Chapter 633, Florida Statutes, authorizes the City to charge and collect user fees to pay for the costs of fire
prevention and protection related activities such as inspections, plan review, administrative fees, and other
costs related to the aforementioned.
Complete Plans, Specifications and Fee Must Accompany Application. Commencement of work without written approval of
the Fire Department's Fire Marshal or required permits or opening up for commercial activity without an approved final
inspection shall be charged double permit fee per day of operation or a minimum of $100.00, whichever is greater. All
work shall be performed in accordance with City Codes and Ordinances.

A. NOTICE OF DEED RESTRICTIONS
The undersigned understands that this permit may be subject to "deed restrictions" which
may be more restrictive than City regulations. The undersigned assumes re~ponsibility for
compliance with any applicable deed restrictions.
B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
If the owner has hired a contractor or contractors to undertake work, they may be required
to be licensed in accordance with state and local regulations. If the contractor is not
licensed as required by law, both the owner and contractor may be cited for a misdemeanor
violation under state law. If the owner or intended contractor are uncertain as to what
licensing requirements may apply for the intended work, they are advised to contact the
City of Zephyrhills Building Department, 813-780-0020.
Furthermore, if the owner has hired a contractor or contractors, he is advised to have the
contractor(s) sign po~tions of the "Contractor Sections" of this application for which they
will be responsible. If you, as the owner signs as the contractor, you are indicating that
you, rather than the contractor, are responsible for the work. If the contractor wishes
you to sign as contractor that may be an indication that he is not properly licensed and is
not entitled to permitting privileges in the City of Zephyrhills.

I certify that I, the applicant, have been provided with a copy of "Florida's Construction
lien Law - Homeowner's Protection Guide" prepared by the Florida Department of Agriculture
and Consumer Affairs. If the applicant is som~one other that the "owner", I cerify that I
have obtained a copy of the above described document and promise in good faith to deliver
it to the "owner" prior to commencement.
E. CONTRACTOR'S/OWNER'S AFFIDAVIT
I certify that all the information in this application is accurate and that all work will
be done in compliance with all applicable laws regulating construction, zoning, and land
development.
Appli~ation is hereby made to obtain a permit to do work and installation as indicated. I
certify that no work or installation has commenced prior to issuance of a permit and that
all work will be performed to meet standards of all laws regulating construction, City
codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other governmental agencies may apply to
the intended work, and that it is my responsibility to identify what actions I must take to
be in compliance. Such agencies include but are not limited to: *Department of
Environmental Regulation-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment
*Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas,
Altering Watercourses
*Army Corps of Engineers-Seawalls, Docks, Navigable Waterways
*Department of Health & Rehabilitative Services, Environmental Health Unit-Wells,
Wastewater Treatment, Septic Tanks

I also certify that, if fill material is to be used in Flood Zone "A" or "A, etc.", it is
understood that a drainage plan addressing a "compensating volume" will be submitted which
is prepared by a professional engineer registered in the State of Florida prior to permit
issuance.
A permit issued shall be construed to be a license to proceed with the work and not as
authority to violate, cancel, alter, or set aside any provisions of the technical codes,
nor shall issuance of a permit prevent the Building Official from thereafter requiring a
correction of errors in plans, construction, or violations of any code. Every permit
issued shall become invalid unless the work authorized by such permit is commenced within
six months of issuance, or if work authorized by the permit is suspended or abandoned for ~a
period of six months after the time the work is commenced. One 90 day extension of time
may be allowed for the permit with fee charge of $15.00. The extension shall be requested
in writing to the Building Official. An approved inspection must be logged during each six
month period, or the project will be considered abandoned.

I have reviewed and approved the plans for a sprinkler system modification located at
38109 Market Sq. I have attached the comments for the plan approval. If there are any
questions please contact my office at 813-780-0041.
1. Installation/modification shall compliant with the Florida Fire Prevention
Code and NFP A 13, 2007 Edition.
2. Sprinkler system shall be recertified once complete.
3. FOC for this system shall be placarded so it is easily located in darkness,

5. If system is not specific to this unit, if possible, flow switch shall be installed
in system for this unit and wired into fire alarm to indicate that this unit has a

1. System shall be isolated and pressure tested. Pressure test shall be witnessed
by the Fire Marshal.

General
Description
The Series TV-FRS, 2,8, 4.2, 5.6, and
8.0 K-factor, Upright and Pendent
Sprinklers described in this data sheet
are quick response - standard cover-
age, decorative 3 mm glass bulb type
spray sprinklers designed for use in
light or ordinary hazard, commercial
occupancies such as banks, hotels,
shopping malls, etc.
The recessed version of the Series
TV-FRS Pendent Sprinkler, where ap-
plicable, is intended for use in areas
with a finished ceiling. It uses either a
two-piece Style 10 (1/2 inch NPT) or
Style 40 (3/4 inch NPT) Recessed Es-
cutcheon with 1/2 inch (12,7 mm) of
recessed adjustment or up to 3/4 inch
(19,1 mm) of total adjustment from the
flush pendent position, or a two-piece
Style 20 (1/2 inch NPT) or Style 30 (3/4
inch NPT) Recessed Escutcheon with
1/4 inch (6,4 mm) of recessed adjust-
ment or up to 1/2 inch (12,7 mm) of
total adjustment from the flush pen-
dent position. The adjustment provided
by the Recessed Escutcheon reduces
the accuracy to which the fixed pipe
drops to the sprinklers must be cut.
Corrosion resistant coatings, where
applicable. are utilized to extend the
life of copper alloy sprinklers beyond
that which would otherwise be ob-
tained when exposed to corrosive at-

