UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be Iicersed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the ownerl and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are unc,rtain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division-Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or Icontractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which Ithey will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly lifensed and is not entitled to permitting privileges in Pasco
County. I
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RES~URCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fee may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, a specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impac I Fees and Resource Recovery Fees must be paid prior to
receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit iss~ance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in acchrdance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes~as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a co y of the "Florida Construction Lien Law-Homeowner's
Protection Guide" prepared by the Florida Department of Agri ulture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of he above described document and promise in good faith to
deliver it to the "owner" prior to commencement. ~
CONTRACTOR'S/OWNER'S AFFIOA VIT: I certify that all th information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating onstruction, zoning and land development. Application is
hereby made to obtain a permit to do work and installatioljl as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, anb land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other governm~nt agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in c mpliance. Such agencies include but are not limited to:

_ Use of fill is not allowed in Flood Zone "V" unless expressly permitted.
If the fill material is to be used in Flood zonel "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida. :
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fil the area within the stem wall.
If fill material is to be used in any area, I cert fy that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affe~t adjacent properties, the owner may be cited for violating
the conditions of the building permit issued unde the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drai age plan is required.

A Delivery includes a one time building placement and leveling (up to 6 inches).
B If the customer requires the delivery driver to perform duties that make the delivery longer
than 45 minutes, the customer agrees to pay a charge of $40.00 per hour.
C The customer may incur an attempted delivery fee if the answers to questions 1-5 above
prove inaccurate and the driver is dispatched but cannot deliver the building.
D The customer must pay all deposits, fees, delivery costs, and applicable taxes before the
scheduling of Lease with option to purchase orders.
E The customer must pay a non-refundable deposit (amount to be determined by
management) on special built buildings prior to the order being processed for construction.
F Cook Portable Warehouses carry a lifetime warranty against termite damage and decay.
Therefore, it is not necessary to place them on blocks. However, at the customer's request
at the time of placing this order, the driver will block the buildings no more than 1 concrete
block high provided the customer has the blocks on hand at the time of delivery. There is a
charge of $30.00 for buildings up to 16' and $40.00 for buildings up to 24'. Buildings
longer than 24' must be placed on level ground.
G The Lease with option to purchase program has been explained. The customer understand
that the term of the agreement is for 36 months and that 55% of each rental payments
(excluding taxes, fees and other charges) will be applied towards the purchase price if the
option to purchase is exercised.
H The customer understands that all rental payments are due on the last day of each month.
Also, if the building is delivered between the 1st and t'he 15th of the month, the first rental
payment is due by the last day of the same month. If the building is delivered between the
16th and the last day of the month, the first rental payment is due by the last day of the

RELEASE FORM - DELIVERY
As the undersigned has guaranteed. in writing, dear and unobstructed ingress and egress to the
deHvery locations) the undersigned hereby releases Cook Portable Warehouses and it's agents
or employees from any and all claims of damages to shrubbery, trees, lawn, septic tank,
overhead and underground utilities, anclior other personal property associated vvith installing a
portable storage building on the undersigned's premises, and agrees to hold Cook Portable
\JvTarehouses harmless for any claim of damages by reason of such installation.
If the undersigned has a fence around the delivery location, or installs a fence after del1very, the
undersigned agrees to make the property accessible in the case of a default on the lease and/or
Cook Portable Warehouses must rettieve the building. The undersigned also hereby grants
pel1l1ission for Cook Portable Warehouses to remove the fence or similar structures to gain
access to the portable storage building and, in doing so, agrees to hold Cook Portable
Warehouses harmless for any claim of damages by reason of such removal.
[n addition, if, at the insistence of the undersigned, the driver attempts to install the portable
storage building and becomes stuck, the undersigned agrees to pay any and aU associated costs,

Standards, with 2006 supplement, subject to the following limitations:
1. Approval covers factory-built structure only.
2. Items installed at the site are subject to review, approval, and
