Chapter 633, Florida Statutes, authorizes the City to charge and collect user fees to pay for the costs of fire
prevention and protection related activities such as inspections, plan review, administrative fees, and other
costs related to the aforementioned.
Complete Plans, Specifications and Fee Must Accompany Application. Commencement of work without written approval of
the Fire Department's Fire Marshal or required permits or opening up for commercial activity without an approved final
inspection shall be charged double permit fee per day of operation or a minimum of $100.00, whichever is greater. All
work shall be performed in accordance with City Codes and Ordinances.

that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of

- Use of fill is not allowed in Ffood Zone "V" unless expressty permitted.

- If the filI material is to be used in Flood Zone "A" in connection with a permitted building using stem wall

this affidavit prior to commencing construetion. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
pertnit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside arry provisions of the technical codes, rwr shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans, construction or violations of any codes. Every permit issued sha�l become invalid
unless the work authorized by such permit is commenced within soc months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of sbc (6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the exteruion. If work ceases for ninety (90) consecutive days, the job is considered abandoned.

1 have received and reviewed the plans for the fire alarm modification located at 38451
Market Square and will allow the project to move forward. Paying for permit, contractor
acknowledges complying with the comments listed below. Should anyone have any
questions, please do not hesitate to contact the Fire Marshal's office.

2. Ensure telephone jacks are marked
3. Ensure there is a breaker lock on the breaker for the fire alarm panel. Also ensure
that location is labeled inside the fire alarm panel.
4. Ensure there is a zone map located by FA panel.
5. Strobes shall remain on when system is silenced.
6. Install an outside/weather proof horn strobe at the front door area.
7. Label devices according to fire alarm programming.
8. Ensure there is a FA log book at the panel.
9. Ensure all penetrations to a fire rated wall are done properly keeping the rating
intact.
10. A Knox Box shall be installed, if one not already located near main entrance. An
application for the Knox Box can be obtained from this authority.

The BPS provides additional power and circuits for notification ap- 1

GENERAL FIRE ALARM SYSTEM NOTES
1. WHERE CEILING HEIGHTS ALLOW, WALL MOUNTED AUDIBLE APPLIANCES SHALL HAVE THEIR TOPS
AT HEIGHTS OF NOT LESS THAN 90 in. AFF, AND BELOW THE FINISHED CEILING OF NOT LESS
THAN 6 in.
2. WALL MOUNTED VISUAL OR AUDIBLE /VISUAL APPLIANCES SHALL BE MOUNTED SUCH
THAT THE ENTIRE LENS IS NOT LESS THAN 80 in. A.F.F. IN ACORDANCE WITH NFPA72 -7.5.4
(k) ADAAG
4.28.3 Visual Alarms.
The appliance shall be placed 80 in above the highest floor level
within the space or 6 in below the ceiling, whichever is lower.
3. DO NOT MOUNT SMOKE AND /OR HEAT DETECTORS CLOSER THAN 3' TO ANY SUPPLY REGISTER
OF THE A/C SYSTEM.
4. WHEN CEILING MOUNTING SMOKE AND /OR HEAT DETECTORS SHALL BE MOUNTED NO CLOSER
THAN 4" TO A SIDE WALL, OR WHEN WALL MOUNTED NO HIGHER THAN 4" OR LOWER THAN
12" FROM THE TOP OF THE DETECTOR TO THE CEILING.
5. CONTROL RELAYS MUST BE MOUNTED WITHIN 3' OF THE CONTROLLED CIRCUIT OR APPLIANCE.
6. IF FIRE ALARM CONTROL RELAYS ARE TO CONTROL THE EQUIPMENT OF ANOTHER TRADE, THE
OTHER MUST TERMINATE CIRCUITS TO CONTROL RELAY CONTACTS, OR LEADS IF PROVIDED,
7. UNDERGROUND /OVERHEAD WIRING ENTERING OR LEAVING A BUILDING SHALL BE INSTALLED AND
PROTECTED IN ACCORDANCE WITH NFPA72- 4.4.4.3./4.4.4.4 AND NFPA70- ARTICLE 280,
WHEN REQUIRED, 'TVSS' SHALL BE INSTALLED IN ACCORDANCE WITH NFPA70- ARTICLE 285.
8. CALCULATIONS ARE BASED ON RACEWAY /WIRE ROUTING AS SHOWN ON PLANS. ANY DEVIATION FROM
THIS DESIGN MAY RESULT IN SYSTEM NOT MEETING PROPER PERFORMANCE STANDARDS. IF
DEVIATION MUST BE MADE IT IS THE INSTALLERS RESPONSIBILITY TO COMMUNICATE TO THE
OFFICE ANY INTENDED OR REQUIRED CHANGES.
9. WHERE HEAT DETECTORS ARE INSTALLED TO SHUT DOWN ELEVATOR POWER PRIOR TO SPRINKLER
OPERATION THE DETECTOR SHALL BE INSTALLED WITHIN 2' OF EACH SPRINKLER HEAD, AND
SHALL BE RATED AT 135' FIXED TEMPERATURE.
10. ONLY ASSOCIATED DEVICE WIRING IS REQUIRED ENTERING INTO BACK -BOX, ALL OTHER WIRING
MAY CONTINUE .PAST BACK -BOX.
