prevention and protection related activities such as inspections, plan review, administrative fees, and other
costs related to the aforementioned.
Complete Plans, Specifications and Fee Must Accompany Application. Commencement of work without written approval of
the Fire Department's Fire Marshal or required permits or opening up for commercial activity without an approved final
inspection shall be charged double permit fee per day of operation or a minimum of $100.00, whichever is greater. All
work shall be performed in accordance with City Codes and Ordinances.

UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor Dr
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If thE-'
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division-Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law-Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. Ifthe applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
Department of Environmental Protection-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment.
Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
Army Corps of Engineers-Seawalls, Docks, Navigable Waterways.
Department of Health & Rehabilitative Services/Environmental Health Unit-Wells, Wastewater Treatment,
Septic Tanks.
US Environmental Protection Agency-Asbestos abatement.
Federal Aviation Authority-Runways.
I understand that the following restrictions apply to the use of fill:
Use of fill is not allowed in Flood Zone "V" unless expressly permitted.
If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.

Performance flow rates
Flow rates shown are maximum test rates.
Actual delivery rates will vary depending
upon the installation conditions, product
dispensed, added accessories and size of
submersible pump. Tested and calibrated
for accuracy at any speed or pressure.
Safety Warning
Failure to follow proper installation
instructions may result in property damage,
personal injury, or loss of life. Gasboy
products must be installed by a qualified
installer and used in conformance with all
building, fire, and environmental codes and
other safety requirements applicable to their
installation and use, including, but not
limited to, NFPA 30, NFPA 30A, and NFPA
70. This product is only part of a fuel
dispensing system. Additional accessories
or safety devices may be necessary to
meet the applicable codes. Gasboy
dispensers shall not be used for the direct
fueling of aircraft without filters, separators,
and other equipment necessary to ensure
product purity.

· Two-piece spout - the OPW spout offers the highest flow rates in the industry. Replacement Kits
available from OPW.
· "Accu-Stop@" flow control - allows your customers to top off their tanks to the exact amount they
want...easy to stop gasoline flow precisely.
· Low-profile aluminum body - light-weight easier to handle, provides an attractive, image- enhancing
appearance.
· Unique Hand Insulators- OPW offers both a full hand insulator and a unique two-piece option.
Protects both nozzle and vehicle, and insulates users' hand.
· Hold open rack is available for full service applications - the one-hand control means easy setting
to flow rate for customer convenience.
Duratuff@ lever and lever guard - for additional durability to protect against rough handling.
· Blocker on lever guard - to help prevent the nozzle from being jammed into open position with a
foreign object.
· Listed by Underwriters Laboratories, Inc. - for gasoline, 15% ethanol/methanol gasoline blends. UL

NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit may be subject to "deed" restrir;tions"
which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance 'It th any
applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division-Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse. Recovery Fees may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a "certificate. of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law-Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development reglJlations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work, and that it is
my responsibility to Identify what actions I must take to be in compliance. Such agencies include but are not limited to:
Department of. Environmental Protection-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment.
Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
Army Corps of Engineers-Seawalls, Docks, Navigable Waterways.
Department of Health & Rehabilitative Services/Environmental Health Unit-Wells, Wastewater Treatment,
Septic Tanks.
US Environmental Protection Agency-Asbestos abatement.
Federal Aviation Authority-Runways.
I understand that the following restrictions apply to the use of fill:
Use of fill is not allowed in Flood Zone "V" unless expressly permitted.
If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitthlg which is prepared by a professional engineer
licensed by the State of Florida.
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in

may be requested, in writing, from the Building OffiCial for a period not to exceed nln~ty ~90) da~s and Will demonstrate

The OPW 66 is designed to be installed on fuel dispensing hoses, and will separate when subjected
to a designated pull force. The dual valves seat automatically, stopping the flow of fuel and limiting
any fuel spillage, while protecting the dispensing equipment.
For proper operation, the OPW 66V-3/4" must always be installed with a "straightening" hose with a
minimum length of 9", such as the OPW 66H. The 66V-1" must be installed with a minimum length of