ponents. Improper handling and in-
stallation can permanently damage
a sprinkler system or its compo-
nents and cause the sprinkler to fail
to operate in a fire situation or cause
it to operate prematurely.

tant coated sprinklers have passed the
standard corrosion tests of the appli-
cable approval agencies, the testing is
not representative of all possible cor-
rosive atmospheres. Consequently, it
is recommended that the end user be
consulted with respect to the suitability
of these coatings for any given corro-
sive environment. The effects of ambi-
ent temperature, concentration of
chemicals, and gas/chemical velocity,
should be considered, as a minimum,
along with the corrosive nature of the
chemical to which the sprinklers will be
exposed.

Operation
The glass Bulb contains a fluid which
expands when exposed to heat. When
the rated temperature is reached, the
fluid expands sufficiently to shatter the
glass Bulb, allowing the sprinkler to
activate and water to flow.

(1.6 mm) for the 1350F/57"C to 3/32
inch (2,4 mm) for the 2860F/1410C
temperature ratings,
A leak tight 1/2 inch NPT sprinkler joint
should be obtained with a torque of 7
to 14 ft.lbs. (9.5 to 19,0 Nm). A maxi-
mum of 21 ft. Ibs. (28,5 Nm) of torque
may be used to install sprinklers with
1/2 NPT connections. A leak tight 3/4
TFP17,1
inch NPT sprinkler joint should'be ob-
tained with a torque of 10 to 20 ft.lbs,
(13,4 to 26,8 Nm). A maximum of 30
fUbs. (40,7 Nm) of torque is to be used
to install sprinklers with 3/4 NPT con-
nections. Higher levels of torque may
distort the sprinkler inlet and cause
leakage or impairment of the sprinkler.
Do not attempt to make-up for insuffi-
cient adjustment in the escutcheon
plate by under- or over-tightening the
sprinkler. Readjust the position of the
sprinkler fitting to suit,
The Series TV -FRS Pendent and Up-
right Sprinklers must be installed in
accordance with the following instruc-
tions.
Step 1. Pendent sprinklers are to be
installed in the pendent position, and
upright sprinklers are to be installed in
the upright position.
Step 2. With pipe thread sealant ap-
plied to the pipe threads, hand tighten
the sprinkler into the sprinkler fitting.
Step 3. Tighten the sprinkler into the
sprinkler fitting using only the W- Type
6 Sprinkler Wrench (Ref. Figure 13).
With reference to Figures 1, 2, 3, and
4, the W- Type 6 Sprinkler Wrench is to
be applied to the sprinkler wrench
flats.
The Series TV-FRS Recessed Pen-
dent Sprinklers must be installed in
accordance with the following instruc-
tions.
Step A. After installing the Style 10.
20, 30, or 40 Mounting Plate, as appli-
cable, over the sprinkler threads and
with pipe thread sealant applied to the
pipe threads, hand tighten the sprin-
kler into the sprinkler fitting.
Step S. Tighten the sprinkler into the
sprinkler fitting using only the W- Type
7 Recessed Sprinkler Wrench (Ref.
Figure 14). With reference to Figure 1,
2, 3, and 4, the W-Type 7 Recessed
Sprinkler Wrench is to be applied to
the sprinkler wrench flats.
Step C. After the ceiling has been in-
stalled or the finish coat has been ap-
plied, slide on the Style 10, 20, 30, or
40 Closure over the Series TV-FRS
Sprinkler and push the Closure over
the Mounting Plate until its flange
comes in contact with the ceiling,

