TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESQURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fee~ may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, a~ specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, thatl such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impac~ Fees and Resource Recovery Fees must be paid prior to
receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit iss~ance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in acc?rdance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes\ as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law-Homeowner's
Protection Guide" prepared by the Florida Department of Agripulture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the "owner" prior to commencement. ~

will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is

A Delivery includes a one time building placement and leveling (up to 6 inches).
B If the customer requires the delivery driver to perform duties that make the delivery longer
than 45 minutes, the customer agrees to pay a charge of $40.00 per hour.
C Additional charges are payable up front and must be approved on site by phoning the main
D The customer may forfeit all monies paid if the answers to questions 1-4 above prove
inaccurate and the building cannot be delivered.
E The customer must pay all deposits, fees, delivery costs, and applicable taxes before the
scheduling of Rent-To-Own orders.
F The customer must pay a deposit (amount to be determined by management) on special built
buildings prior to the order being processed for c9nstruction.
G Cook Portable Warehouses carry a lifetime warr~nty against termite damage and decay only.
Therefore, it is not necessary to place them on blocks. However, at the customer's request
and approved on site by calling the main office, the driver will block the buildings no more than
1 concrete block high, provided the customer has thel,locks on hand at the time of delivery for
a charge of $30.00 for buildings up to 16' and $40.00 for buildings up to 24'. Buildings longer
than 24' must be placed on level ground.
H In the event legal proceedings are filed to enforce your obligation to pay for the building
described herein, you agree to pay Cook Portable Warehouses its reasonable attorney's fees
and cost of suit, in addition to any and all other sums due.
I The Rent-To-Own program has been explained. The customer understand that the term of the
agreement is for 36 months and that 55% of each rental payments (excluding taxes, fees and
other charges) will be applied towards the purchase price if the option to purchase is exercised.
J Any permits and/or compliance of local or state regulations as well as the anchoring of the
building are the customer's responsibility.
K The customer agrees to give the delivery driver a clear and unobstructed ingress and egress to
the delivery location.
L The customer releases Cook Portable Warehouses and its agents or employees from any and all
claims of damages to property (e.g. shrubbery, trees, lawn, fences, driveways, septic tank,
overhead, underground utilities, vehicles, children's toys, etc...), and any other personal
property associated with moving a portable storage building on-and-off the customer's

PERMISSION OF DELIVERY
As the undersigned has guaranteed, in writing, clear and unobstructed ingress and agrees to the
delivery location, the undersigned hereby releases Cook Sales, Inc. and its agents or employees
from any and all claims of damages to shrubbery, trees, lawn, septic tank, overhead and
underground utilities, and or other personal property associated with installing a portable storage
building on the undersigned's premises and agrees to hold Cook Sales, Inc. harmless for any
claim of damages by reason of such installation.
In addition, if. at the insistence of the undersigned, the driver attempts to install the portable
storage building and becomes stuck, the undersigned agrees to pay any and all associated costs,
including but not limited to tow truck fees.
Upon delivery all Cook buildings are leveled by the driver. I understand the site where the
building is to be placed must be prepared and ready for the building at the time of delivery, as
per question B on the order form. This could possibly result in an additional charge. The
building could become un-level due to the door to be hard to open. Cook will be glad to tell you
how to re-Ievel the building. If Cook comes to re-Ievel the building it could result in a re-
leveling fee. Installing building on cement blocks is an extra cost.
I understand that it is against Cook policy to put a building being bought on Rent- To-Own
behind a fenced in area unless there is a gate wide enough without taking part of the fence down
to get truck and trailer inside. The building must be accessible at all times.
I understand if for any reason the Cook building should be picked up that I must make it
accessible and if for any reason the building needs to be moved-it must be moved by Cook.
I also acknowledge and understand that the obtaining and completion of any permits and or
compliance of local or state regulations as well as the anchoring of the building are my
responsibility.
I understand Cook will not cross any property other than mine unless it is a cash sale and Cook
has permission from the other said property owner.

reviewed and approved 88 being in compliance with the 2004 Florida Codes and
Standards, with 2006 supplement, subject to the following limitations:
1. Approval covers factory-built Stroctu re only.
2. Items installed at the site are subject to review, approval, and
inspection by the local authority having Jurisdiciton.
3. The Chapter 833 Plan Review and 'napection shall be
conducted by the local fire safety inspector.
