prevention and protection related activities such as inspections, plan review, administrative fees, and other
costs related to the aforementioned.
Complete Plans, Specifications and Fee Must Accompany Application. Commencement of work without written approval of
the Fire Department's Fire Marshal or required permits or opening up for commercial activity without an approved final
inspection shall be charged double permit fee per day of operation or a minimum of $100.00, whichever is greater. All
work shall be performed in accordance with City Codes and Ordinances.

UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired :a-contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division-Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or contra9tors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco

CONSTRUCTION LIEN LAW (Chapter713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law-Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and
that all work will be done in compliance with all applicable laws regulating construction, zoning and land
development. Application is hereby made to obtain a permit to do work and installation as indicated. I certify
that no work or installation has commenced prior to issuance of a permit and that all work will be performed to
meet standards of all laws regulating construction, County and City codes, zoning regulations, and land
development regulations in the jurisdiction. I also certify that I understand that the regulations of other
government agencies may apply to the intended work, and that it is my responsibility to identify what actions I
must take to be in compliance.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered abandoned.

Plan Review Comments
I have reviewed and approved the plans for a fire alarm system located at 5216 7th St.
(Tourist Club) under the following conditions. My comments have been placed below.
Please contact me if you have any questions with regards to my comments.
1. Alarm shall be compliant with NFP A 72, 2007 edition.
2. Provide a zone map located at the alarm panel.
3. When the alarm is silenced, strobes shall remain flashing.
4. Label detectors and pulls to coincide with the alarm panel.
5. Documentation shall be provided at acceptance test to show monitoring company
is a listed central station.
Inspections required:
1. Acceptance test. (includes pulls, detectors, audible/visual, ducts, fire/smoke
doors, telco, battery, penetrations)

The E3 Series™ Expandable Emergency Evacuation Sys-
tem by Gamewell-FCI is in the forefront of the latest gener-
ation of fire alann control panels. Employing the new high-
speed Velociti™ sensors, the E3 Series provides previ-
ously unattainable polling speed and response together
with the flexibility demanded by today's emergency evacu-
ation systems. In addition to their high-speed polling rate,
the Velociti™ series of sensors feature bi-polar LEOs that
flash green for normal polling, and light red steadily to indi-
cate an alann.
The E3 Series is equipped with an 80-character LCD-E3
alphanumeric: LCD display that allows 40 characters to be
user-defined for customizing installations. Up to six (6),
keyboard LCD displays may also be remotely located in
addition to five of the familiar LCD-7100 remote displays,
allowing for instant system status information to be avail-
able in any desired area of an installation.
A high-speed 32-bit processor easily tackles a wide array
of applications from small office buildings to sophisticated
high-rise installations.
54-node networking is made possible by 625K baud/ARC-
NET communications using twisted-pair copper cable,
fiber-optic cable, or a combination of both.
The basic E3 Series is equipped with an ILI-MB-E3 Intelli-
gent Loop Interface-Main Board, Intelligent Loop Interface
Expansion Board, and ASM-16 Addressable Switch Mod-
ule that featu res sixteen (16), software programmable
switches, each accompanied by red, green and yellow
LEOs that can be programmed to indicate operation of the
switches. Additional ASM-16 mOdules may be added to
expand the operation to a plateau previously unimagined.
The Intelligent Loop Interface - Expansion Board (ILI-S-E3)
provides the E3 Series control panel with two (2), additional
signaling line circuits. The layout is similar to the ILI-MB-E3
with the exception that a number of components are omit-

Each ILI-MB-E3 can support as many as sixteen (16).
ANU-48 LED Driver modules supporting hundreds of LEDs
on a 3rd party graphic annunciator for remote annuncia-
tion. The ANU-48 modules may be installed in any Listed
remote annunciator. It can be remotely located via an RS-
485 serial interface.
Cabinet B contains a space for the ILI-MB-E3, PM-9 mod-
ules and batteries set inside the backbox. Additional mod-
ule options mounted on the back:box include the DACT-E3,
RPT-E3 and ILI-S-E3. The 2-bay inner door houses one
(1), LCD-E3 module and one (1), ASM-16 module.

