NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit may be subject to "deed" restrictions"
which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance With any
applicable deed restrictions. . . .
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply tor the
intended work, they are advised to contact the Pasco County Building Inspection Division-Licensing Section at 727-847-
8009, Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which they will be responsible. If you, as the oWner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County,
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned Imderstands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the constrUction of new buildings, change of
use in existing buildings, .or expansion of existing buildings. as specified in Pasco County Ordinance humber 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): if valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law-Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that 1 have obtained a copy of the above described document and promise in good faith to
deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards. of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
Department of Environmental Protection-Cypress Bayheads, Wetland. Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment.
Southwest Florida Water Management District-Wells, Cypress Bayheadsj Wetland Areas, Altering
Watercourses.
Army Corps of Engineers-Seawalls, Docks, Navigable Waterways.
Department of Health & Rehabilitative Services/Environmental Health Unit-Wells, Wastewater Treatment,
Septic Tanks.
US Environmental Protection Agency-Asbestos abatement.
Federal Aviation Authority-Runways.
I understand that the following restrictions apply to the use of fill:
Use of fill is not allowed in Flood Zone "V" unless expressly permitted,
If the fill material is to be used in Flood Zone "N, it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a profeSSional engineer
licensed by the State of Florida. .
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem Wall.
If fill material is to be used in any area, I certify that Use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically Included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter
reqUiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if Work authorized by
the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the extension, If work ceases for ninety (90) consecutive days, the job is considered abandoned.

management system facilities. If the project includes 0 wetland mitigation
area, os defined In section 1.7,24. or 0 wet detentlan pond, no
vegetation in these areas ,hall be removed, cut. trimmed or sprayed with
herbicide without specific written approval from the District. Construction
and maintenance activities which ore cansistent with the design and
permit conditions approved by the District in the Environmental ResOl.lrce

SANITARY GRAVITY SEWERS
C.I.I GENERAL:
C.1.3.5 Pipe Depth and Protection: The minimum allowable cover for gravity sewers shall be three feet from the top of the
pipe to finish grade. However, should this depth not be feasible, where grade depressions along the alignment ore
unavoidable, protective concrete slobs sholl be provided over the pipe and/or ductile Iron pipe sholl be Installed within the
limits of the lesser cover. In no case sholl pipe cover be less than 1e inches, unless special design considerations hove been
approved by the Engineer. Where
I'\Ovigab!e waterwa~ ore crossed, ductile Iron pipe sholl be Installed aCfOU Clnd to ten feet each sIde of the bottom.
Additiol'\OlIy, approved utility crossing signs sholl be placed on the pipe alignment at each side of the waterway. Sign sholl be

