain Amendment dated as of September 17, 1997 (collectively the "MSA");
WHEREAS, the parties desire that except as set forth in this Site Agreement, the terms and
conditions of the MSA shall govern the relationship of the parties under this Site Agreement;
WHEREAS, GTE Wireless of the South Incorporated, as successor-in-interest to GTE MobiInet of
Tampa Incorporated, is the owner of certain real property located in Pasco County, Florida consisting of
approximately 0.455 acres, as more particularly described on Exhibit "A" attached hereto and incorporated
herein by reference.
NOW, THEREFORE, for valuable consideration, the receipt, adequacy and sufficiency of which is
hereby acknowledged by the parties hereto, the parties hereby agree as follows:
I. MSA and Defined Terms. Unless otherwise defined herein, capitalized terms shall have
the meaning set forth in the MSA. The parties agree that except as otherwise set forth herein, the terms and
conditions of the MSA shall govern the relationship of the parties under this Site Agreement and the MSA is
incorporated herein by reference. In the event of a conflict or inconsistency between the terms of the MSA
and this Site Agreement, the terms of this Site Agreement shall govern and control. ~:T
2. Demise. GTE hereby leases to User and User hereby leases from GTE the following:

(a) Tower SDace. Tower space on GTE's Tower between the two hundred fourteen foot
(214') and two hundred twenty two foot (222') level (at two hundred eighteen foot (*4') centerline), as more
particularly described on Exhibit "B" attached hereto and incorporated by this reference (the "Tower Space"),
for the placement of User's antenna array, platfonn, cables, brackets, wires and accessories, as more particularly
described on Exhibit "E" attached hereto and incorporated herein by reference (the "Tower Facilities"); and
(b) Ground SDace. Ground space containing two hundred sixteen (216') square feet, as
approximately shown on Exhibit "C" attached hereto and incorporated herein by this reference and which will
be more specifically shown on the As-Built Survey, as defined in Section II (vii) of the MSA, delivered by User
to GTE in accordance with Section II (vii) of the MSA and which As-Built Survey shall be attached to and
become a part of this Site Agreement as Exhibit "D" when initialed by GTE and User in accordance with
Section II (vii) of the MSA (the "Ground Space"), for the placement of equipment shelters and cabinets,
telecommunications equipment within such equipment shelters and cabinets, concrete pads, generators, cables,
wires and accessories, as more particularly described on Exhibit "F" attached hereto and incorporated herein by
reference (the "Ground Facilities"); together with
