UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. TRANSPORTATION IMPACT /UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89 -07 and 90 -07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water /Sewer Impact fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances. CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law — Homeowner's Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone other than the "owner ", I certify that I have obtained a copy of the above described document and promise in good faith to deliver it to the "owner" prior to commencement. CONTRACTOR'S /OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. If the fill material is to be used in Flood Zone "A ", it is understood that a drainage plan addressing a "compensating volume" will be submitted at time of permitting which is prepared by a professional engineer licensed by the State of Florida. If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall construction, I certify that fill will be used only to fill the area within the stem wall. If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating the conditions of the building permit issued under the attached permit application, for Tots less than one (1) acre which are elevated by fill, an engineered drainage plan is required. If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work, plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A permit issued shall be construed to be a license to proceed with the work and not as authority to, violate, cancel, alter, or set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extension may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered abandoned. Before final Drawings and Specifications are issued for construction, they shall be submitted to all governing agencies to insure their compliance with all applicable local and national codes. 2. If code discrepancies are discovered during the construction process, the Engineer shall be notified and allowed ample time to remedy sold discrepancies. B. All contractors, subcontractors, suppliers and fabricators shall be responsible for the content of drawings, and for the supply and design of appropriate materials and work performance. C. All manufactured articles, materials and equipment shall be applied, installed, erected, used, cleaned, and conditioned in strict accordance with manufacturer's recommendations. D. Provide soil treatment for termites as required. III. CONCRETE A. Provide reinforcement of the size and spacing shown on drawings. Steel reinforcing shall comply with the following criteria: 1. Truss manufacturer and /or engineer shall be responsible for the design and spacing of all trusses and shall submit shop drawings to the builder and engineer for opprovol. D. All wood plates bearing on, or coming in contact with, concrete or masonry shall be pressure treated or installed over 6 mil polyethylene moisture ba rri er. E. Provide 2" nominal pressure treated lumber over 6 mil polyethylene moisture barrier beneath full bearing of wood beams on concrete or masonry. F. "X" bracing shall be provided at 4' -0" o.c, first three framing spaces each end of gable. PROTECTION AGAINST TERMITES R320.1 Termite Protection. Termite protection shall be provided by registered termiticides, including soil applied pesticides, baiting systems and pesticides applied to wood, or other approved methods of termite protection lobeled for use as a preventative treatment to new construction. See Section 202, Registered termiticide, Upon completion of the application of the termite protective treatment, a Certificate of Compliance shall be issued to the building deportment by the licensed pest control company that contains the following statement: 'The building has received a complete treatment for the prevention of subterranean termites. Treatment is in accordance with rules and laws established by the Florida Deportment of Agriculture and Consumer Services.' R320.1.1 If soil treatment is used for subterranean termite prevention, the initial chemical soil treatment inside the foundation perimeter shall be done after all excavation, bockfilling and compaction is complete. R320.1.2 If soil treatment is used for subterranean termite prevention, soil area disturbed after initial chemical soil treatment shall be retreated with a chemical sot treatment, including spaces boxed or formed R320.1.3 If soil treatment is used for subterranean termite prevention, space in concrete floors boxed out or formed for the subsequent installation of plumbing traps, drains or any other purpose shalt be created by using plastic or metal permanently placed forms of sufficient depth to eliminate any planned soil disturbance after initial chemical soil treatment. R320.1.4 If soil treotment is used for subterranean termite prevention, chemically treated soil shall be protected with a minimum 6 ml vapor retarder to protect against rainfall dilution. If rainfall occurs before vapor retarder placement, retreotment is required. Any work, including placement of reinforcing steel, done after chemical treatment until the concrete floor is poured, shall be done in such manner as to avoid penetrating or disturbing treated sal. R320.1.5 If soil treatment is used for subterranean termite prevention, concrete overpour or mortor accumulated along the exterior foundation perimeter shall be removed prior to exterior chemical soil treatment, to enhance vertical penetration of the chemicals, R3201.6 If soil treatment is used for subterranean termite prevention, chemical soil treatments shall also be applied under ail exterior concrete or grode within 1 foot (305 mm) of the primary structure sidewalls. Also, a vertical chemical barrier shall be applied promptly after construction is completed, including initial landscaping and irrigation /sprinkler installation. Any soil disturbed after the chemical vertical barrier is applied shall be promptly retreated. R320.1.7 If a registered termiticide formulated and registered as a bait system is used for subterranean termite prevention, Sections R320.1.1through R310.1.6 do not apply, however, a signed contract assuring the installation, maintenance