C.I.3.6 Pipe Bedding: Special core sholl be exercised in design and Installation to provide adequate bedding for the t)'pe of
pipe used, taking into consideration trench width and depth, superimposed loodings obove grode, and the material below
trench grade. Pipe loading capabilities sholl be computed In accordance with established design criteria and special supporting
bedding or facilities sholl be provided os required by the Engineer (see Section 8.1.3),
C.I.3.7 Special Exterior Protection for Corrosion: Extra protection sholl be provided for underground cost or ductile iron pipe
and fittings within areas of severe corrosive conditions. This sholl be accomplished by the installation of polyethylene
encasement, as specified in Section 8.7, through the area of concern. The soli test evaluation to determine the necessity for
extra protection in suspect areas sholl be os set forth In ANSI Standard A21.5. Additionally, where other existing utilities ore
known to be
cathodically protected, cost or ductile iron pipe crossing said utility shall be protected for 0 distance of 20 feet to each
side: and when installed parallel to and within ten feet of some, protection sholl also be pro\lided.
C.I.J.8 Connections at Structures: Where sanitary sewers connect to structures, pipe joints sholl be provided at the wall face.
When It is necessary to extend sewers through structures, such os conflicting elevation storm drain b)'passing chambers, the
pipe within sholl be ductile iron with no inside joints. '"Link-Seal" or equal sholl be used at 011 wet well penetrations.
C.I.3.9 Transition Connections: Where pipes of alternate materials (Ve to CI. etc.) ore to be connected between manholes,
suitable approved transition couplings sholl be installed, Couplings sholl be '"C- T Adopters'" os manufactured by Can- Tex
Industries, Minerai Wells, Texas, or approved equal. Special designed units may be submitted for approval; however, concrete
collars ore not acceptable.
C.I.3.10 Pipe Cutting: The cutting of ductile iron pipe for installation length adjJstment, or connections for future services to
existing sewers, sholl be in strict compliance with the methods specified In AWWA C600.
C.1.3." Service Connections: Service laterals will not empty directly into manholes, Installation sholl be os shown on Detail
Sheet .Service Connection Details'"; including the wye branches installed in the sewer main at the point of connections. and
the service pipe and required fittings extend to the property line, perpendicular to said line, terminating with stoppered ends
or fittings, os indicated. The minimum service pipe size
sholl be four inches in diameter for a single service and six inches tn diameter for 0 double service. On curbed streets, the
exact location for each installed service sholl be marked by pointing a two- inch by four-inch green stripe on the concrete
curb. Point sholl be of a t)'pe recommended for exterior concrete, Where no curb exists or is planned, locations sholl be
adequately marked by a method approved by the DCUD. The end of the service lateral sholl be marked with a two-inch by
four-inch wooden stake or 0 two-
inch PVC stoke extending three feet aboveground and pointed green. Service lines to building sholl conform to the Pasco
County Plumbing Code.
C.I.4 T!STlNG:
C.LI.I This section includes general technical criteria for the design and installation of sanitary gravity sewer systems.
C.i.1.2 The rele\lOnt provisions included in DivIsion e, "TechnIcal Requirements - General'", and shown on .Standard Details -
Sanitary Seweroge Systems'" and .Standard Location Plan - Underground Utilities'" shall be applicable to this section, unless
otherwise indicated herein or changed in writing by the Director.
C.I,2 DESIGN STANDARDS:
C.1.2.1 required Reference: The Contractor sholl comply with the applicable requirements 9pecified within the Pasco County,
florida, Land Development Code and os established by the mER, Rule 17-6, F,1,C. The criteria set forth in Chapter 20,
"Design of Sewers'", of the "Ten State Standards - Recommended Standards for Sewage Works" sholl be used as 0 design
guide. Conflicts will be brought to the attention of the Director.
C.I.2.2 S~tem Design:
C.1.2.2.1 Average Design Flows: The sewer system design
sholl be based on full ultimate development os shown in the following table:

The overage doily flow (ADF) from domestic units sholl be
calculated at the minimum rate of 75 gallons per capita per day, which will
normally cO\ler infiltration; but should conditioos be unfavorable, on
additionol allowance sholl be included. Flow requirements from commercial,
industrial, institutional. or other special development areas shall be
estoblished from existing records, or by estimated projections using the best
available data; however, in no case sholl a rote of less than 1,000 gallons
per acre per day be used, unless specifically approved otherwise. The progressive summation of the previously described
contributions sholl constitute the ADF for specific sections of the sewer system.
C.I,2.2.2 Peak Design flows: Gravity sewers sholl be designed on the basis of ultimate development maximum rates of flow,
which will be the product of selected peak factors times the accumulative Maximum Monthly Average Daily flow (MMADF) for
the subject portion. In general, the following peak factors sholl be applicable for the range of average doily flow rates
indicated (million gallons per day- mgd), unless lorger values ore required for speCific conditions or prior approval is
received from the Engineer for modification thereof.

C.IA.1 The Contractor sholl perform testing of 011 sanitary gravity sewers, os set forth in the following. and sholl conduct
sold tests in the presence of representatives from the DCUD with two days' advance notice provided.
C.I.4.2 The installed gravity sewer9 shall be "\amped'" between manholes or other structures in order to ascertain that they
ore crear and in correct alignment, The concentricity of the lamp image received sholl be such that the diameter of said
image sholl hove no vertical reduction from that of the pipe inside diameter and not more than 20 percent horizontal
reduction. Testing sholl not proceed until all facilities ore complete in ploce and concrete cured. All piping shall be
thoroughly cleaned prior to testing to clear the lines of 011 foreign motter, Lines sholl be flushed with water prior to lamping
to aid in the detection of dips in the line. Lines which have dips which retain greater than one inch of water may be
sub ject to repair at the discretion of the DCUD,
C.I.4.3 TelevisIon Inspectlon: New gravity sewer mains sholl be television inspected by the contractor upon request of the
DCUD, A television inspection may also be requested prior to the final inspection by the DCUD. Tapes sholl be furnished on a
VIiS format, in color, and of suitable clarity and quality for good definition, The intent is to require television Inspection of
lines which appear to be defectIve or otherwise unsatisfactory.
C.I.4.4 Deflection Testing: New gravity sewer mains ore subject to deflection testing at the discretion of the DCUD. Tests will
be conducted using a 92.5 percent mandrel. The mandrel sholl be pulled through the sewer line by hand, using a 150-pound
test line. If the mandrel cannot traverse the pipe as such, then the deficiency must be corrected. The intent is to require
testing of lines which appear to be defective or otherwise unsatisfactory.
C.I.4.5 Smoke Testing: New gravity sewer mains may be smoke tested by the DCUD at the discretion of the DCUD, Any
defects detected os a result of the testing sholl be promptly repaired by the Contractor.
SECnON C.2 SANITARY SEWER FORCE I.4AINS
C.2.1 GENERAL:
C.1.2.2.3 Sewer Size Computation: Sanitary sewers shall
be sized to provide ample copocity for the required peak flow rates. The
minimum allowable size for any sewer, other than service connections, sholl be
eight inches in diameter. All sewers sholl be designed at slopes providing
minimum velocities of not less than two feet per second when flowing futl or
half full. Said computation shall be based on Kutter's formula, using a
roughness coefficient ("N") of not less than 0.013, unless jJstifiably
appro\led otherwise. In general, the following minimum slopes sholl be
provided for sewer sizes to 24 inches:

