NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit may be subject to "deed" restrictions"
which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance with any
applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division-Licensing Section at 727-847-
,8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law-Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
Department of Environmental Protection-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment.
Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
Army Corps of Engineers-Seawalls, Docks, Navigable Waterways.
Department of Health & Rehabilitative Services/Environmental Health Unit-Wells, Wastewater Treatment,
Septic Tanks.
US Environmental Protection Agency-Asbestos abatement.
Federal Aviation Authority-Runways.
I understand that the following restrictions apply to the use of fill:
Use of fill is not allowed in Flood Zone "V" unless expressly permitted.
If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to, violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the~ob is considered abandoned.

Plan Review Comments
I have reviewed and approved the plans for a tent location for retail sales of tools at 5858
Gall Blvd under the following conditions. My comments have been placed below.
Please contact me if you have any questions with regards to my comments.
1. Tent and sides to have flame rating on tent.
2. Certified fire extinguisher on site.
3. No smoking signs placed around the tent.
4. Adequate exit access and egress provided.
5. If providing lighting, bulbs shall have cages and wiring shall be positioned in a
safe manner.
6. If a generator is used, it shall b e a minimum of30' away from tent/stand.
7. Aisles shall be maintained for accessibility.
8, All appropriate paperwork shall be on site,
Inspections required:
1, Site/tent inspection required before opening to public. This inspection is required
for an approved permit.

To Whom It May Concern~
Following is information on our tent that is requested from time to time~ The queStions usually regard
longevity~ wind capacity, etc. We have certain strengths, tensile, etc. that are guaranteed by our
suppliers. Our suppliers are controlled by government regulated laboratories.
We have industry standards to follQw in the tent business. We build a tent that is much stronger than
in!fustry standard. Our own experience, since 1980, and over 2000 tents in 126 countries and 50 states,
has shown the superior construction of our tents against wind, rain and snow loads. And f1naIly~ we
guarantee customer satisfaction in writing.
The following will cover the information you need.
Center poles: (50' and wider) These are extruded under federal controls for aluminum by Jarl
Extrusions, a division of Alcan Aluminum Corp. They are made, specifically, of structural
grade~ the strongest kind, called 6061 T6. The tensile strength is officially 38~000 lbs PSI
"Ultimate", and 35,000 PSI "Yield". We have sold about 2,000 aluminum center poles of all
sizes. They just don't break. The winch pole galvanized cable has a breaking strength in excess
of 7 tons, and all you can put on it is a few hundred pounds. The winch is rated by Dutton
Lainson conservatively at 2000 lbs, which again~ with a maximum of a few hundred pounds on
each pole, you can exceed only if you try to stretch a taut rope or two with it.

Center poles: (20' - 40') & Quarter poles: (60' and wider.) These are made by Wheatland Steel
of Little Rock, AR of high-tensile steel. The physical tests prove the tensile strength in Ibs.
PSI at 59,680 ultimate, and 47,290 yield. As tested by us, to compete against the best of
hardwood wall poles, we fQWld them to be about 20% stronger. We stood in the middle of
them with each end of the pole on a saw horse. They took 175 pounds with some bouncing.
This has been one of our most reliable tests for wall, quarter, and center poles for the last 14
years.
They have several additional advantages, including the impossibility of splinters. Their
dimensions are, top pipe diameter 2.375: wall .095, base pipe diameter 2.875: wall .1 10. To
date, the galvanized quarter poles have never bent.
Wall Poles: These were tested as described above and with the same results. Further, the large
galvanized foot has just twice the cross section Of our old wood wall poles. That means it will
stand on sod and other soft surfaces with much less tendency to sink in. One of our 60' tents
stood for days this Spring in flood waters without sinking in at all.
Dimensions are, diameter 1.66", wall.OSS". (Our 7.5" wall poles have_a ~a11 thickness of .065.
None of these has bent yet either.) I

Stakes: Our stakes are made from hardened steel made for the oil industry. They not only
resist bending most effectively, but at 48", hold more than twice as much as a 30" stake. In
fact, the shortest stake we send with any tent is 40".
The combination rope we provided you is the best available for tents, being exceptionally
strong (7,040 lbs. tensile), and with the polyester wrap, it is naturally sun r~sistant.
Finally, the quarter poles have a pin that secure them to the quarter pole dome in the tent top.
This insures that they will never fall.
Specifically concerning tent & vinyl: If properly staked and stretched, it can take sustained winds of 40
- 50 MPH and gusts of75 MPH. Eventually, age reduces this slightly.
Takes 6" of wet snow. Theoretically, the tent can take a few feet of dry snow. In reality, dry snow will
always slide off after just a few inches of accumulation, or the wind will keep it clear.