Care and
Maintenance
The Series TV-FRS Sprinklers must be
maintained and serviced in accord-
ance with the following instructions:
NOTES
Before closing a fire protection system
main control valve for maintenance
work on the fire protection system that
it controls, permission to shut down the
affected fire protection system must be
obtained from the proper authorities
and all personnel who may be affected
by this action must be notified.
Absence of an escutcheon, which is
used to cover a clearance hole, may
delay the time to sprinkler operation in
a fire situation.
Sprinklers that are found to be leaking
or exhibiting visible signs of corrosion
must be replaced.
Automatic sprinklers must never be
painted, plated, coated or otherwise
altered after leaving the factory. Modi-
fied sprinklers must be replaced.
Sprinklers that have been exposed to
corrosive products of combustion, but
have not operated, should be replaced
if they cannot be completely cleaned
by wiping the sprinkler with a cloth or
by brushing it with a soft bristle brush,
Care must be exercised to avoid dam-
age to the sprinklers - before, during,
and after installation. Sprinklers dam-
aged by dropping, striking, wrench
twist/slippage, or the like, must be re-
placed. Also, replace any sprinkler that
has a cracked bulb or that has lost
liquid from its bulb. (Ref. Installation
Section).
Frequent visual inspections are rec-
ommended to be initially performed for
corrosion resistant coated sprinklers,
after the installation has been com-
pleted, to verify the integrity of the cor-
rosion resistant coating. Thereafter,
annual inspections per NFPA 25
should suffice; however, instead of in-
specting from the floor level, a random
sampling of close-up visual inspec-
tions should be made, so as to better
determine the exact sprinkler condi-

and efficient method of changing direction.

+- Proper hole preparation is required for effective sealing and performance. Check the pipe seal surface within 5/8" of the hole to be certain it is free from conditions that would affect
proper gasket sealing. Remove any sharp or rough edges from the hole or upper housing contact area, that might affect assembly, proper seating 01 the locating collar or flow from
the outlet. Check gasket grade to be certain it is suitable for the service. For crosses, ensure double outlet holes are aligned on opposite sides of the pipe. The use of threaded
products other than steel pipe. such as dry pendants, etc. may not be compatible with the female threaded outlet on the Mechanical Tee. Always confirm compatibility by
contacting Tyeo Fire Products.

Merit Weld-Mlsel'rM Tee.tet. Welding Branch Outlet Fittings offer the user a
high strength, low cost forged threaded and grooved line of fittings specifi-
cally designed and manufactured to be Installed on schedules 5 thru 10,
proprietary thin wall flow pipe and standard wall pipe,
Merit Tee-Lets are forged steel welding outlet fittings. The material used in
manufacture meets the chemical and physical requirements of ASTMA 53,
Grades A or B, Type E, Tee:-Lets employ a low weld volume design to provide
for either a partial or full penetration weld employing' a single pass with mini-
mum bum-through and pipe distortion. Weld Miser Tee-lets are recommend-
ed for use on proprietary thin wall, schedules 5, 10 and 40 pipe. Threads
comply with ANSI 81,20.1 or 1807/1. They are UL Usted and FM Approved
for use conforming to the requirements of 8ulletin 13 1999 of the National
Fire ProtecUon Association, When used in fire sprinkler systems, Tee-lets are
rated for 300 psi, When used in mechanical systems, maximum pressures

Installation: Hanger load nut above clevis must be tightened securely to assure proper hanger
performance. When an oversized clevis is used, a pipe spacer should be placed over the clevis bolt
as a spacer to assure that the lower U-strap will not move in on the bolt. For ductile iron pipe sizes,
see Figure 590,
Adjustment: VertIcal adjustment without removing
pipe may be made from 3fs'through 5118', varying with
the size of clevis. Tighten upper nut after adjustment.
Features:
· Design has yoke on outside of lower U-strap so
yoke cannot slide toward center of boll,
thus bending of bolt is minimized.
· Sizes 5' and up have rod and, two nuts instead
of bolt and nut; thread length on clevis rod
is such that the thread Jocks the nuts in place,
and threads are not in shear plane.
Ordering: Specify pipe size, figure number, name
and finish.
Note: Punched forming holes may be present on
certain sizes of this clevis hanger. These holes are
solely for the purpose of manufacturing, and do not
effect the structural integrity or load carrying capacities
of these hangers.

Dyna-Flow pipe is the "original" high-
strength lightwall sprinkler pipe. Dyna-
Flow has outstanding hydraulic
capabilities and is recognized as the
most popular alternative to Schedule-lO
pipe. Lightweight, easy to cut and easy
to handle for installation. Dyna-Flow is a
valuable addition to any fire protection
system,
With an inside diarneter of up to 11 %
larger than Schedule-40 and up to 7%
larger than Schedule-lO, Dyna-Flow
pipe hydraulics are exceptional. Larger
I.D.s enable Dyna-Flow, and related
components, to be down-sized within the
system, thus increasing the potential for
job cost savings. For complete Hazen-

Hydraulic Data Tables."
Dyna-FloW products are coated with
an environmentally approved and
specially formulated modified-acrylic or
water-based coating, This durable coating
is paintable. The black coating acts as
an excellent primer and is resistant to
weathering and u.v. degradation from
outdoor storage.
Metallurgical properties provide excellent
fabrication characteristics for end prep
finishes, welding and roll grooving.
There is no special process or equipment
needed for fabrication and installation,
The internal surface of all black Allied
Tube & Conduit Fire Sprinkler pipe

coated with our new Antibacterial
Formula, "ABF". In scientific laboratory
test, ABS proved to have superior
resistance to microbial colonization of
pipe walls, thereby delaying or possibly
preventing the onset of Microbiologically
Influenced corrosion (MIC) when the