C.2.1.1 This section includes the general requirements for design and installation

the depth of flow will not be less than 0.3 of the pipe diameter or the veloc-

C.I.2.2.4 Design Considerations:
C. 2.2 DESIGN STANDARDS:
(0) Sewers 24 Inches in diameter or less shall be installed with straight alignment and grade between manholes, with
manhole spacing not to exceed 300 feet.
(b)AII sanitary sewers sholl terminate at manholes.
(c) Sewers of diverse sizes sholl always join at manholes. with no size conversions between. Where different siZe!! join, the
pipes sholl be placed at elevations where the depth points ore equal, unless h~her points ore required. If the entrance pipe
elevation exceeds two feet above the effluent sewer. drop manhole connections sholl be provided.
(d) flow direction changes in excess of 90 degrees within manholes sholl not be permitted. When directional changes exceed
45 degrees, on extra flow line elevation drop (0.1'+-) across manholes sholl bt provided.
(e) Where design velocities greater than 15 feet per second ore attained, due to topography or other reasoos, special
equipment sholl be provided for sewer protection, os approved by the Director.
(f) Minimum size of gravity main inches; minimum slope for an eight- inch man is 0.40 percent.
C.2.2.1 Required Reference: The Contractor sholl comply with the applicable design and installation requirements os specified
within the Pasco County Land Development Code, and os established by the FDER, Rule 11-6, FAC.
C.2.2.2 System Design: force main systems sholl be no leu than six inches In diameter (unless specifically approved by the
OCUD on an individual basis) and of adequate size to efficiently transmit the tatol ultimate p~k operotlonal flows, applled
by the ~onnecte~ sewage pumping station(s),. .to the point a! djscho~ge, Consideration sholl be given to possible future
connecting pumping stations and this probability shall be reViewed With the DCUO. Capacity computations sholl be coordinated
with the proposed pumping systems(s), along with any future flow requirements, if applicable. In order to provide adequate
pipeline cleansing, force main flow velocity sholl not be less than two feet per second at ultimate design minimum pumping
copacity. however, with multiple pumping station s~tems or phose development, this requirement may not be possible and
the system design sholl receive special attention regarding cleaning maintenance.
C.2.2.3 Operational Cost Considerations: for T)'pe II and T)'pe III lift stations (see Section C.3, Paragraph C.3.4), in addition
to initial capital expenditure, long-term pumping station operatiooal costs sholl also receive consideration when sizing force
main systems. Should a pipe size option be available within the design limits, the cost of sewage pumps and motors, force
main system, and pump operating power (computed for design overage doily flow rote for ten years at existing electricity
cost) sholl be compared to
like amount9 for the alternate designs. The final force moin size selection sholl be directed tOwards the system with the
I~st long-range capital and operational cost. Said cost analysis shall be subject to review by the DCUD.

