12. The holder shall post a cash or surety bond with the City Manager In the sum of $ dollars to guarantee performance of the
obligations herein, and to guarantee maintenance of the rlght-or-way described herein for a period of one (1) year following completion of the requested
Installation. In the event a surety bond Is posted, the surety bond shall be made payable to the City and shall obligate the surety to hold the City harmless In
the event the holder of this permit should fall to meet any of Its obligations hereunder. The bond shall also indemnify the City for all court costs and
reasonable attorneys fees In the event legal action Is required to collect on said bond.

Mr. LeBlanc,
In regards to the questions you asked about the ROW permit for the monitoring well installation at 151 Street:
. How long will the well be in use?
The monitoring well will be in use indefinitely.
· Is the wellhead housing and installation within the right of way FOOT compliant?
The housing and installation performed will be in compliance with FOOT standards.
· Will the wellhead housing be marked to avoid a pedestrian I motorist hazard?
Upon completion the wellhead housing will be flush to surface level. The manhole cover will be painted to
increase visibly and bolted down to eliminate any movement by coming into contact with it. No hazards
will be created by its presence.
· Does your firm assume all liability for the wellhead while in place on public right of way?
It is the Florida Department of Environmental Protection who is funding the installation of the monitoring
well. Upon completion, the well becomes property of the state.

Dear Mr. Ogelby:
The Florida Department of Environmental Protection (FDEP) has approved the ROW
installation, under the supervision of Handex Consulting & Remediation - Southeast
LLC (Handex) of one monitoring well at the above mentioned site. Due to the location
of this proposed monitoring well being in a ROW, the installation of this well will require
a City of Zephyrhills ROW Use Permit. This permit along with the legal description of
the location is included as Attachment A.
The well (designated MW-8S) will serve as a supplemental well to existing MW-8. The
proposed location of MW-8S is approximately 5 feet northeast of MW-8 as shown on the
site map included in Attachment B as Figure 1. This well will be installed by a hollow-
stem auger drill rig in the easement between Maine-Iy New England Restaurant and 1 st
St. MW-8S will be constructed similarly to the existing MW-8 but with a deeper total
depth (25 feet bls) and larger screen interval (10 feet to 25 feet bls) in order to obtain a
more representative groundwater sample from this location. Figure 2 depicts the well
construction details of proposed monitoring well MW-8S. The newly installed well will
be graded to surface level and set beneath a steel manhole.
On February 20, 2008, Handex contacted Sunshine State One Call of Florida (SSOC)
for the mark-out of all utilities in the area and was issued ticket number 051802941 for
the utility mark-out procedure. Upon the City of Zephyrhills' review and issuance of this
ROW permit, Handex will again contact SSOC to update all utility locates (if necessary)
before beginning work at the site.

Handex Consulting and Remediation, Southeast, LLC, (Handex) is in the process of
preparing a Right-Of-Way (ROW) Use Permit with the City of Zephyrhills Building
Department for underground construction activities performed in a Zephyrhills, Pasco
County ROW located in the easement between 1st St. and the property occupied by
Maine-Iy New England Seafood Restaurant.
The construction will entail the installation of one monitoring well to a total depth of 25
feet below land surface (bls) at the location marked on the included site map. A
condition of the permit required is that we notify you of our work in writing and request
that you make any necessary utility mark-outs. Sunshine State One Call was contacted
on February 20, 2008 for the utility locate at 5039 1st St. in Zephyrhills and issued ticket
number 050803751 for the marking-out of the utilities along 1 st St.
Upon the City of Zephyrhills review and approval of this permit, Handex will re-call
Sunshine State One Call and renew this ticket (if necessary) that will reference the
location of the above proposed well.