Installation
The E3 Series expandable emergency evacuation system
offers four (4), cabinet size options. A typical cabinet
includes a backbox, an inner door, and an outer door. The
E3 Series cabinet assembly is a compact 193/8" (49 em)
wide wall-mounted enclosure.
Cabinet A includes the following four options:
· Cabinet A 1 inner door mounted to the backbox. The
backbox houses one NGA module.
· Cabinet A2 inner door mounted to the backbox. The
backbox houses one LCD-E3 module.
. Two or three-bay inner door mounted to the backbox.
The backbox typically houses one (1) LCD-E3, or one
(1) NGA. and one (1) or two (2), ASM-16 modules.

sub-assembly with sixteen (16), switches and
forty-eight (48), status LEDs and may. be remotely
located via the RS-485 serial interfacE!. Each
ASM-16 addressable switch module has sixteen
(16), push-button switches that can be
programmed to serve any function the application
demands.

The ASM-16 Addressable Switch Module is a
component of NetSOLOTM and E3 Series™ fire
alarm and voice evacuation systems. It serves as
the point of interface between an operator and the
system's audio evacuation, fire fighter intercom,
and building control circuits.

An ASM-16 switch can be programmed as a
speaker circuit switch, fire fighter phone switch, an
auxiliary control switch using a bank of three (3),
switches (one switch each for on-off-auto
functions), or switches with custom defined
functions (e.g. system reset, system silence,
system acknowledge, all-call, phone patch, lamp
test, alarm tone on, manual select, etc.).
Each ASM-16 switch also has three (:I), fully
programmable LEOs in red, yellow, and green.
These LEOs can be programmed to work in
concert with their associated push-button switch or
they can be made to work independently as status
indicators (e.g. on, off, normal, etc.).
. Each INI-VGC supports up to sixtE!en (16),
ASM-16 switch modules for a total of 256
switches.
· Each ILI-MB-E3 supports up to sixteen (16),
ASM-16 switch modules for a total of 256
switches.
· Each ASM-16 switch has three (3), fully
programmable status, indicatin!) LEOS: red,
yellow, and green.
· All switch functions are fully software
programmable.
· Slip-in label inserts allow easily modified switch
designations.

An INI-VGC assembly or ILI-MB-E:3 can

The Intelligent Loop Interface-Main Board (/L1-MB-E3) is
the main interface for the E3 Series™ product line. With its
state-of-the-art 32 bit RISC processor, this compact "panel
on a board" provides a powerful addition to Gamewell-
FCl's single pair conductor solutions. This intuitive design
provides two (2), signalin';J line circuits, two (2), notification
application circuits, local energy city box output, auxiliary
relay functions, and auxiliary power output. These features,
combined with built-in nE,twork and serial protocols, allow
this module to support a host of new and existing products,
resulting in a building block approach to fire alarm panel
design.

Features
The ILI-MB..E3 is network ready and occupies one of 64
nodes, operating at 625K baud. When this sub-assembly is
integrated with proven Broadband components, the result
is a flexible yet powerful integrated audio solution. When
transmitting to remote locations, the optional RPT-E3 pro-
vides the ILI-MB-E3 with valuable signal boosting and tran-
sient protection, as well as connectivity using both wire and
fiber-optic cables.

face can support up to a total of sixteen (16), ASM-16 and/
or sixteen (16), ANU-48 remote LED driver modules.
The ILI-MI3-E3 relay outputs include system alarm, super-
visory, and system trouble contacts. The ILI-MB-E3 pro-
vides output I'or a local energy city master box or remote
location which is non power-limited. All other wiring is
power-limited.

