NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit may be subject to "deed" restrictions"
which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance with any
applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division-Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law-Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above desO"ibed document and promise in good faith to
deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
Department of Environmental Protection-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, WaterlWastewater Treatment.
Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
Army Corps of Engineers-Seawalls, Docks, Navigable Waterways.
Department of Health & Rehabilitative ServiceslEnvironmental Health Unit-Wells, Wastewater Treatment,
Septic Tanks.
US Environmental Protection Agency-Asbestos abatement.
Federal Aviation Authority-Runways.
I understand that the following restrictions apply to the use of fill:
Use of fill is not allowed in Flood Zone 'Y" unless expressly permitted.
If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not spedfically induded in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered abandoned.

WE PROPOSE hereby to furnish LABOR AND MATERIALS in accordance with the above defined SCOPE OF WORK
for the sum of:
A)There is ene Mobilization included in the above price. Additional mobilizations will be charged as provided for below.
B) Payment to MEGALlFT is not contingent upon acceptance of MEGALlFT work by any third party, and is not contingent upon payment being
received by buyer.
C) Materials delivered to the job to be included in invoicing.
D) There are no bonds or liquidated damages included. This proposal does not include fees, or other charges as may be required by
state or local agencies. Megalift accepts no responsibility for design of the project and does not warrant any products delivered to
be merchantable for any particular purpose. This proposal is offered as a service, and it is Buyer's responsibility to provide Megalift
with correct and comprehensive plans and specifications, and with adjacent and/or adjoining work which conform to those plans
and specifications. Any defect in project design or installation of adjacent and/or adjoining work voids any and all warranties, either
express or implied, and shall discharge Megalift from all liability.
E) All agreements contingent upon strikes, accidents or delays beyond our control. Owner to carry fire, tomado, builder's risk and other
necessary insurance. Our workers are fully covered by Worker's Compensation Insurance.
F) All payments will be conditioned solely on acceptability of completed work. Unless Megalift receives notice, in writing, of any claimed defect
or non-confonnity within thirty (30) days of its last date of fumishing labor, services or materials ('Work"), all Work fumished by Megalift shall
be needed to have been accepted. In the event that any payment is not made, the amount shall be deemed delinquent and shall bear interest
at 1.5% per month. Buyer also agrees to pay all collection costs and expenses, including reasonable attomey's fees, incurred by Seller in
collecting or attempting to collect any invoice that has not been paid within sixty (60) days of invoice date.
G) Megalift will not indemnify any other parties' negligence. Buyer shall indemnify, defect, or hold hannless Megalift from any against all
liability that does not result solely from any act or omission of Megalift. This obligation to indemnify shall be limited, in amount, to the contract
price for constructing the Project. Megalift will not be responsible for, and will not indemnify any party for any act or omission of that party.
H) Any reduction and/or change in the Scope of work may result in additional charges. Price quoted is based upon typical installation,
adequate access to the site and work area, and timely completion of related work by other parties. In the event that Megalift work is impaired
by an act or omission, not solely due to Megallft own forces, additional charges may result.
I) This Proposal is valid for 60 days from the date of submission.

B. Felt Underlayment: ASTM D 226, Type I, No. 15 or ASTM D 226, Type II, No. 30, unperforated,

C. Perimeter Underlayment: ASTM D 1970; self-adhering, polymer-modified, bituminous sheet
underlayment; 40 mils (1 mm) thick. Provide primer when recommended by underlayment
manufacturer. As required by local building code.
D. Sealant: One-part elastomeric polyurethane, sealant as recommended in writing by panel
manufacturer. Where sealant will be exposed, provide in color to match panels.
1. Standard: ASTM C920-86.
E. Fasteners: Screws - Minimum No.8, 1 inch (25 mm) long, or of sufficient length to penetrate
substrate 1/2 inch (12.7 mm) minimum with either a black or color finish matching the panels.

PART 3 - EXECUTION

EXAMINATION
Examine substrate and conditions for compliance with requirements for maximum moisture
content, soundness of framing, and other conditions affecting performance of DECRA metal
roofing. [Damaged, rotted or loose roofing materials shall be removed and the substrate
corrected for re-roofing applications]. Proceed with installation only after unsatisfactory
conditions have been corrected.
B.
PREPARATION
Inspect and verify roof framing spacing and installation is straight, true and ready for installation
of battens and DECRA roof panels.
Coordinate installation of metal panels with flashing and other adjoining work to ensure proper
sequencing. Do not install roofing until vent stacks and other penetrations through roofing have
been installed, are securely fastened and flashing is in place.
Inspect and verify exterior stucco and EIFS wall enclosures are completed.

INSTALLATION
General: Comply with manufacturers written instructions for products and applications indicated,
unless more stringent requirements apply.
Valleys: Install in accordance with manufacturers instructions with a minimum 6 inch overlap in
direction of flow.
Flashing: Install as indicated on approved submittals and in accordance with manufacturers
written instructions.
Tile Panels: Install DECRA Villa Tile, accessories, flashing, and hip & ridge level and plumb.
Use fasteners per above specifications.
1. Install each DECRA Villa Tile Panel with alternating courses staggered in accordance
with manufacturer's instructions.

Specify the appropriate number of fasteners per code requirements. Metro-Dade may require additional
fasteners.

2. Fasten each panel with minimum 4 fasteners horizontally along the back flange of each
panel at pre-punched tabs.
3. First course of panels positioned at eave will require 4 fasteners in the nose of the panel.
E. Cut DECRA panels into each side of valleys in accordance with the manufacturer's instructions
straight and true to the line of the valley.
F. Hip & Ridge: Install Barrel Hip & Ridge along hips, ridges and rakes as indicated on approved
submittals and in accordance with manufacturers written instructions. Bend and fold exposed
ends of hips ridges and neatly, cap with an end cap or a piece of similar material.

3.4 CLEANING AND PROTECTION
G. Damaged Units: Replace panels and other components of the work that have been dented,
damaged or have deteriorated beyond successful repair by finish touchup with acrylic coating
and stone chip granules.
H. Cleaning: After completing installation, remove any debris from the roof.
I. Foot Traffic: Avoid walking on side laps. Foot traffic should be kept to the tile pans or low points,
not on the high points, to prevent denting of panels.