C.2.3.4 Pipe Depth and Protection: The standard minimum cover for sewer force main systems sholl be three feet from the
top of the pipe to finish grade. However, should this design not be feasible, protective concretu slobs shoJI be provided aver
the pipe and/or ductile iron pipe Installed within the limits of the lesser cover, Where waterway', canals, ditches, or other
cuts are crossed, ductile iron pipe sholl be required across and to ten feet each side of the b~ttom. Additionally. al>proved
utility crossing signs shall be placed on the pipe alignment at each side of the conal, etc.. for
navigable waterways.
C.2.J.5 Connections and Structures: Where pipes are to extend into or through structures, f1ex!.>le joints shoJI be provided at
the wall face. The pipe within the structure will be ductile iron with no inside joints.
C.2.3.6 Special Exterior Pratection for Corrosion: Extra protection sholl be provided for undergrc.und ductile iron pipe and
fittings within areas of severe corrosive conditions. This snail be accomplished by the installatio" af polyeth~ene encJsement,
per AWWA C109, os specified in Section B.3, through the area of coocern, The soil test evaluation to determine the
necessity for extra protection in suspect areas sholl be as set forth in ANSI Standard A21.5. Additionally, where oth!r
existing utilities ore known to be cathodically protected, cost or ductile iron pipe crossing said utility shall be protected for a
distance of 20 feet to each side; and when installed parallel to and within ten feet of some, ~rotection sholl also be
provided.
C.2.3.7 Air and Vacuum Venting: Where the force main profile is such that air pockets or entr&pment could occur, r:esulting
In flow blockage, provisions for air release sholl be provided. Manually operated vent valves sholl be provided along minor
farce mains where continual problems ore not anticipated. Automatic air release assemblies sholl be installed where venting
is required on 011 major force mains and at critical points on lesser mains. At profile break points on major force mains.
such as tops of hills,
etc., where free flow will occur during operation or after pumping stops, combined air release and vacuum valve assemblies
may be required, Air and vacuum \IOlves and/or air release valves sholl be os specified under Section e.3 and sholl be
suitably housed In a properly vented underground chamber (manhole). All Interior surfaces of the structure and the valves
are required to be coated with cool tor epoxy.
C.I.J.3.J Manhole frames and covers sholl be grey cost iron or ductile iron heavy-duty conforming to ASTM DesIgnation MB,
Closs 30, and shall be U.S. foundry and Manufacturing Corp. , or approved equal. Covers sholl hove no perforations and sholl
be marked with the word .SANITARY". Sewer frames and covers sholl be fully bedded in mortar to the correct finish grade
elevation, with adjJstment brick courses placed below. as detailed, for
precast manholes. frames sholl be suitable for the future addition of cost iron rings for upward adjJstment of top ele\lOtion.
All frames and covers sholl meet HS20-44 load requirements.
C.l.3.3.4 Manhole flow channels sholl be os shown on the .Standard Detail Sheets'" with smooth and carefully shaped
bottoms, built-up sides and benching constructed from concrete. Channels shdl conform to the dimension of the adjacent
pipe and provide changes in size, grode, and alignment evenly.
C.I.J.3.S The Interior surfaces of 011 manholes shall be protected by the opplication of two coots of cool tor epoxy applied at
the rote of 120 sq. ft. per galfon per coot (minimum). This Includes walls, chimneys, floors. benches, invert channels, lids,
and 011 joints after grouting. Exterior surfaces shall receive two coots of block sealant for
moisture protection, applied at 0 minimum rote of 375 sq. ft. per golf on per coot. Surface preparation and point application
sholl comply with the manufacturer's recommendations.
C.1.3.3.6 Where additional pipe connections or modification of existing factory-made openings ore required on new or existing
precast concrete manholes, all cutting relative thereto sholl be performed only by abrasive wheel sow, It is specificalfy noted
that such connections to existing manholes sholl be instalfed in accordance with the details for new units shown on
.Stondord Detail Sheets'" and sholl be sleeved and caulked watertight
with nonshrinking grout.
C.1.3.3.7 Manholes sholl be designed and constructed within rood rights-of-way and sholl be specifically located either in the
pavement or within four feet of the edge of curb. In no case will manholes be located within backyard easements or any
location not readily accessible to maintenance vehicles and equipment.

other) at a pOint equal to or below the operational water level of said receiving unit.
C.2.3.1 0.1 Discharge Into Manholes; force mains entering gravity manholes sholl discharge at Invert level. A properly formed
and directed channel sholl be provided to ensure 0 smooth transition of flow.
C.2.3.10.2 Discharge Into Pumping Stotion Wet Wells: force mains entering wet wells sholl be provided with a top vented drop

provided.
C.2.4.2 Piping and appurtenances to be tested sholl be within sections between \I01"es or adequate plugs with prior approval
from the Engi0e~r, Testing sholl not proceed until concrete thrust blocks are in place and cured, or oth~r restraining devices