OPW 10 Series Em.ergency Shut-off Valves are installed on fuel supply lines beneath dispensers at
grade level to minimize hazards associated with collision or fire at the dispenser. If the dispenser is
pulled over or dislodged by collision, the top of the valve breaks off at the integral shear groove,
activating poppets and shutting off the flow of fuel. Single-Poppet models shut off supply flow, while
,r ,...ble-Poppet models shut off supply as well as prevent release of fuel from the dispenser's internal
piping.
The base of the Emergency Valve is securely anchored to the concrete dispenser island through a
stabilizer bar system within the dispenser sump or pan to ensure proper shearing action. The
valve base is secured to the stabilizer bar using a three-point boss mount system or U-Bolt Kit (not
included); 10UBK-015 (1-1/2") and 10URK-0200 (sold separately.) Value inlet (bottom) connections
are female pipe threads and outlet (top) connections are available with female threads, male threads
or a union fitting.
Other options include suction system models with a normally closed secondary poppet which
maintains prime, and models with external threads on the inlet body which connect to secondary
containment systems.

. High Flow Capacity - The primary poppet is held out of the flow stream while the secondary
poppet is held normally open to minimize head loss across the valve and to protect the poppet
seals from damage and erosion. True 1 1/2" (4 cm) and 2" (5 cm) body sizes ensure
maximum flow.
. Fire Protection - A fusible link trips the valve closed at 1650 F to shut off fuel supply to the
dispenser.
. Main Poppet Seat Integral to the Top Assembly - Having the main poppet seat as an integral
part of the valve top assures a new, clean seating surface is installed each time the top is
replaced. This design also ensures that the integrity of the seal between the valve top and
bottom is verified during line testing and allows full inspection of the main poppet when the top
is removed.

copper/nickel/chrome-plated brass stem are designed to prevent tar build-up and corrosion

· Integral Test Port - A 3/8" (9.5 mm) Test Port allows the piping system to be air tested without
breaking any piping connections.
· Patented Thermal Relief Valve - Relieves excessive pressure build-up caused by thermal
expansion of fuel in the dispenser piping system in the event of fire (double-poppet models
only).
· Low-Profile Tops - Female and Union-top double-poppet valves have a low-profile top to allow
upgrading from OPW single-poppet valves without changing existing piping.
· Multiple Mounting Options - Valves are boss-mounted to stabilizer bars in sumps and pans or
mounted to bars embedded in the island with optional U-Bolt kits 10UBK-015 (not included).
Versatile Combination Body (boss mountlU-Bolt mount) models are available to accommodate

When mounting the OPW 10 Series Emergency Shut-off Valves, the anchoring system employed
must withstand a force greater than 650 ft. pounds per valve. NFPA Code 30A, Automotive and
Marine Service Station Code, requires that the automatic closing feature of emergency shut-off valves
be checked at least once per year by manually tripping the hold-open linkage. OPW recommends the

Features
Electronic ReadlWrite keys and cards are great for fleet
operations or gas dub members. PROKEEfIIS are high
strength, plastic composite keys containing nonvolatile,
readlwrite memory chips, which can be encoded with
confidential information to control access to dispensers
and streamline product transaction data.
FuelMastef"J's patented radio frequency tag system
takes driver data entry out of the information collection
equation. The odometer or chronometer data, along with
other information is automatically collected by the island
fuel management unit without driver effort. An AIM (auto-
motive information module) unit is installed on the equip-
ment and collects and stores information and sends it to the
fuel management unit by RF during fueling operations. This
system works concurrently with the use of PROKEEflls or
smart cards so that the (AIM) units may be added to equip-
ment currently using electronic readlwrite access devices.
The FuelMaste,.e 2500 Plus' capabilities to support self-
serve retail applications have also been increased. As
well as being on an ever increasing number of credit card
networks, prepaid smart cards and PROKEEflls can be
i issued to customers. The system operator may also enter
up to 300 credit card numbers of repeat customers with a
specified discount for each. The Plus system also retums
more credit card information and settle-up data to the PC
software for easier tracking of fuel purchases. The ability
to price fuel at multiple remote sites has been enhanced
to ensure you're in control of your fuel.
A real-time, on-site journal printer can provide hard copy
backups of all transactions. A receipt printer is also