Description
The Digital Alarm Communicator Transmitter (DACT-E3) is
an optional component of the E3 Series 1M Expandable
Emergency Evacuation System. The DACT-E3 sends digi-
tal signals over the telephone network to a central station.

PM-9 Power Supply
The PM-9 is a switching power supply that provides 9
amperes of filtered and regulated 24 VDC (nominal). It has
an intemal battery charging circuit capable of maintaining
up to fifty-five (55), A1H batteries. This module is designed
for use with the Gamewell-FCI distributed audio networks.
The PM-9 Power Supply is a component of NetSOLO@

Description
The FCI MS-7 Style manual fire alarm stations are
available in a wide variety of configurations. They
meet the requirement of the Americans with Disabili-
ties Act (ADA) of a 5-lb. maximum pull force to acti-
vate, Operating instructions are molded into the
handle along with Braill,e text. All stations have a key
lock/reset which is keyed alike with FCI fire alarm
control panels and other manual fire alarm stations.
MS-7AF Velociti Addrl~ssable Station
The MS-7AF Velociti™ Series addressable station is
a double action station designed for installation in
the signaling line circuit of FCI analog addressable
control pcmels. Activation of the station causes its
assigned address to r,egister at the control panel.
The door contains an LED which flashes green in
normal condition and lights steadily red when the
station has been activated:
The station features screw terminals.
The Velociti™ Series stations use a communication
protocol that substantially increases the speed of
communication between the sensors and certain
FCI analog addressable tire alarm controls. These
devices operate in a grouped fashion. If one of the
devices in the group has a status change, the
panel's microprocessor stops the group poll and
concentrates on the single device. The net effect is
response speed up to five times gn:ater than earlier
designs.
MS-7A Addressable Station
The MS-7A Addressable Station is a double action
station designed for installation in the signaling line
circuit of any FCI analog addressable control panel.
Activation of the station causes its assigned address
to register at the control panel.
The door contains an LED which flashes red in nor-
mal condition and lights steadily when the station
has been activated. Thl3 station features screw ter-
minals.

The MS-?L08 station must be mounted on a
Model S8-1I0 backbox, In retrofit applications, tile
station is UL Listed for use with the WP-10 back-
box. It is intended for use with conventional control
panels and has a set of single pole contacts and
screw terminals.

mounted (use backbox S8-1I0) or semi-flush
mounted on a standard double-gang, or 4-inch

Description
The Gamewell-FCI Velc>citi™ Series, analog addres-
sable plug-in smoke sensors with integral communi-
cation provide features that surpass conventional
sensors. Sensitivity can be programmed in the con-
trol panel software, and is continuously monitored
and reported to the panel. Point ID capability allows
each sensor's address to be set, providing exact
locations for selectiv,e maintenance when the cham-
ber contamination n3aches an unacceptable level.
The ASD-PL2F photoelectric sensor's unique optical
sensing chamber is engineered to sense smoke pro-
duced by .a wide range of combustion sources. Dual
electronic thermistors add 1350F (570C) fixed-tem-
perature thermal sensing on the ASD-PTl2F model.
The Velociti™ Series use a communication protocol
that substantially increases the speed of communica-
tion between the sensors and certain Gamewell-FCI
analog addressable fire alarm controls. These
devices operate in a grouped fashion. If one of the
devices in thl3 group has a status change, the panel's
microprocessor stops the group poll and concen-
trates on the single device. The net effect is a
response speed up to f'ive times greater than earlier
designs.

Description
Addressable Thermal Sensor
The Gamewell-FCI Ve:lociti™ Series, addressable plug-in
thermal sensors with integral communication provide fea-
tures that surpass conventional sensors. Point 10 capabil-
ity allows Elach sensor's address to be set, providing exact
locations for pinpointing alarm locations and for selective
maintenance. ATO thermal sensors use an innovative
thermistor sensing circuit to produce 1350F/570C fixed-
temperature (ATO-L2F). The ATO-RL2F provides a combi-
nation 15"/minute rate-of-rise with 1350 fixed thermal
detection that is included in a low-profile package. The
ATO-HL2F provides fixed high-temperature detection at
1900F/880C. These thermal sensors provide cost-effec-
tive, addressable property protection in a variety of appli-
cations.

