If the owner has hired a contractor or co~tractors to undertake work, they may be required
to be licensed in accordance with stat& and local regulations. If the cbntrlctor ~s not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law. tf the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work, they are advised to contact the
City of zephyrhills Building Department, 813-780-0020:
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contractor(s) sign portions of the "Contractor Sections" of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, ~ather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indica~ion that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

it to the "owner" prior to commencement.

certify that no work or install~tion has commenced prior to issuance of a permit and that
all work will be performed to meet standards of all laws regulating construction, City
codes, zoning regulations, and land'development regulations in the jurisdiction. I also
certify that I understand th~t the regulations of other governmental agencies may apply to
the intended work, and that it is my responsibility to identify what actions I must take to

I also certify that, if fill material is to be used in Flood Zone "A" or "A, etc.", it is
understood that a drainage plan addressing a "compensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida prior to permit
issuance.
A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor Shrll issuance of a permit prevent the Building Official from thereafter requiring a
correc ion of errors in plans, construction, or violations of any code. Every permit
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for la
period of six months after the t~me the work is commenced. On~ 90 day extension of time
may be allowed for the permit with fee charge of $15.00. The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month period, or the project will be considered abandoned. .

Whenever the City decides to further exploit the City Rights of Way, any or alIsald poles, wires, pipes, cables, or other facilities and

1 . The holder shall save and keep the City harmless from any and all damages, claims, or injuries that may occur by reason of the constnaction,

this permit s ould fall to meet any of its obligations hereunder. The bond sbaR also indemnify the City for all court costs and reasonable attomey's fees in the

Dear Mr. Ryman:
A meeting was held between City staff to discuss deficiencies at
the above prqject.
a. All sidewalks/driveway ramps shall meet ADA, including
truncated domes at intersections of sidewalks;
b. The house located in the Right of Way is to be demolished
immedilty, no .later than June 30, 2007;
c. Obtain maintainence bond for a period of one year;
d. Stop signs do not meet Manual Uniform Traffic Control Devices
(MUTeD) height requirements;
e. Obtain a Right of Way permit for the required sidewalk and
driveway repairs;
f. Connect the sidewalk along Daugherty Rd with the sidewalk along
Medical View Lane, as shown in the plans; and
g. Relocate the signs that are in the Right of Way.
Failure to remedy these matters will result in the City pursuing all
rights and remedies available to it, whether legal or equitable, all of which
are hereby expressly reserved. In the event that these matters are not
resolved the City shall take all necessary actions to ensure that Ryman
Construction's repeated deficiencies no longer occur within the City.

6841 Med' al View Lane.
All the ins ections appear to be completed except for the final. We have all the necessary paperwork required
including t e sealed engineer's letter in order to send out a Development Order but is held up due to the two signs
which nee to be relocated. Bill had waived a new permit for the sign relocation which was relayed to Richard
approxima ely a week or so ago. Bill has also agreed to a one time extension for 90 days in order for the signs to
be relocat d.
Once the igns are completed/finaled, I can send a Developmental Order. Upon their approval we can schedule
the final in pection.
