The grout program was performed in an attempt to fill existing subsurface void space that may lead to future ground subsidence beneath the referenced 2nd Floor Slab on Grade. The locations of the grout injection points in the field were provided and placed by Earth Tech, Inc. The grout injection point locations were based on subsurface information from Dynamic Cone Penetrometer (DCP) testing and Ground Penetrating Radar (GPR) survey as indicated in the Report of Geotechnical Engineering Services-East Pasco Medical Center Expansion Slab on Grade Remediation of 2"°Level [Tierra, Inc. dated April 7]. Thirty-three injection points were installed on a grid spacing of approximately 15 foot center-to-center. Based on utility locations beneath the slab, some proposed grout pipe locations were moved to avoid utility conflicts. Procedure On May 1, 2005, Earth Tech, Inc. mobilized to the subject project site and installed grout injection pipes for the subsurface soil densification program. Between May 3 and May 5, 2005, a Tierra representative observed Earth Tech, Inc. performing the subsurface soil densification program consisting of pressure grouting. 7805 Professional Place •Tampa, FL 33637 Phone(813)989-1354•Fax(813)989-1355 Geotechnical Engineering Services Letter Report East Pasco Medical Center Expansion Slab on Grade Remediation of 2 Level Pasco County, Florida Tierra Project No.: 6511-04-013 Page 2 of 4 The program consisted of advancing grout injection pipes to depths ranging from approximately 10 feet to 18 feet below existing grade and injecting a cement grout mix with an approximate compressive strength in excess of 100 pounds per square inch (psi) at pressures ranging from 50 to 600 psi. As target pressures were reached at each pipe location, the pipe was backed out of the hole at a rate allowing for the target pressure to rebuild. A total of 33 injection points were utilized. A summary of the grout program is provided in Table 1. Conclusion Based on the results of the ground modification program,the referenced location has been grouted to conditions considered adequate for the anticipated construction. This report should, in no way, be considered a guarantee that the previously subsided areas will not subside again.
