Thi~ is to advise that Verizon will submit an application for a permit 15 days from the above date to the City
of Zephyrhills to place 1 - 1.25" HDPE and fiber optic cable by Directional Bore and Trench to facilitate
installation ofFOC along the South side of Green meadow Dr. for a distance of300' onto private property of
GREEN MEADOW APTS. We are enclosing a copy of our sketch. If no response is received within 15 days
