costs related to the aforementioned.

UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by taw, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division-Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign

contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco

TRANSPORTATION IMPACTIUTlLlTIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buikfmgs, as speciied in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law-Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and prorrise in good faith to
deliver it to the "owner" prior to commencement

Use of fill is not allowed in Rood Zone V unless expressly permitted.
If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
.compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida,
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fiR wiI be used only to fill the area within the stem wall.
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect acfJScent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre Vllhich are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I prorrise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction, I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invatid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. Art extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered abandoned.

I have reviewed and approved the plans for a fire alarm system addition located at 38107
Market Sq for Florida Med Clinic. I have attached the comments for the plan approval. If
there are any questions please contact my office at 813-780-0041.
1. System to be installed in accordance to NFPA 72,2007 edition.
2. Make sure batteries are dated and phone jacks are marked.
3. Ensure dedicated breaker is marked and locked off.
4. Provide a zone chart next to panel.
5. Entire system shall be recertified as if it was an annual.
6. Add pull station by rear exit.
7. Add outside strobe main parking lot side (south) to indicate location ofumt
panel is in.

Installation ease. The i31ine redetines installation ease with its
plug-in design. This allows an installer to pre-wire the bases included
with the heads. The large wire entry port and in-line terminals
provide ample room for neatly routing the wiring inside the base.
The base accommodates a variety of back box mounting methods,
as well as direct mounting with drywall anchors. To complete the
installation, i3 heads plug in to the base with a simple Stop-Drop 'N
LockWaction.
Intelligence. i3 detectors offer a number of intelligent features
to simplify testing and maintenance. Drift compensation and
smoothing algorithms are standard with the i3 line to minimize
nuisance alarms. Two-wire i3 detectors needing cleaning can
generate a remote maintenance signal, when connected to the
2W-MOD2 loop test/maintenance module, or to a panel equipped
with the i3 protocoL This signal is indicated by LEDs located at the
module and the paneL The SENS-RDR, a wireless device, displays the
sensitivityof i3 detectors in terms of percent per-foot-obscuration.
Instant inspection. The i3 series provides wide-angle red and
green LED indicators for instant inspection ofthe detector's
condition: normal standby, out-oF-sensitivity, alarm, or freeze trouble.
When connected to the 2W-MOD2 loop test/maintenance module
or a panel with the j3 protocol, the EZ Walk loop test feature is
available on two-wire i3 detectors. This feature verities the initiating
loop wiring by providing LED status indication at each detector

(b Smoke Detector Specifications
Smoke detector shall be a System Sensor i3 Series model number~ listed to Underwriters Laboratories UL 268 for Fire Protection Signaling Systems. The
detector shall be a photoelectric type (ModeI2W..f3. 4W-B) or a combination photoelectric/thermal (Model 2WT-B,4WT-B) with thermal sensor rated at 13S'F
(S7.2'C). The detector shall include a mounting base for mounting to 3'h-inch and 4-inch octagonal, single gang. and 4-inch square back boxes with a plaster
ring, or direct mount to the ceiling using drywall anchors. Wiring connections shall be made by means ofSEMS screws. The detector shall allow pre-wiring of
the base and the head shall be a plug-in type. The detector shall have a nominal sensitivity of 25 percent-per-foot nominal as measured in the UL smoke box.
The detector shall be capable of automatically adjusting its sensitivity by means of drift compensation and smoothing algorithms. The detector shall provide
dual color LED indication which blinks to indicate power up, normal standby, out of sensitivity, alarm, and freeze trouble (Model 2WT-B, 4WT-B) conditions. When
used in conjunction with the 2W-M0D2 module, 2-wire models shall include a maintenance signal to indicate the need for maintenance at the alarm control
panel, and shall provide a loop testing capability to verify the circuit without testing each detector individually.

Description:
The Wheelock Series Z notification appliances feabJre an easy snap
on base that is designed to simplify the installation and testing of
horns, strobes, and hom/strobes. The separate Series Z snap on
base can be pre-wired so circuit wiring can be fully tested before
the appliance is installed and before the walls are covered. Once
all surrounding work is complete, the appliance can be simply
installed.by snaping it on the base. Shorting contacts in the base,
which provide continuity for circuit testing, are permanently opened
when the appliance is installed so any subsequent removal of the
appliance will indicate a trouble condition on that circuit at the control
panel when circuit supervision is enabled. The same base is u~d
for all Series Z horns, strobes and hom/strobes to provide consistent
installation and easy replacement of appliances if required. A locking
screw is also included for the appliance to provide extra secure

Strobes are designed to flash at 1 flash per second minimum over their "Regulated Voltage Range".
All candela ratings represent minmumeffective strobe intensily based on UL Standard 1971.
Series ZNS Strobe products are listed under UL Standards 1971 and 464 for indoor use with a temperature range of 32"Fto 120"F (O"C to
49"C) and maximum humidity of 93% (:t 2%).
Series ZNH horns are listed under UL Standard 464 for audible signal appliances (Indoor use only).
"Regulated Voltage Range" is the newest tenninology used by UL to identify the voltage range. Prior to this change UL used the terminology

ARCHITECTS AND ENGINEERS SPECIRCATIONS
General
AudibleMsual notification appliances shalt be listed for indoor use and shall meet the requirements of FCC Part 15 Class B. These
appliances shall be listed under Ul Standard 1971, (Standard for Safety Signaling Devices for Hearing Impaired) and UL Standard 464
(Fire Protective Signaling)_ The appliances shall use a Patented Universal EZMount backplate that shall allow mounting to a single-
lang, double-gang, 4-inch square, 4" octal, ora 3-1/2" octal backbox. Two wire appliance wiring shall be capable of directly connecting
to the mounting back plate. Continuity checking of the entire NAC circuit prior to atlachinQ any audiblelvisual notification appliances
shall be allowed. A dust cover shall fit and protect the mounting plate. The dust cover shall be easily removed when the appliance is
installed over the backplate. Removal of an appliance shall result in an alarm condition by the Fire Alarm Control Panel (FACP).
Strobes
Strobe appliances shall produce a minimum flash rate of 60 flashes per minute (1 flash per second) over the Regulated Voltage Range
of 16 to 33 VDC and shall incorporate a Xenon f1ashtube endosed in a rugged Lexan lens. The strobes shall be available with two
or four field selectable settings in one unit and shall be rated, per UL 1971, for up to 185 cd for waD mounting and 177 cd for ceiling
mounting. The strobes shall operate over an extended temperature range of 32"F to 1200F (OOC to 490C) and be listed for maximum
humidity of 95% RH. Strobe inputs shall be polarized for compatibility with standard reverse polarity supervision of circuit wiring by a
Fire Alarm Control Panel (FACP).
Audibles and Audible/Strobe Combinations
Horns and horn/strobes shall be listed for Indoor use under UL Standard 464. The horns shall be able to produce a continuous output
or a temporal code-30utput that can be synchronized. The horns shall have at least 2 sound level settings of 90 and 95 dBA.
Synchronization Modules
V\lhen synchronization of strobes or temporal Code-3 audibles is required, the appliances shall be compatible with the V\lheelock Series
8M and DSM Sync Modules or the Wheelock PS-24-8MC Power Supply with built-in, patented sync protocol. The strobes shall not
drift out of synchronization at any time during operation. Audibles and strobes shall be able to be synchronized on a 2-wire circuit with
the capability to silence the audible if required. If the sync module or power supply fails to operate (Le., contacts remain closed), the
strobes shall revert to a non-synchronized f1ashrate
