Obtaining permit, the applicant acknowledges complying with the items listed below. Should anyone have any questions, please do not hesitate to contact the Fire Marshal's office. 1. Plans were reviewed with all applicable NFPA codes. Chapter 43 of NFPA 101 is the main section in which it determines the codes to use. Plans submitted do not show the space as it is currently and the usage for the space. In the south end of the unit there is a mezzanine that will actually be classified as a second story due to it covering more than 1/3 of that space. During a walk through it was stated that the space may be used as business/office area. That area needs to be shown in plans and labeled for use. If not used for office space then the building is treated as an assembly for the entire space. The rest of the space also needs to be shown how it is to be used; what area is actual gym equipment, what area is for aerobics /jazzercise, etc... This is needed to determine the correct occupant load, if over 300 persons, additional items would be required. 2. Construct an electrical room/closet that has a UL listed 1 hr rated assembly with a 1 hr rated door where multiple electrical panels are located. (NFPA 101, 8.7) 3. Certified fire extinguishers shall be installed in accordance of NFPA 10 for intended use and shall be located within 75' of travel distance. 4. Panic hardware shall be installed on all exit doors. 5. Change front exit light to combination exit/emergency light. 6. An assembly permit will need to be obtained by occupant. Permit may be obtained at the Building Department. 7. Ensure unit is addressed properly on main door and rear door. 8. Ensure rated walls (adjacent units) are sealed at roof deck and any penetrations are properly sealed to maintain the rating. 9. If A/C units are 2000 cfm's or more, duct detectors will be required with remote key switches to reset units. 2 10. Door by testrooms leading to south end shall have door swing in direction of egress. New plans shall be submitted relating to the above items of concern. Modification can begin while these plans are being corrected and submitted. There will be no additional charge for the review. Inspections Required: 1. Firewall (to ensure it is properly sealed and no penetrations) 2. Final , KERR 1 1, 7V 4-- TT, FIRE MARSHAL ***Please be advised this review of plans submitted is a cursory review to assist the contractor in compliance with applicable fire safety codes. This review is not intended to be a final approval of the submitted plans. It is the contractor's sole responsibility to ensure that the plans are in complete compliance with all applicable NFPA codes and local ordinances. In the event that further examination or site inspection reveals areas of non - compliance, it shall be the contractor's sole responsibility, at their sole expense to bring those areas in compliance. The City assumes no responsibility for the contractor's failure to be in compliance with all applicable NFPA codes and local ordinances. r o I Zephyrhills Fire Rescue 6907 Dairy Road, Zephyrhills, FL 33542 Fire Marshal Bus (813) 780 -0041 Kerry Barnett Fax (813) 780 -0044 E -mail: kbarnett@fire.zephyrhills.fl.us Plan Review #: 09 -087 Project: Interior Remodel Number of Pages: 4 October 15, 2009 I have received and reviewed the revised plans for the interior remodel located at 5939 Gall Blvd and will allow the project to move forward. By paying for permit, contractor acknowledges to comply with the items listed below. Should anyone have any questions, please do not hesitate to contact the Fire Marshal's office. 1. Add smoke detectors on second level through out storage area and in mechanical rm. Add smoke detectors in first floor storage area. Smoke detectors shall be hardwired with battery backup. Smoke detectors shall be tied together. 2. Wall on first floor between nursery and storage shall be a 2 hr rated fire wall. A UL listing shall be submitted to this office for approval. NFPA 101 6.1.14.4.1 (b). Firewall shall be fire caulked at roof deck and labeled. Any penetrations shall be sealed to maintain fire rating. 3. Occupant load is under 300 so there will be no additional requirements with regards to alarm or sprinkler systems. Occupant load calcs for storage is 1 person per 500 sq ft and for the jazzercise area cals are 1 person per 15 sq ft. Nursery is figured at 1 person per 20 sq ft net. NFPA 101 7.3.1.2 Additional fees have been charged because of the additional construction not originally shown on the first set of plans. Nothing was stated about building additional walls. Fees were only for those areas (nursery, jazzercise, small corridor). Fees were set at the standard rate and not at the revised rate which is double the standard rate. 2 Inspections Required: 1. Fire wall screw inspection for each layer added to the wall and then an overall fire wall inspection. 2. Final Illi W/.
