prevention and protection related activities such as inspections, plan review, administrative fees, and other
wsts related to the aforementioned.
Complete Plans, Specifications and Fee Must Accompany Application. Commencement of work without written approval of
the Fire Department's Fire Marshal or required permits or opening up for commercial activiry without an approved final
inspection shall be charged double permit fee per day of operation or a minimum of $100.00, whichever is greater. All

NOTICE OF DEED RESTRICTIONS: The undersigned unde�stands that this permit may be subject to "deed" restrictions"
which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance with any
applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division—Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a"certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law—Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the "owner" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
- Department of Environmental Protection-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment.
- Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses
- Army Corps of Engineers-Seawalls, Docks, Navigable Waterways.
- Department of Health & Rehabilitative Services/Environmental Health Unit-Wells, Wastewater Treatment,
Septic Tanks.
- US Environmental Protection Agency-Asbestos abatement.

I understand that the following restrictions apply to the use of fill:
- Use of fill is not allowed in Flood Zone "V" unless expressly permitted.
- If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
- If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
- If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered abandoned

I have received and reviewed the plans for the fire sprinkler modification located at 7326 Gall
Blvd and will allow the project to move forward. By paying for the permit, contractor
acknowledges to comply with the item below. Should anyone have any questions, please do not
hesitate to contact the Fire Marshal's office.
1. FDC shall be a 5" 30 degree turndown Storz connection, if not already in place.
2. FDC sign shall be located at FDC. Sign shall state "FDC" in 6" red reflective letters
on white background and shall also state the address served. (should be there)
3. Tampers shall be installed on double backflow, if not already that way.
4. Properly tag system once completed.
5. Label all doors where riser is located, or where any other valves are located.
(EXAMPLE: "SPRINKLER RISER")
6. Ensure adjacent system (Save-a-Lot) remains in operation. If that system is to be shut
down, notify that business. If to be shut down for more than 4 hours, notify this
authority.

C O N S T R U C T I O N
SUBCONTRACTOR AGREEMENT
This Agreement made this 16_day of _March 2011_, by and between _Eagle Fire Protection , hereinafter called the
Subcontractor, whose principal office is at _1205 Crown Park Circle, Winter Garden FL 34787 and Tenant Build inc.. 7604 Baker
Blvd. Richland Hilis, TX 76118, hereinafter called the Contractor Witnesseth that the Subcontractor and the Contractor, for the
consideration hereinafter named, agree as follows:
1 Subcontractor agrees to furnish and pay for all materials, labor, supervision, equipment, tools, appliances, and everything
necessary to fuliy do and complete the Fire Sprinkler work described in paragraph 2 hereof, for work at TJ Maxx 1194,
7326 Gali Blv�. Zephyrhills FKL 33541 for _TJX Companies , hereinafter called Owner, in accordance with the
Drawings and Specifications prepared by _Rogue Architects_, dated _1.2Q 11 and hereby made a part of this
suacon#ract.
2. Subcontractor shall provide all labor. materiais, eQUipment, supervision and insurance necessary or incidental for the execution and
completion of the Subcontractor's Work to the full extent of the Contract Documents, regardless where shown or specified,
specifically including but not necessarily limited to the following:
a. Provide all labor. materials. svstem desiqn, permittinq and equipment to perform all Fire Sprinkier work to make job
complete per plans and specifications. It is agreed and understood that the scope of work includes, but is not limited to
the following:
1 Design of the Fire protection System.
2. Required permits for the Fire protection scope of work
3 Ail piping & Materials to be UL listed and per NFPA Standards
4 Gutting & capping of existing lines no longer required, this may involve some work in adjacent store spaces
5 Sprinkler Heads and trim rings are to be white in color
6. Supply and instali all flow and tamper switches
7 Contract includes applicabie taxes.
b Subcontractor's application for payment must be submitted with a complete list of all material suppliers, equipment
suppliers, and any subcontractors.
c. Subcontractor shall do whatever is required to adhere to Tenant Build Inc.'s Construction schedule.
d. It is the Subcontractor's responsibility to abide by all applicable OSHA and Safety procedures.
e. Any secondary subcontractor must be approved by the Contractor prior to their work and their work at the site is the sole
responsibility of the Subcontractor and is to be supervised by the Subcontractor
f Subcontractor agrees to complete their punch list items within (7) calendar days of receipt of punch list. If all punch list
items are not complete within that time frame, Tenant Build Inc. will proceed to have those remaining items corrected by
others and these costs deducted from the balance due Subcontractor
g. Suhcon'ractor agreas te provide ad�quate !abor. materials, and other resa�rces as �equired io mamtain the project
schedule and achieve the date for Substantial Completion as it applies to the Subcontractor's Work
h. Subcontractor must have a full time, competent foreman on the jobsite at all times while performing work.
3. An Insurance Certificate including General Liability coverage and any additional coverage required by the Subcontractor's policy
and State requirements where work is being performed, must be provided by the Subcontractor's Insurance Company prior to wor{:
being started and/or before any payments are made to Subcontractor
4 The Contractor agrees to pay the Subcontractor for the performance of his work the sum of $23,545 00 and to
make payments on account thereof as follows: 100_% of the value of labor and materials incorporated by the Subcontractor in
the work and of materials stored on the job site in a manner acceptable to Contractor No payment to Subcontractor shall operate
as an approval of Subcontractor's work or materials, or any part thereof The final payment hereunder shall not be due until
Subcontractor has delivered to Contractor a complete release of all liens in form and text satisfactory to Contractor
5 Subcontractor agrees to commence the work when directed by the contractor and to diligently and continuously prosecute work and
to coordinate the work with other work being done on the project by other trades, so that Contractor shall not be delayed by any or
omission of Subcontractor in completion of the general contract within the time specified in the contract documents.
6 No alterations shall be made in the work as shown or described by the Drawings and Specifications except on the written order of
the Contractor, and when so made the value of the work or materials added or omitted shall be computed and determined by the
Subcontractor, subject to the written approvai and accept4nce by the contractor, and the amount so determined shall be added or
deducted from the subcontract price or prices. The subcontractor shall have no claim for additional work unless such work has
been done in pursuance of a written order from the Contractor

