Discussion Points
• The applicant and City met prior to the formal Site Plan meeting and agreed that
i.) the sign could be moved to the southwest corner of the site per conditions
outlined below, 2.) parking and landscaping modification would take place, and
3.) architectural additions would be made to the western roofline

at 7800 Gall Blvd and will allow the project to move forward. By paying for permit, contractor
acknowledges in complying with the comments below. Should anyone have any questions, please
do not hesitate to contact the Fire Marshat's office.
I. On the cover sheet, pages S 1.01 — S 1.04 are identified wrong, should be S0.1 — 50.4.
2. Page S0.5 is not shown on cover page.
3. Page S1.0 is not included in the plans.
4. Page P0.0 is identified wrong, should be P0.1.
�. A Knox box is indicated in the plans. A Knox box application may be obtained from the
address above. Box should be installed to the left of the main door at a height of 6'.
6. Ensure door 1 I 3A (egress) has panic hardware installed.
7. Room 108 (electric) shall be rated with a] hour assembly due to a higher degree of
hazard then the rest of the structure. Ensure door 108A is changed to a 45 min door.

I have received and reviewed the plans for the revision of for the construction of a bank located in
Zephyr Commons at 7800 Gall Blvd and will allow the project to move forward. By paying for
permit, contractor acknowledges in complying with the comments below. Should anyone have
any questions, please do not hesitate to contact the Fire Marshal's office.
1. All items noted on review dated 8/29/11 remain to be addressed.
2. The revisions submitted did not indicate what changed and had to be matched to the
original submitted drawings to discover the changes being made.
3. Cover sheet indicated 16 pages that changed, however an additional 12 pages were
submitted. There did not appear to be any changes on these pages. The following pages
that were additional are: A0.0, A1.2, A6.1, A6.4, A6.6 — A6.9, A7.2, A8.3, A9.1
4. Wall between room 115 and 116 is noted as a rated wall. Screw/nail inspections will need
to take place on this wall.
5. Wall around room 117 (ladder area) is noted as rated walls. Ensure door (117A) is also
rated (should be a 45 minute door). Ensure room is labeled as roof access.
6. Ensure electric room is labeled as such.

Plan Review Comments:
1. F.F E. shall be a minimum 8" above road elevation or to the engineered site
plan.
2. Tie-in survey required before pre-slab inspection to show elevation.
3. Compaction test required.
4. All setbacks shall be met.
5 All property markers to be exposed before first inspection.
6. Drainage Plan Requirements must be made before pre-slab inspection.

7. All glazing requirements to be met per chapter 24 of the F.B.C.
8. All means of egress are to be per chapter 10 of the F.B.C.
9. No plumbing, mechanical, efectric, or framing shall be cover with inspection

Inspection and approval shall be done before any concrete is poured.
11. R.O.W. use permit required for all driveways and all driveways to include
expansion materials.
12. All handicap and A.D.A. codes and requirement shall be met.
13. All welds, if any, shall be inspected by an independent engineering company,
and a copy of their report shall be given to the building department.
14. All Architectural plans must be approved by the Planning Department.
15. All plumbing, mechanical, and electric shall be separate from unit to unit. This
shall include underground plumbing and electric.
16. All signage shall require separate plans and permits and be done in
accordance with Ordinance No. 870-04

Are you installing fire alarm system or security system?
We will need a small plan showing security system if you are installing and we do charge a fee based on valuation ofjob.
If you are doing the fire alarm installation we will need the plans and schematics for the fire marshall and he charges a
fee for this there is a different application.

which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance with any

UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hired a contra+ctor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation

TRANSPORTATION IMPACTlUTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a°cert�cate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
final power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
fees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that 1, the applicant, have been provided with a copy of the "Florida Construction Lien Law—Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the "owne�" prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and that ali work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Appiication is
hereby made to obtain a permit to do work and installation as indicated. I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be perFormed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction. I also
certify that I understand that the regulations of other government agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in compliance. Such agencies include but are not limited to:
- Department of Environmental Protection-Cypress Bayheads, Wetland Areas and Environmentally Sensitive
Lands, Water/Wastewater Treatment.
- Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
- Army Corps of Engineers-Seawalls, Docks, Navigable Waterways.
- Department of Health & Rehabilitative Services/Environmental Health Unit-Wells, Wastewater Treatment,
Septic Tanks.
- US Environmental Protection Agency-Asbestos abatement.

I understand that the following restrictions apply to the use vf fill:
- Use of fill is not allowed in Flood Zone "V" unless expressly permitted.
- If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
- If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
- If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered abandoned.
