TRANSPORTATION IMPACT /UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89 -07 and 90 -07, as amended. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to receiving a "certificate of occupancy" or final power release. March 2, 2009 EVALUATION REPORT No,: ER -09 -0003 Reference No.: 29006 Product: Exterior Doors - Rolling Overhead Doors Models 501/502, 651, and 652 Manufacturer: U S Door 10407 Rocket Blvd. Orlando, FL 32824 Statement of Compliance: The Rolling Overhead Doors, Models 501 /502, 651 and 652 described in this report were evaluated to be in compliance with the 2007 Florida Building Code, Sections 1701 and 1714.5.3.1. The doors are, for the purpose intended, at least equivalent to that required by the Code when manufactured and installed as described below. Description of the Product: All Models covered in this report consist of a corrugated steel sheet curtain suspended from an axle assembly. The curtain is raised by coiling the curtain around the assembly. Models 501 and 502 differ only in the type of axle assembly and this is shown on the drawing. The different axle assemblies do not affect the wind load capacity of the door. The sides of the curtain are constrained from lateral movement along their vertical edges by steel guides that are attached to the door jambs. This constraint provides resistance to lateral wind. forces. Various guide configurations are used for the different door styles included in this report. The lateral wind forces are transferred from the curtain to the guides and then through the attachment elements to the door jamb. The door jambs are part of the main wind frame resisting system and usnatly are constructed of steel, concrete, or concrete masonry units. All door models included in this report have curtains with a nominal thickness of 0.017 in. and are made of Hot -Dip Galvanized steel sheet, ASTM A653 structural steel grade 80 with a full coat of primer and baked polyester finish coat. The corrugated sheets are interlocked mechanically to form the curtain. Lap splices are at 20 inches on center vertically in the installed door. This includes two types of Model 501 /502, two types of Model 651 and one type of lvlodel 652. Style variations may include door width, guide geometry, anchorage size and spacing, and wind load rating, windlocks, or any combination. Each style is described in detail on a drawing for that style. The drawing number is coded for the Model No., the rated design wind pressure for the test width, and the opening width of the test door in inches. For example, the Drawing No. 501/502 -30 -104 identifies the particular style to be Model 501/502, rated design wind pressure of the test door is 30 psf, and the test door opening width is 104 inches (8' -8 "). The design pressures for other widths are determined by comparative analysis with the test door. The particular guide geometry and attachment method is fully described on the drawing. Maximum door heights are shown on the drawing. FL No. 7768 Evaluation Report: ER -09 -0003 March 2, 2009 Limitations and Conditions of use The use of any door is limited to buildings for which the design wind loads for wall components and cladding, determined in accordance with Section 1609 of the 2007 Florida Building Code, do not exceed the rated design wind loads of the door as shown in Table 1. The maximum width and height limitations are shown in Table 1. Door manufacturing is limited to those plants that have met the 2007 Florida Building Code Product Approval quality assurance requirements. Doors are to be assernbled as shown on the appropriate drawing referenced above, and the doors are to be installed in accordance with the installation instructions referenced above.
