, NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit may be subject to "deed" restrictions"
which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance with any
applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division-Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law-Homeowner's
Protection Guide" prepared .by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other govemment agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
Department of Environmental Protection-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, WaterlWastewater Treatment.
Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
Army Corps of Engineers-Seawalls, Docks, Navigable Waterways.
Department of Health & Rehabilitative Services/Environmental Health Unit-Wells, Wastewater Treatment,
Septic Tanks.
US Environmental Protection Agency-Asbestos abatement.
Federal Aviation Authority-Runways.
I understand that the following restrictions apply to the use of fill:
Use of fill is not allowed in Flood Zone "V" unless expressly permitted.
If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the BUilding Official from thereafter
requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered abandoned.

Require A Six Nail Fastening Pattern?
Use the directions above with the following modification:
Replace the inner two fastening locations with four fastening placements.
Place two fasteners 8" (203mm) from either end +/- 1" (25mm), and place
two additional fasteners 15" (381mm) from either end +/- 1" (25mm).
GENERAL INSTRUCTIONS
. ROOF DECKS: For use on new or reroofing work over well-seasoned, supported wood deck,
tightly-constructed with maximum 6" (152mm) wide lumber, having adequate nail-holding capac-
ity and smooth surface. Plywood decking as recommended by The Engineered Wood Assn, is
acceptable Where a Class A rating IS mquired over decks less than 15/32" thick, an underlayment
is required. 00 not fasten shingles directly to insulation or insulated deck unless authorized in writ-
ing by GAFMC. Roof decks and existing surtacing material must be dry prior to application of shin-
gles.
. UNDERLAYMENT: Underlayment beneatll stlingles lias many benefits, II1cluding preventing
wind driven rain from reacl1ing tile interior of the buildll1g and preventing sap in some wood deck-
ing from reacting with asptlalt sllingles. Underlayment is also requirelf by many code bodies.
Consult your local building department for its requirements. Where an underlayment IS to be
installed, a breather-type underlayment such as GAFMC's Silingle-Mate" underlayment is recom-
mended. Underlayment must be installed flat, without wrinkles.
. FASTENERS: Use of nails (rattler Ulan staples) is recommendelt. Use oniy Zinc coated steel or
alurl1lnlJm, 10-12 gauge, barbed, aeforrnea or srnooth stlank roofing nails with heads 3/8" (10mm)
to 7/16" (12mm) in diameter. Fasteners Sllouid be long enougn to penetrate at least 3/4" (19mm)
into wood decks or Just through fhe plywood decks. Fasteners must be driven flush with the sur-
face of the stlingle. Over driving willllamage the stlingle. Raised fasteners 1'1111 interfere with the
sealing of the stlingles. If tire temperature IS below 40' F or In areas where alfburne dust or sand
can be expected before sealing occurs sllingles must be hand sealed (see WIND
RESISTANCE/HAND SEALING).
.WIND RESISTANCE/HAND SEALING: Tlwse shingles have a special tlwrrnal sealant trlat
firmly bonds the sllinCJles togethor atter application wilen exposed to sun and warm tempera-
twes. Shingles instalied in Fail or Winter may not seal until the followlllg Spring. If shingles are dam-
agert by Wifllls before sealrng or are not exposed to adequate surface temperatures, or if the self-
seaiant gets dlrly, tile sl11ngle5 may never seal. Failure to seal unrJer these circumstances results
from the nature of self-sealing shingles and is not a manufacturing defect.
To insure immediate sealing, apply 4 quarter-sized dabs of shingle tab adhesive on the back of
the shingle 1" (25mm) and 13" (330m'll) in from eacll side and 1" (25m'll) up from bottom of the
shingle. The shingle must be pressed firmly into the adhesive.
NOTE: Application of excess tab adheSive can cause blistering of the shingle.
For maximum wind resistance along rakes, cement shingles to underlayment and each other in
a 4" (102m'll) width of asphalt plastic roof cement.
NOTE: The tilm strips on the back of each shingle are to prevent sticking together of tile shingles
while 1f1 the bundle. Their removal is NOT required during application.
. CANADIAN COLD WEATHER APPLICATIONS: CSA 123.5-M90 mandates tl1at sllin-
gles applied between September 1 and April 30 shall be adhered with a compatible field-applied
adhesive. See Wind Resistance for GAF Materials Corporation's recommendations for the appli-
cation of that adheSive.
. MANSARD AND STEEP SLOPE APPLICATIONS: For root slopes greater than 21"
(1750mm/m) per foot (do NOT use on vertical Side walls), shingle sealing must be enhanced by
hand scaling. After fastening the shingle in place, apply 4 Quarter-sized dabs of shingle tab adhe-
sive as indicated in Wlild Resistance above. The shingle must be pressed firmly into the adhe-
sive.
. EXPOSURE: 5" (127mm)
. THROUGH VENTILATION: All roof structures must be provided with through ventilation to
prevent entrapment of moisture laden air bel1ind roof sheathing. Ventilation proviSions must at
least meet or exceed current F.H.A., H.U.D. or local code minimum requirements.
. NON-CORRODING METAL DRIP EDGES: Recommended along rake and eave edges on
all decks, especially plywood decks.
. ASPHALT PLASTIC CEMENT: For use as shingle tab adhesive. Must conform to ASTM
D4586 Type I or II.

characterrstics of the high QUality waterproofing material used, these shingles will be stiff in cold
weather and fiexibie In hot weather.

If oid asphalt sTiingles are to rema<n in plaee, nail down or cut away all loose, curled or lifted shin--
gles; replace with new; and just before applying the new roofing, sweep the surface clean of all loose
debris, Since any irregularities may show through the new shingles, be sure the underlying shingles
provide a smooth surface. Fasteners must be of sufficient length to penetrdte the wood ,jeck at least
3/4" (19mm) or lust through plywood. Follow other above instructions for application.
Note: Shingles can be applied over wood shingles when precautions have been taken to prOVide
an acceptabie smooth surface, This includes cutting back old shingles at eaves and rakes and
Installing new WOOlf edging strips as nee,fed. Make surface smooth and use beveied wood strips
if necessary. Install #30 underlayment to maintain Class A rating.

Qualitv Assurance Documentation:

Installation Requirements:
1. GAF Modified Bitumen Roof Systems shall be installed in accordance with GAF published installation
instructions, subject to the Limitations / Conditions of Use noted below.
2. System attachment requirements for wind load resistance are set forth in Appendix 1.
Limitations / Conditions of Use:
1. This Evaluation Report is not for use within the HVHZ.
2. Refer to a current Approved Roofing Materials Directory for surfacing requirements and fire rating.
3. For steel deck installations, foam plastic insulation shall be separated from the building interior in
accordance with FBC Section 2603.4 unless the exceptions stated in FBC Sections 2603.4.1 and
2603.8 apply.
3. Unless otherwise noted in Appendix 1, roof decking and its attachment shall be specified and
installed to meet project design criteria to the satisfaction of the AHJ.
4. For recover installations, the existing roof shall be examined in accordance with FBC Section 1510.
5. For mechanically attached components, the maximum design pressure for the selected assembly
shall meet or exceed the Zone 1 design pressure determined in accordance with FBC Chapter 16.
Zones 2 and 3 shall employ a tighter component attachment density designed by a qualified design
professional to resist the elevated pressure criteria. Commonly used methods are RAS 117 and FM
LPDS 1-29.
6. For adhered insulation, the maximum design pressure for the selected assembly shall meet or
exceed critical design pressure determined in accordance with FBC Chapter 16. No rational analysis
is permitted for these systems.
7. For mechanically attached com ponents over existing roof decks, fasteners shall be tested in the
existing deck for withdrawal resistance. A qualified design professional shall review the data for
comparison to the minimum requirements for the system. Testing and analysis shall be in
accordance with TAS 105.
8. For adhered insulation over existing substrates, the existing deck or roof surface shall be examined
for compatibility with the membrane or insulation adhesive to be installed. If any surface conditions
exist that bring system performance into question, field uplift testing in accordance with ASTM E907
or FM LPDS 1-52 shall be conducted on mock-ups of the proposed new roof assembly.
9. For recover installations, the existing roof system shall be capable of resisting project design
pressures on its own merit to the satisfaction of the AHJ, as documented through field uplift testing
in accordance with ASTM E907 or FM LPDS 1-52.