SpectrAlert@ Advance selectable-output horns, strobes and
horn/strobes are rich with features guaranteed to decrease
installation times and maximize profits. The SpectrAlert
Advance series of notification appliances is designed to
simplify your installations, with features such as: plug-in
designs, instant feedback messages to ensure correct
installation of individual devices, and eleven (11), field-
selectable candela settings for wall, ceiling strobes and
horn/strobes.
More specifically, whem installing Advance products, first
attach a universal mounting plate to a four-inch square,
four-inch octagon, or double-gang junction box. The two-
wire mounting plate attaches to a single-gang junction box.
Then, connect the notification appliance circuit wiring to the
SEMS terminals on the mounting plate.
Finally, attach the horn, strobe, or horn/strobe to the
mounting plate by inserting the producfs tabs in the mount-
ing plate's grooves. The device will rotate into position,
locking the product's pins into the mounting plate's termi-
nals. The device will temporarily hold in place with a catch
until it is secured with a captured mounting screw.
SpectrAI'ert Advance products offer the following
options:
. 12 or 24 volts.
. At 24 volts, 15, 15/75,30,75,95,110, 115, 135, 150,
177, or 185 candela bll way of rear-mounted slide
switch and front viewing window.
. Horn tones and volume by way of rotary switch.
The SpectrAlert Advance series includes outdoor notifica-

Engineering Specifications
Outdoor Products
SpectrAlert Advance outdoor horns, &trobes and horn!
strobes shall be listed for outdoor use by UL and shall
operate between -40oF and '1510F (-40oC and 660C). The
products shall be listed for use with a System Sensor out-
door/weatherproof backbox with half-inci1 and three-
fourths-inch conduit entries.
SpectrAlert Advance hOrnS, strobes, and horn/strobel; shall
mount on a standard 4.0" x4.0" x 1.5" (10.16 x 10.16 x 3,81 em)
backbox, 4.0" (10.16 em) octagonal backbox, or a double-
gang backbox. Two-wire products shall also mount on a
single-gang 2.0" x 4.0" x 1.875" (5.08 x 10,16 x 4.763 em) back-
box. A universal mounting plate shall be used for mO'Jnting
ceiling and wall products. The notification appliance circuit
wiring shall terminate at the universal mounting plate, Also,
SpectrAlert Advance products, when used with the
Sync.Circuit™ Module accessory, shall be powered from a
non-coded notification appliance circuit output and shall
operate on a nominal 12 or 24 volts. When used with the
Sync-Circuit Module, 12-volt rated notification appliance

Synchronization Module
The module shall be a System Sensor S:fnc.Circuit
Listed to UL STD 464 and shall be approved for
fire protective service. The modul,e shall synchronize Spec-
trAlert strobes at 1 Hz and horns at Temporal 3. Also, while
operating the strobes, the module shall silence the horns
on horn/strobe models over a single pair of wires. The
module shall mount to a 4.688" x 4.688" x 2.125" (11.906 x
11.906 x 5.398 em) backbox. The module shall also control
two Style Y (Class B) circuits or one Style Z (Class A) cir-
cuit. The module shall synchronize multiple zones. Daisy-
chaining two or more synchronization modules together will
synchronize all the zones the modull~S control. The module
shall not operate on a coded power supply.
Strobe
The strobe shall be a System Sensor SpectrAlert Advance
Model Listed to UL STD 1971 and shall he
approved for fire protective service. The strobe shall he
wired as a primary-signaling notification appliancEl and
comply with the Americans with Disabilities Act require-
ments for visible signaling appliances, flashing at 1 H;~ over
the strobe's entire operating voltage range. The Strobi3 light
shall consist of a xenon flash tube and associated lens!
reflector system.