7• Subcontractor agrees to be bound to the contractor by all of the terms of the agreement between the Contractor and the Owner and
by the Drawings and specifications, and to assume toward the Contractor all of the obligations and responsibilities that the
Contractor by those instruments assumed toward the Owner
8. The Subcontractor shall maintain at his own cost such insurance as will protect him from claims under the Worker's Compensation
Law of the State in which the work is being performed and also Public Liability and Property Damage insurance in such amounts
and for such coverage as required by Contractor Prior to commencing work, the Subcontractor shall have his insurance company
or companies, which are acceptable to the Contractor, execute a Certificate of Insurance form and file a copy of same with the
contractor, and should the Subcontractor fail or neglect to provide the required insurance, the Contractor shall have the right, not
the duty, to provide said insurance and deduct from any money that may be due or become due to the Subcontractor any and all
premiums paid by said Contractor for and on account of said insurance
9 Subcontractor agrees to indemnify and hold contractor, owner, and architect, and their respective owners, partners, members,
shareholders, directors, officers, employees, agents, servants, and representatives (collectively "Indemnified Parties") harmless
from and against any and all losses, expenses, demands, claims, damages, proceedings, or causes of action, whether at law or in
equity, arising out of or alleging any bodily injury, (Including, but not limited to, death) property damage, or economic damage
actually or allegedly by Subcontractor It is expressly agree that the foregoing indemnity shall include, without limitation, the
indemnity of the Indemnified Parties against any claims arising out of or relating to the actual or alleged negligence or gross
negligence �f an indemn�fied party, either solely or in cumbination with the negligence or other liability of Subcnntractor and/or
others. Subcontractor agrees to defend any suit or action brought against an indemnified party based on any such injury or
damage and to pay all costs and expenses in connection therewith or resulting there from, including, but not limited to, attorney's
fees, pre-judgment and post-judgment interest, appeal bonds, and supersedes bonds.
10. Subcontractor shall not let, assign or transfer the Subcontract or any part thereof, or any interests therein, without the written
consent of Contractor
11 Subcontractor will pay all social security and other taxes imposed upon him, as an employer in connection with the performance of
this Subcontract, will furnish evidence, when required by Contractor, showing that all such payments required to be made have
been paid. Subcontractor shall pay all local, state and federal taxes in connection with his work, and charges for all utility services,
including water and power, used by him in performance of his work.
12. Payments othervvise due may be withheld by Contractor on account of defective work not remedied, claims filed, reasonable
evidence indicating probability of filing of claims, failure of Subcontractor to make payments properly to it's subcontractors or for
material or labor, or a reasonable doubt that the subcontract can be completed for the balance then unpaid. If the said causes are
not removed, on written notice, contractor may rectify the same at Subcontractor's expense.
13 Subcontractor will save and keep the building or buildings referred to in this subcontract and the lands upon which they are situated
free from all mechanics liens and all other liens by reason of his work or of any materials or other things used by him therein. If the
Subcontractor fails to remove the lien by bonding it or otherwise, the Contractor may retain su�cient funds, out of any money due
or thereafter to become due the Contractor to the subcontractor, to pay the same and all costs incurred by reason thereof and may
rectify the same at Subcontractor's expense.
14 Subcontractor shall submit an invoice as a requisition for payment which will be processed and paid within 10 business days from
the date the invoice is received. As a condition of payment, Contractor may require lien waivers from Subcontractor as required by
the Owner
15 Subcontra�tor wiil ciean up and 'naui away all debris occasioned �y work done by him hereunder and will leave the building and
premises clean insofar as the work perFormed under this agreement is concerned. If, after 24 hours notice by the Contractor's
representative to the Subcontractor's representative at the site of the work, the subcontractor has not diligently proceeded with the
clean-up as outlined in this paragraph, then the Contractor has the right to proceed with the clean-up work at the Subcontractor's
cost and expense.
16 Time is of the essence of this Subcontract and any breach of same shall go to the essence thereof, and Subcontractor, in agreeing
to complete the work within the time herein above mentioned, has taken into consideration and made allowances for all the
hindrances and delays incident to his work.
17 If the Subcontractor at any time shall refuse or neglect to supply adequate and competent supervision, or a sufficiency of properly
skilled workmen or of materials of the proper quality or quantity, or fail in any respect to prosecute the work with promptness and
diligence, or fail in the performance of any agreement on his part herein contained, the Contractor shall have the option, after 24
hours written notice to the Subcontractor, to provide any such labor or materials and to deduct the employment of the Subcontractor
on said work and to enter upon the premises, and take possession of all materials or appliances, of any kind whatsoever, thereon
and to employ any other person or persons to finish the work and to provide the materials therefore, and in the case of such
discontinuance of the Subcontractor he shall not be entitied to receive any payment under this Subcontract which might otherwise
be due him, until the said work shall be finished and payment in full therefore shall be made by the Owner to the Contractor, at
which Subcontractor's work, such excess shall be paid by the Contractor to the Subcontractor, but if such expense shall exceed
such unpaid balance the Subcontractor shall pay the difference to the Contractor Subcontractor agrees not to assign any monies
due or to become due him hereunder Whenever any monies are expended or costs or expenses are incurred by contractor on