The OPW 61fSTOP AST Overfill Prevention Valve ("The Stopper") is designed to prevent overfilling
of aboveground storage tanks by providing a positive shut-off during a pressurized-fill (pump-on fill)
delivery. "The Stopper" threads into the fill opening and is an integral part of the fill tube. The OPW
61fSTOP is fully adjustable to allow for easy installation in new or existing aboveground tanks of
various height and storage capacities.
'---'13 Stopper is a single-action, complete shut-off valve. When the liquid level rises to your specific,
pre-determined tank capacity, the valve mechanism is released and automatically closes with the flow
of the product. Any excess product left between the valve and the fuel delivery coupler is drained into
the tank through internal drain vents. The drain vents on the 61fSTOP act as an anti-siphon, by
introducing air/vapor into the fill line, to help isolate the tank from a potential siphon due to a broken
or leaking fill pipe.
The 61fSTOP, available in either 2" or 3" full-flow models, maximizes flow rates and minimizes
delivery times. The 2" 61fSTOP requires a 4" tank opening and minimum tank ullage (space from the
top of the stored product to the inside top of the tank) of 8" on the 61fSTOP-1 000 Cylindrical Float
model and minimum ullage of 4" on the 61fSTOP-2000 Float Arm model. The 61fSTOP-1000, with
the cylindrical float, eliminates the need to position the valve to avoid tank walls or cross bracing. The
61fSTOP-2000, with the float arm allows for increased usable storage capacity in the tank and is well-
suited for small capacity rectangular AST's. The 3" 61fSTOP requires a 6" tank opening and
incorporates a cylindrical float requiring 8" of tank ullage space. The 3" Stopper is a high-flow unit
It can accommodate flow rates over 400 gallons per minute (depending on pump capacity).
The 61fSTOP-100M and -200M are anodized and compatible with 100% methanol or ethanol
fuels. The OPW 61fSTOP-305A meets with the specifications of the Handbook of Aviation Fuels
Properties for Aviation/Jet Fuel Compatibility (as developed by the Coordinating Research Council of
the Society of Automotive Engineers). The 61fSTOP-305A is compatible with aviation fuels (including
jet fuels), gasoline, gasoline-alcohol fuels, diesel, kerosene, mogas, fuel oil, motor oils (up to 60W),
and anti-freeze.

The OPW 61 FT Drop Tube is designed to install on the bottom of the 61fSTOP AST Overfill
Prevention Valves when a lightweight, longer lower drop tube is required. Constructed of extruded
aluminum, the OPW 61 FT is connected to the bottom of the 61fSTOP by a clevis and cotter pin
assembly and allows for submerged filling of ASTs.

The OPW 199ASV Anti-Siphon Valve is designed to help prevent siphoning of an aboveground
storage tank should, a leak or break occur in the fuel supply line. The 199ASV installs on the top of an
aboveground storage tank with a suction rod on the inlet and the fuel supply line (to a remote pump)
on the outlet of the valve. A poppet in the valve remains normally closed, isolating the tank from
c>i~honing. When the remote pump is activated, it overcomes a factory-adjusted spring set to a
~determined hydrostatic head pressure and allows flow of fuel out of the AST. An internal pressure
relief valve helps absorb thermal expansion in the fuel supply line. The 199ASV is available in female
NPT threads with four different factory-adjusted hydrostatic head pressure settings.

Veeder-Root's TLS Overfill Alarm provides an early warning of potential
tank bulk delivery overfills as required by the Federal regulations
governing underground storage tanks. Wired to an alarm relay in a TLS-
350R, TLS-350, TLS-35OJ, TLS-300, TLS-300C, TLS-300i, TLS-300, ILS-
350, and TLS-250 or TLS-250i System, the alarm relay activates the TLS overfill alarm horn and light
when a potential overfill is detected. All TLS systems have programmable overfill alarm limits that
can be set for each tank at a location.
An optional Alarm Acknowledgement Switch is also available for locations where driver Intervention is
required by local codes. When the driver presses the acknowledgement button, the overfill alarm
shuts off and the alarm acknowledgement light illuminates, This light will stay on until the TLS prints
an inventory increase report.
A built-in timer lets you adjust length of time that the audible alarm will stay on from 0 to 60
seconds, You can also adjust noise level from 78 to 103dB (at 10 feet),