and monitoring of the baiting system for a minimum of 5 years from the issue of the Certificate of Occupancy shall be provided to the building official prior to the pouring of the slob, and the system must be installed prior to final building approval. If the baiting system directions for use require a monitoring phase prior to installation of the pesticide active ingredient, the installation of the monitoring phase components shall be deemed to constitute installation of the system. R320.1.8 If a registered termiticide formulated and registered as a wood treatment is used for subterranean termite prevention, Sections R320.1.1 through R320.1.6 do not apply. Application of the wood treatment termiticide shall be as required by label directions for use, and must be completed prior to final building approval. R320.2 Penetration. Protective sleeves around metallic piping penetrating concrete slob -on -grade floors shall not be of cellulose - containing materials and, if sol treatment is used for subterranean termite protection, shall receive application of a termiticide in annular space between sleeve and pipe. R320.3 Cleaning. Cells and cavities in masonry units and air gaps between brick, stone or masonry veneers and the structure shall be cleaned of all nonpreservotive treated or nonnaturally durable wood, or other cellulose - containing material prior to concrete placement. Exception: Inorganic moterial monufactured for dosing cells in foundation concrete masonry unit construction or clean earth fill placed in concrete masonry unit voids below slab level before termite treatment is performed. R320.4 Concrete bearing ledge. Brick, stone or other veneer shall be supported by a concrete bearing ledge of such thickness as required in Chapter 14, which is poured integrally with the concrete foundation. No supplemental concrete foundation pours which will create a hidden cold joint shall be used without supplemental treatment in the foundation unless there is an approved physical barrier. An approved physical barrier shall also be installed from below the wall sill plate or first block course horizontally to embed in a mortar joint. If masonry veneer extends below grade, a termite protective treatment must be applied to the cavity created between the veneer and the foundation, in lieu of a physical barrier. Exception: Veneer supported by a structural member secured to the foundation sidewall as provided in Section 1403, provided at least a 6 inch (152 mm) dear inspection space of the foundation sidewall exterior exist between the veneer and the top of any soil, sod, mulch or other organic landscaping component, deck, apron, porch, walk or any other work immediately odjocent to or adjoining the structure. R320.5 Pressure preservatively treated and naturally resistant wood. Heartwood of redwood, eastern red cedor, and pressure- preservative- treated wood in accordance with the AWPA standards listed in Section R319.1 sholl be considered termite resistant. Pressure preservotively treated wood and naturally termite- resistant -wood shall not be used as o physical barrier unless a barrier can be inspected for any termite shelter tubes around the inside and outside edges and joints of a barrier. R320.5.1 Quality mark. Lumber and ply -wood required to be pressure - preservolive treated in accordance with Section R320,1 shall bear the quality mark of an approved inspection agency which maintains continuing supervision, testing and inspection over the quality of the product and which has been approved by on accreditation body which complies with the requirements of the American Lumber Standard Committee's treated wood program. R320.5.2 Field treatment. Field cut ends, notches and drilled holes of pressure- preservatively treated wood sholl be retreated in the field in accordance with AWPA M4. R320.6 Foam plastic protection. Extruded and expanded polystyrene, polyisocyonurate and other foam plastics shall not be installed on the exterior face or under interior or exterior foundation walls or slab foundations located below grade. The clearance between foom plastics installed above grade and exposed earth shall be at least 6 inches (152 mm). I r o w its floors cel ins and roofs are entire) Exceptions: 1. Buildings where the structure members f a g y of noncombustible materials or pressure - preservative- treated wood. 2. When in addition to the requirements of Section R320.1, an approved method of protecting the foam plastic and structure from subterranean termite damage is used. 3 On the interior side of basement walls. R320.7 Protection against decay and termites. Condensate lines and roof downspouts shall discharge at least 1 foot (305 mm) away from the structure sidewall, whether by underground piping, tail extensions or splash blocks. Gutters with downspouts are required on all buildings with eaves of less than 6 inches (152 mm) horizontal projection except for gable end rakes or on a roof above another roof. R320.8 Preparation of building site and removal of debris. R320.8.1 All building sites shall be groded to provide drainage under all portions of the building not occupied by basements. R320.8.2 The foundation and the area encompassed within 1 foot (305 mm) therein shall hove all vegetation, stumps, dead roots, cardboard, trash and foreign material removed and the fill moterid shall be free of vegetation and foreign material. The fill shall be compacted to assure adequate support of the foundation. R320.8.3 After all work is completed, loose wood and debris shall be completely removed from under the building and within 1 foot (305 mm) thereof. All wood forms and supports shall be completely removed. This includes, but is not limited to: wooden grade stakes, forms, contraction spacers, tub trop boxes, plumbing supports, bracing, shoring, forms or other cellulose - containing material placed in any location where such materials are not clearly visible and readily removable prior to completion of the work. Wood shall not be stored in contoct with the ground under any building. R320.9 Naturally resistant wood. Heartwood of redwood and eastern red cedor shall be considered termite resistant.
