A meeting was held between City staff to discuss deficiencies at
the above project.

All sidewaIks!driveway ramps shall meet ADA, including
truncated domes at intersections of sidewalks;
The house located in the Right of Way is to be demolished
immedilty. no.laterthan June 30, 2007;
Obtain maintainence bond for a period of one year;
Stop signs do not meet Manual Uniform Traffic Control Devices
(MUTeD) height requirements;
Obtain a Right of Way permit for the required sidewalk and
driveway repairs;
Connect the sidewalk along Daugherty Rd with the sidewalk along
Medical View Lane, as shown in the plans; and
Relocate the signs that are in the Right of Way.

Failure to remedy these matters will result in the City pursuing all
rights and remedies available to it, whether legal or equitable, all of which
are hereby expressly reserved. In the event that these matters are not
resolved the City shall take all necessary actions to ensure that Ryman

10. The holder shaD save and keep the City harmless from any and aD damages, claims, or injuries that may occur by reason of the constmetion,

11. The holder shall complete the requested work wi~ day~or flUs permit shaD become nuD and void. In the event the work requested
is not complet d upon the expiration date of this permit, the City shaD have the right to complete such work and to charge the holder of the permit for all costs
Incurred in co pleting said work.

This is in response to your request regarding the expiration of the above two permits for the location of 6833 and
6841 Me ical View Lane.
All the in pections appear to be completed except for the final. We have all the necessary paperwork required
including the sealed engineer's letter in order to send out a Development Order but is held up due to the two signs
which ne d to be relocated. Bill had waived a new permit for the sign relocation which was relayed to Richard
approxim tely a week or so ago. Bill has also agreed to a one time extension for 90 days in order for the signs to
be reloca ed.
Once the signs are completedlfinaled, I can send a Developmental Order. Upon their approval we can schedule
the final i spection.
