I have received and reviewed the plans for the interior improvements located at 7810 Gall
Blvtl and will allow the project to move forward. Please note that this review does not
eliminate any further requirements as the project continues moving forward. By receiving
Wit, contractor acknowledges to comply with the items listed below. Should anyone
have any questions, please do not hesitate to contact the Fire Marshal's office.
1. Penetrations in fire wall shall be completed to a listed UL design.
2. No storage is allowed with 18" of sprinlder heads.
3. Egress shall be maintained at all times.
4. Gate shall remain open as noted on plans.
5. If fire sprinlder system has to be changed because of obstructions, plans will
need to be submitted to obtain a permit.
6. If fire alarm system changes, plans will have to be submitted to obtain a
permit.
7. Fire extinguishers shall be within 75' of travel distance, typically one in front
and one in the rear satisfies the requirement.

NOTICE OF DEED RESTRICTIONS: The undersigned understands that this permit may be subject to "deed" restrictions"
which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance with any
applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division — Licensing Section at 727 -847-
8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90 -07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water /Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law — Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the "owner" prior to commencement.
CONTRACTOR'S /OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
Department of Environmental Protection - Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment.
Southwest Florida Water Management District - Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
Army Corps of Engineers - Seawalls, Docks, Navigable Waterways.
Department of Health & Rehabilitative Services /Environmental Health Unit - Wells, Wastewater Treatment,
Septic Tanks.
US Environmental Protection Agency- Asbestos abatement.
Federal Aviation Authority- Runways.
I understand that the following restrictions apply to the use of fill:
Use of fill is not allowed in Flood Zone "V" unless expressly permitted.
If the fill material is to be used in Flood Zone "A ", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. t understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered abandoned.

General Notes For Prioritv Walls
1. Encase all vertical and horizontal ducts, pipes, conduits, panels, to match adjacent surfaces,
Extend the furred spaces length of the wall between intersecting walls unless otherwise
indicated. The contractor shall notify the Architect should conflicts arise between furring and
casework, equipment or any other items. Provide access panels painted to match adjacent
surfaces where access is required for valves, dampers or other devices requiring
adjustment. Provide labeled access door where enclosure constitutes part of a fire rated
assembly.
2. The maximum allowable area of individual U.L, listed electrical back boxes permitted in
frre/smoke walls is 16 square inches. With a maximum aggregate of 100 square inches per
100 square feet of wall area. Back boxes on opposite faces of such walls shall be offset a
minimum horizontal distance of 24 inches. Penetrations exceeding the allowable areas the
devices shall be set in a furred space in front of the fire or fire /smoke wall as described in
notes above or provided with a five -sided enclosure as described below.
3. Items recessed in corridor, smoke or fire walls such as recessed toilet accessories, fire
extinguisher and/or hose cabinets, fire alarm signal boxes, electrical and signal cabinets, or
other back boxes which exceed 16 square inches shall be installed in a five -sided enclosure
as indicated on the drawings. Where depth of box exceeds wall thickness, provide framing
as required to accommodate depth as described above.
4. Where ducts, pipes, conduit, bar joists, beams or any other materials penetrate a fire rated
wall or partition, openings shall be tightly sealed. Annular openings in CMU walls shall be
filled with grout. Do not cover penetrations of CMU walls with drywall. Penetrations in metal
stud partitions shall be constructed with 20 ga. Galvanized runner channel sill and header
secured to metal studs, with the space around the perimeter of the penetrating object
packed tightly with compressed mineral fiber insulation. Insulated pipes shall be sleeved
with foam glass insulation. Annular opening between sleeve and pipe shall be sealed with
approved fire and smoke retardant sealer. Sleeves shall extend a maximum of 2" beyond
faces of wall. Refer to partition details and mechanical drawings.
5. Provide proper clearances at fire dampers between the sleeve and stud framing members.
Fire damper sleeves shall be installed and secured in accordance with SMACNA standards.
Fire dampers shall be installed in the plane of the rated assembly.
6. Low voltage wiring which penetrates a smoke or fire /smoke wall Shall be sleeved with
conduit extending not more than 2 inches either side of the wall and securely anchored to
prevent slipping through the wall or damaging the drywall. Remaining openings in the
conduit sleeve shall be filled with tightly compressed mineral fiber safeing insulation and
sealed with acoustical sealant. It is the responsibility of the general contractor to assure
coordination of installation of such systems between vendors and subcontractors. The
contractor shall assure that penetrations for utilities are made with the utmost care and
minimum of damage to the integrity of all partitions and walls, Sleeves and conduit shall be
installed prior to running cables.
7. Furnish access panels in walls and non - accessible ceilings where service or adjustment to
mechanical, plumbing or electrical equipment is required, and at intervals not to exceed 30
feet in non - accessible ceilings or fire membranes adjacent to fire and fire /smoke walls.
Where access panels occur in rated assemblies they shall bear a label rating equal to that
of the assembly. Access panels in gypsum wallboard membranes above non - accessible
ceilings shall be aligned to permit access and shall be sized to permit positioning of a step
ladder through the access panel in the ceiling. Rated access panels in ceilings and
membranes shall be supported from the structure, not the ceiling framing.
8. Coordinate installation of plumbing, vent stacks and conduit to permit installation of
recessed toilet accessories, fire extinguisher cabinets and other recessed equipment.
Where such items are recessed in corridor, smoke or fire /smoke walls they shall be installed
in a five -side enclosure as described above.
9. All wood blocking within the building envelope shall be pressure treated. Isolate salt treated
wood from metal framing with 15# felt and use corrosion resistant fasteners to minimize
galvanic corrosion.
10. Provide acoustical sealant to prevent passage of smoke at penetrations of either
smoke - resistant or fire /smoke partitions. Where fire- safing is used, the acoustical sealant
shall be required only on one side of wall.
Fire Protection Criteria
1. One hour rated corridor partitions shall extend to underside of floor or roof deck.
2. Smoke resistive partitions shall extend to underside of floor or roof deck or underside of
gypsum wallboard membrane frreskin or to 4" above ceiling - Refer to Wall Rating Legends,
3. Smoke barrier partitions shall be constructed uninterrupted, tight from back of exterior wall
and from floor slab to underside of floor or roof deck.
4. Firestopping shall be provided at the plane of the gypsum wallboard membrane or rated
ceilings all penetrating partitions, exterior stud walls and furred spaces.
5. All backboxes, panels, valve boxes, toilet accessories and other recessed items in excess of
sixteen square inches, which are recessed in fire rated walls shall be protected with a five
sided enclosure equal to the rating of the wall.
6. The contractor shall construct all fire rated assemblies in the following order of priority:
Four Hour Fire walls
Three Hour Fire walls
Two Hour Fire walls
One Hour Fire walls
Gypsum wallboard horizontal fire membrane
One hour rated partitions
Smoke resistive walls
Non -rated interior partitions
7. The Contractor shall provide signage above the ceiling spaced at 10' O.C. indicating the
rated partition type and to "PROTECT ALL OPENINGS ". Signage letters shall be 3" high in
a dark red color and be placed on both sides of priority walls along their entire length.
