8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign

TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law-Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
Department of Environmental Protection-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment.
Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
Army Corps of Engineers-Seawalls, Docks, Navigable Waterways,
Department of Health & Rehabilitative Services/Environmental Health Unit-Wells, Wastewater Treatment,
Septic Tanks.
US Environmental Protection Agency-Asbestos abatement.
Federal Aviation Authority-Runways.
I understand that the following restrictions apply to the use of fill:
Use of fill is not allowed in Flood Zone "V" unless expressly permitted.
If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addreSSing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent

glazing tape. The sash utilized two (118") thick clear annealed glass Iiteswjth a 518"
continuous metal spacCer. An exterior $nap-insingle leafdual dUfo11leterrigid vinyl-glazing
bead secured the glass.

Two (318'~x 3/32''}weepslotswereJfJcated at theg1~ng track through the bottom of the
top sash bottom hori,z-ootal rail,one 4-118" from each end. Two (114" x 3/32") weep slots
were.located at the glazing track of the bottomsashbotJ;om horizontal rail, one 2" ftom each
end. One (318" x 3116"} w~p slot were locat.ed just above e;:tchofthe pivot bars on the ends
of the bottom sash rai1 ofthebotn sashes. Two (l/2"x liS") baffled weep slots were located
on the exterior face ofth~ sill, each one 1-7/8." from' each end. Four (112" x liS") weep slots
were located on the inner chamber wall of the sill, two on very end of each side. Two (112"
x 3/16") weep slats were located on the interior of the sill, one 1-3/4" from each en<:!.The
frame'sill Vias a sloped silL Three (3/8" diameter) baftled weep holes were located. on the
bottQmrail wtne screen, one weep in the center and one 5-3/4" from each end.
Seal-ant;
Silicone sealant was applied to all the following areas:
· Entire bottom of the si11to buck intersection..
· Screw heads hole openings used tor anchorage of the frame to buck.
· Exterior and interior offrame to buck. inters~tion.
· A \.{"wood Shim around the perimeter of the frame secured to the frame.
· Cover flange a.t the s.ill and header on the exterior of the frame was sealed to the
frame/flange intersection and the flange was also sealedto the buck/flange intersection.

A ~'4." wood shim was attached with silicone to the perimeter of the buck frame. Two
additional 1,4" wood shim strips were attached m eachFunb screw attachment to the wood

buck andthentbe sill was attached with silicone to the wood. Thee;crerior of the sill then
used a trimatta.chment and was3ecuted to the frame sill and buck using silicone.

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction materials.
The documentation submitted has been reviewed by Miami-Dade County Product Control Division and accepted
by the Board of Rules and Appeals (BORA) to be used in Miami Dade County and other areas where allowed by
the Authority Having Jurisdiction (AID).
This NOA shall not be valid after the expiration date stated below. The Miami-Dade County Product Control
Division (In Miami Dade County) and/or the AID (in areas other than Miami Dade County) reserve the right to
have this product or material tested for quality assurance purposes. If this product or material fails to perform in
the accepted manner, the manufacturer will incur the expense of such testing and the AID may immediately
revoke, modify, or suspend the use of such product or material within their jurisdiction. BORA reserves the right
to revoke this acceptance, if it is determined by Miami-Dade County Product Control Division that this product or
material fails to meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida Building Code,
including the High Velocity Hurricane Zone.

ApPROVAL DOCUMENT: Drawing No. 6220, titled "1" STD. Wall, Elevations Aluminum Tube Clipped
Mullion", sheets 1 through 5 of 5, prepared by PGT Industries, signed and sealed by Robert L. Clark, P.E., dated
04128/00, with last revision on 05/30/06, bearing the Miami-Dade County Product Control Renewal Stamp with
last the Notice of Acceptance number and expiration date by the Miami-Dade County Product Control Division.
MISSn..E IMPACI' RATING: Large and Small Missile Impact
LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Miami-Dade County Product Control Approved", unless otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change in the
materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any
product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply
with any section of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami-Dade County, Florida, and followed by
the expiration date may be displayed in advertising literature. IT any portion of the NOA is displayed, then it shall
be done in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by t!J.e manufacturer or its distributors
and shall be available for inspection at the job site at the request of the Building Official.

well as approval document mentioned above.