18 behalf of or on account of Subcontractor, or if Contractor continues or completes the work after default by Subcontractor,
Subcontractor shall pay to Contractor interest at a rate of ten percent (10%) per annum from the time said monies are expended or
said costs or expenses are incurred until the same are paid to Contractor Nothing herein contained shall be construed as requiring
Contractor to make any such expenditure, advance any such monies, or incur any such expenses.
19 All negotiations and agreements prior to the date of this agreement not included herein are hereby voided

I have received and reviewed the plans for the fire sprinkler modification located at 7326 Gall
Blvd and will allow the project to move forward. By paying for the permit, contractor
acknowledges to comply with the item below. Should anyone have any questions, please do not
hesitate to contact the Fire Marshal's office.
1. FDC shall be a 5" 30 degree turndown Storz connection, if not already in place.
2. FDC sign shall be located at FDC. Sign shal) state "FDC" in 6" red reflective letters
on white background and shall also state the address served. (should be there)
3. Tampers shall be installed on double backflow, if not already that way.
4. Properly tag system once completed.
5. Label all doors where riser is located, or where any other valves are located.
(EXAMPLE: "SPRiNKLER RiSER")
6. Ensure adjacent system (Save-a-Lot) remains in operation. lf that system is to be shut
down, notify that business. If to be shut down for more than 4 hours, notify this
authority.

1. DESCRIPTION —
Viking Standard/Quick Response Extended Coverage Ordinary Hazard (ECOH)
Upright Sprinkler VK570 and Pendent Sprinkler VK572 are thermosensitive glass
bulb spray sprinkiers equipped with a 14 0(202 metric") nominal K-Factor and a
special deflector.
The special deflector is designed to meet Ordinary Hazard density requirements for
specifically listed extended areas of coverage when the required minimum water
supply is provided at the sprinkler. The sprinkler produces the flows required to meet
Ordinary Hazard density requirements at lower pressures than 8.0 or 11.2 (115.2 or ��0
161.4 metric') K-Factor sprinklers.
Viking EC/QREC Ordinary Hazard Sprinklers are available in various finishes and
temperature ratings to meet design requirements. The special Teflon� and Polyester
coatings can be used in decorative applications where colors are desired.
Viking VK570 and VK572 Sprinklers may be ordered and/or used as open sprinklers Upright VK570
(glass bulb and pip-cap assembly removed) on deluge systems. Refer to Ordering
Instructions on the next page.

4. INSTALLATION

5. OPERATION
During fire conditions, when the temperature around the sprinkler reaches its operating temperature, the heat-sensitive liquid in
the glass bulb expands, causing the bulb to shatter, releasing the pip cap and sealing spring assembly. Water flowing through the
sprinkler orifice strikes the sprinkler deflector, forming a uniform spray pattern to extinguish or control the fire.

Viking Microfast� and MicrofastHP� Quick Response Upright and Conventional
(Old Sryle) Sprinklers are small, thermosensitive, glass-bulb spray sprinklers
available in several different finishes, temperature ratings, and K-Factors to
meet design requirements. The special Polyester and Teflon� coatings
