Chapter 633, Florida Statutes, authorizes the City to charge and collect user fees to pay for the costs of fire
prevention and protection related activities such as inspections, plan review, administrative fees, and other
costs related to the aforementioned.
Complete Plans, Specifications and Fee Must Accompany Application. Commencement of work without written approval of
the Fire Department's Fire Marshal or required permits or opening up for commercial activity without an approved final
inspection shall be charged double permit fee per day of operation or a minimum of $100.00, whichever is greater. All
work shall be performed in accordance with City Codes and Ordinances.

I have received and reviewed the plans for the installation of the fire sprinkler system located at
5610 6' Street and will allow the project to move forward. Paying for permit, contractor
acknowledges complying with the comments listed below. Should anyone have any questions,
please do not hesitate to contact the Fire Marshal's office.
i. Ensure installation is in accordance to all applicable codes and standards.
2. lnstall tracing wire on underground portion — 2 wires required.
3. Underground shall be pressure tested as a section by itself at 200 psi.
4. Underground shall be flushed — witnessed — prior to connection to sprinkler
system.
5. At location of FDC a sign shall be installed stating "FDC — 1%z". This sign shall
have 6" red reflective letters on white background and shall be on corner of
building nearest to riser facing roadway.
6. Install some protection (bollard style) around riser to prevent damage from
motorized equipment such a s lawn mowing equipment.
Inspection Required:
l. Underground pressure test @ 200 psi for 2 hours.
2. Underground flush
3. System pressure test @ 200 psi for 2 hours
4. Acceptance — completed with fire alarm acceptance

portions of the "contractor Block" of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the °Florida Construction Lien Law—Homeowner's
Protection Guide" prepared by the Florida Department of Ag�iculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the "owner" prior to commencement.
- CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and
that all work will be done in compliance with all applicable laws regulating construction, zoning and land
development. Application is hereby made to obtain a permit to do work and installation as indicated. I certify
that no work or installation has commenced prior to issuance of a permit and that all work will be performed to
meet standards of all laws regulating construction, County and City codes, zoning regulations, and land
development regulations in the jurisdiction. I also certify that I understand that the regulations of other
government agencies may apply to the intended work, and that it is my responsibility to identify what actions I
must take to be in compliance.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building O�cial from thereafter
requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
just�able cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered abandoned.

Hey Stacie,
Owner picked up permit and we did not charge correcdy due to credit for water and sewer and Transportation
impact fee.
I orignally charged permit fee amount of $288.60 +(tifl $460.65= $749•25 �see check paid.)
Credit for sewer and water superceded amount due for payment took credit left of $89.52 and $79.52 which is
$i69.o4 applied toward permit cost as you see on new fee sheet. I had orignally charged $288.6o and owner
should have paid $ii9.56 for permit cost we owe $i69.o4 back as you see.
The amount I charged for TIF was $460.65 but according to our ordiance ioo3-o8 section i2 #6 we were not
suppose to charge TIF . So there is also a credit amount of $ 460.65 due back to owner.
Total due back is $629.69. Amount need to apply toward journal entry for new permit is $ 335•00 leaving a
balance of $294.69 credit off the tif since no tif should have not be paid and I can apply for refund on that
amount.
Account credit is from building permit # 001-0000-322-000o and tif o10-0000-363-2490 also city 1% ooi-
0

I have received and reviewed the plans for the building renovation located at 5610 6tn
Street and will allow the project to move forward. Paying for permit, contractor
acknowledges camplying with the comments listed below. Should anyone have any
questions, please do not hesitate to contact the Fire Marshal's office.
1. Scale noted on drawings is wrong (1" = 1'), however did not reflect any concern
with this review. ✓
2. Separate fire ala'rm and fire sprinkler plans will need to be submitted by the
installing contractor. Ensure all cut sheets, details, calculations, appropriate
licenses, etc... are submitted to obtain permit.
3. There are no plans submitted for the gas. Ensure plans are submitted on the gas
installation for the chambers. The area that the gas is located outside shall be
secured and properly labeled. Ensure the area that the gas will be located is
compliant to all applicable NFPA Codes & Standards.
`G�4. Install signage at the chamber entrance "NO FLAMMABLE LIQUIDS, GASES
AND OTHER ARTICLES ALLOWED INSIDE"
5. Install "NO SMOKING" signage outside the chamber and near the gas location.
6. Install a certified fire extinguisher in accordance to NFPA 10. Contact a fire
extinguisher company for correct type, size and placement.
7. Ensure all penetrations in firewall have been sealed properly to maintain the
rating of the firewall.
8. Ensure firewall is fire caulked at roof deck and is labeled on each side as a 2 hour
firewall.
9. Ensure address is located on front door, minimum 4" numerical.
10. A Knox Box is required due to the installation of a fire alarm and fire sprinkler
system. A master key will be provided to e placed in the box. An application can
be obtained from this authority.
11. Building Of�cial will check all electric to ensure code compliant.
