CONSTRUCTiON LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law-Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "oWher", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
Department of Environmental Protection-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands; Water/Wastewater Treatment.
Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
Army Corps of Engineers-Seawalls, Docks, Navigable Waterways.
Department of Health & Rehabilitative Services/Environmental Health Unit-Wells, Wastewater Treatment,
Septic Tanks.
US Environmental Protection Agency-Asbestos abatement.
Federal Aviation Authority-Runways.
I understand that the following restrictions apply to the use of fill:
Use of fill is not allowed in Flood Zone "V" unless expressly permitted.
If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
If the fill material is to be used in Flood Zone "An in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in

justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the Job IS conSidered abandoned.

Chapter 633, Florida Statutes, authorizes the City to charge and collect user fees to pay for the costs of fire
prevention and protection related activities such as inspections, plan review, administrative fees, and other
costs related to the aforementioned.
Complete Plans, Specifications and Fee Must Accompany Application. Commencement of work without written approval of
the Fire Department's Fire Marshal or required permits or opening up for commercial activity without an approved final
inspection shall be charged double permit fee per day of operation or a minimum of $100.00, whichever is greater. All
work shall .be performed in accordance with City Codes and Ordinances.

I have reviewed the plans for fire alarm installation for the sprinkler monitoring located at
6834 Gall Blvd. The plans have been approved based on receiving your letter stating
you would abide by the following items listed below.
Please contact me if you have any questions with regards to my comments.
1. Add (2) two prewired horn/strobes with a pull in the future build-out area. These
shall be located near the front door. This was discussed at the site meeting earlier.
This will provide notification for those tenants.
2. Strobes shall remain on when system is silenced.
3. System shall be addressable, therefore pull locations, smoke location, ansu\ and
flows/tampers shall be identified.
4. Provide an extra key for the pull station to be placed in the Knox Box.
5. A zone map shall be provided at the panel indicating location of all devices.

GENERAL
The Fire-Ute MS-5024 is a 24-volt, five-zone fire alarm
control panel with built-in communicator. The integral com-
municator transmits event information (alarms, troubles,
supervisories, faults, etc.) to a UL-Listed central station.
The MS-5024 may be used in a variety of cost-effective
applications, including: local fire alarm control panel (com-
municator disabled), fire alarm control panel/communica-
tor, stand-alone sprinkler supervisory communicator or
slave communicator to a UL-Listed host fire alarm panel
(requires N.O. dry contacts to trigger five channels). Se-
lectable transmission formats allow the MS-5024 to com-
municate to virtually all central stations.

Initiating Device Circuits (zones)
The MS-5024 includes five programmable Initiating Device
Circuits (zones). Circuits 1, 2, 4, and 5 are Style B (Class
B). Circuit 3 is Style 0 (Class A). Use the CAC-5F, Class A
converter module, to convert all Class BlOCs to Class A.
Each zone may support two-wire smoke detectors. Pro-
gramming options include:
. Waterflow-Silenceable/Non-Silenceable.
. Supervisory-Standard/Auto_Reset.
. Pull Station.
. Normally Open Contact device.
All circuits are power-limited and also fully supervised. They
allow up to 100 ohms of line resistance and allow for use of
12 to 18 AWG (3.1 to 0.78 mm2) wire.
Notification Appliance Circuits
. Two Style Y (Class B) @ 1.5 amps each.
. All circuits are fully power-limited and meet UL power-
limiting requirements using fuseless technology.

Horns, Strobes, and Horn/Strobes
GENERAL
Section: AudioNisual Devices
System Sensor SpectrAlert Series strobes, horns, and combina-
tion horn/strobes are UL listed for primary signaling in life safety
systems and meet ADA public mode visible signaling requirements.
SpectrAlert products can be connected to the alarm indicating cir-
cuit of a fire alarm control panel and are compatible with DC line
supervision. The SpectrAlert product line mounts to standard
backboxes using a universal mounting plate included with each unit.
An optional small footprint mounting plate fils to a single-gang box.
An accessory backbox skirt gives a cosmetic finish to a 4' x 4" x
1-1/2' or a 2' x 4' x 1-7/8' surface-mounted backbox. All strobe
and horn/strobe mounting options require only one screw attach-
ment of product to plate.
These products are designed for 12 and 24 VDC and full-wave rec-
tified unfiltered power. Full-wave rectified operation requires more
current than DC operation. For detailed current draw information,
consult the Current Draw Tables (page 2). The horn/strobe com-
bination products are factory-assembled with jumper wires for in-
tandem operation. For independent wiring of horn and strobe, re-
move jumper wires. When wired for independent operation, the
strobe will continue to run while the horn can be silenced. How-
ever, the strobe must be running for horn to operate.
Horns - The SpectrAlert Series horns and horn/strobes pro-
vide two different field-selectable/reversible tones, a high-low field-
selectable/reversible sound output setting (low setting on 24-volt

are accomplished using pins and jumpers located on the back of
each SpectrAlert horn and hom/strobe. An accessory module is
not needed to make these field selections. The hom on horn/strobe
models will operate on a coded power supply. Those horn-only
models with "He" in their part numbers will a/so operate on a
coded power supply. The hom and horn/strobe series includes
weatherproof models.
Strobes - The ADA-compliant SpectrAlert strobes are elec-
tronic visible warning signals that flash at 1 Hz over their operating
vDltage range. These products are available in 24-volt models at
15, 15n5, 75 and 110 candela intensities and in 12-volt models at
15 and 15n5 candela intensities. The strobe series includes weath-
erproof models. SpectrAlert products feature dramatic reductions
in current requirements.
Sync-Circuit Module (MOL) - The Sync-Circuit Mod-
ule is available for synchronization of strobes and horns and can
synchronize two Style Y (class B) circuits or one Style Z (class A)
circuit. The module can also generate a synchronized temp 3 tone
for System Sensor's Multi-Alert™ and PA400 horn products: The
synchronization module allows the SpectrAlert horns on combina-
tion horn/strobes to be silenced on two-wire systems. SpectrAlert's
Sync-Circuit Module can be daisy-chained for multiple zone syn-
chronization. The module does not operate on a coded power sup-
ply.
'For Multi-Alert and PA400: Strobes must be wired to a continuous
source of power (non-coded power supply).

GENERAL
System Sensor 1 00 Series low-profile detectors use state-
of-the-art sensing chambers to meet all applicable UL per-
formance criteria. The backs of the smoke detectors are
sealed a~ainst back pressure, air flow, and dirt. A fine mesh
screen also protects the chamber against the entry of in-
sects. These detectors are intended for open area protec-
tion and for use with UL-listed control panels.
PhotoelectriclThermal_ The unique design of the
optical sensing chamber in 100 Series photoelectric smoke
detectors can sense smoke particles from a wide range of
combustion sources. These detectors minimize nuisance
alaons by using a custom integrated circuit for signal pro-
cessing.

satisfy NFPA 72 sensitivity testing requirements.
Larger plug-in teoninal block with captured SEMS screws
decreases wiring and installation time.
