TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and 90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances. CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I certify that I, the applicant, have been provided with a copy of the "Florida Construction lien Law-Homeowner's Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to deliver it to the "owner" prior to commencement. Linebaugh Avenue Tampa, FL 33612 Phone: 813-962-4392 Fax: 813-962-4654 Registered Professional Engineer #21780 HUD Compliance Inspector #M034 19 April 2007 ENGINEER'S INSPECTION REPORT Address: 37630 Gill Avenue Zephyrhills, FL 33541 We were retained to inspect the foundation system of a new manufactured home for structural integrity in relationship to newly placed and compacted fill, and make any recommendations for possible remedylrepair. FINDINGS AND RECOMMENDATIONS The home is a new double wide manufactured home installed on a lot that has a drop in grade from the front to the rear with a marked decline at the very rear. The drop in grade precipitated the installation of fresh, compacted fill to level the area under the home, but the rear of the home sits right at the edge of the rapid decline. Recent rain has washed a portion of the fresh fill away slightly undermining at least one pier pad. While the area immediately under the home at the time of our inspection was providing a structurally sound base for the home, the grading at the rear, and the ability of rain water to flow under the home, causes us to doubt this stable condition will last without some additional corrective measures: 1. Water must not be allowed to flow under the home. Other homes in the neighborhood have mortared brick skirting which is traditionally installed on a B~ ~ concrete footer around the entire perimeter of the home; if this (or something similar) is used, that would suffice; if not, the grading will have to corrected to divert the water flow away from, and around, the home. 2. Grading at the rear of the home must be stabilized either by adding more fill to decrease the angle of decline around the immediate vicinity of the home, lj<f-ft or by building a retaining wall to hold the fill in place now (even this approach will require additional fill). In both cases appropriate sod should be placed to help prevent erosion of the fill. 3.
