Please understand that money is not the issue and our main concern is following the guidelines established and making sure everyone does the job right thus resulting in a safer business being occupied by citizens in our commu ity..AIsQbe advised that a sticker is located next to the suppression pull station that states a permit is requ prior to completuiig at type of work. However, being that it sounds like you are a new contractor within our a, I will waive the$100 fe This has been forwarded to the person that handles our accounts. With regards to the permit process you do need to pull a back permit. Permits are obtained from the Zephyrhills Building Dept. The permit needs to be pulled by your agency and not the business you are servicing. Applications can be obtained from the City's website then mailed in with a check. No credit card is accepted at this time. It can also be paid for by visiting that department prior to conducting the work on that same day. The process only takes a few minutes. If you have not done any work in our area you will have to register your agency. A workers comp certificate, business license and insurance are require for registering. There is also a small fee for this. I believe $35. You may contact the Building Department and speak with either Jackie or Bobbie and they can assist you with this. Space Coast Fire and Safety is a state licensed fire suppression/fire extinguisher company located in Merritt Island, Brevard County, Fl. We were unaware of the city Ordinance for obtaining a permit before conducting the semi-annual inspection. We just got the contract for this service with two different companies in Zephyrhills. One is Wendy's, and the other is Long John Silvers. We apologize for this oversight. We will pull a "back" permit for this one, and would like to pull one for the other restaurant. Chief would you please consider waiving this $100.00 fine this time? Also 1/5/2009 Page 2 of 2 can we have an "owner/agent?" Pick up and pay for these permits? I can have an owner/agent authorization letter notarized and faxed to your office today. Will you need a W-9 or certificate of insurance? Please let me know. Kerry Barnett 7) — Fax (813) 780-0044 E-mail: kbarnetbcv.fire.zephyrhills.fl.us Space Coast Fire& Safety 420 Manor Drive Merritt Island, FL. 32952 December 23, 2008 On December 23`d, 2008 our department performed an inspection located at 's (5658 Gall Blvd)to find the hood suppression received its semi-annual inspec however there was no permit pulled prior to performing this work. As of October 2007 (Resolution 589-07)the City of Zephyrhills established fees for fire protection permit at a cost of$25.00. Also, there is an orange/red sticker next to the pull stations and/or on the suppression cabinet stating a permit has to be pulled prior to performing the semi-annual. All businesses registered within the City were notified of this via letter prior to this implementation. Under the resolution, Section 3 states that commencement of work without approval of the Fire Department/Fire Marshal or required permits, the subject shall be charged double the permit fee per day or a minimum of$100, which ever is greater. If not done so already, you are still required to back pull the permit for the work already performed. Therefore a$100.00 fee has been assessed to your company for failure of obtaining a permit prior to performing work. Further, a permit shall be back pulled for the suppression semi-annual.
