I understand that the folbwing restrictions appy to the use of fill:
- Use of flll is not allowed in Flood Zone "V' unless expressly pertnitted.
- If the filt matsrial is to be used in Flood Zone 'A', it is understood that a drainage plan addressing a
"compensating volume' will be submitted at time of pertnitting which is prepared by a professional engineer
lioensed by the State of Fbrida.
- If the fill material is to be used in Flood Zone "A° in connection with a pem►itted building using stem wali
const►uctlon, I certiiy that fill will be used ony to 1i11 the area within the stem wall.
- If flll material is to be used in any area, I oertify that use of such fill will not adversely affect adjacent
properties. If use of fiN is found to adversely affed adjacent propertles, the owner may be cited for violating
the conditions of the building pertnit issued under the attached permit applicatlon, for lots less than one (1)
acre which are elevated by fill, an engineered drainage ptan is required.
If I am the AGENT �OR THE OWNER, I promise in good faith to iMorm the owner of the permitting conditions set forth in
this affldavit prfor to oommencing oonstruclion. I understand that a separate pemiit may be required for electrical work,
plumbing, signs, wells, pools, air condiGoning, gas, or other installatlor►s not speciflcally induded in the application. A
permR issued shall be construed to be a lioense to proueed with the work and not as authority to violate, cancel, alter, or
set aside any provisbns of the technical codes, rwr shall issuance of a pertnR prevent the Building O(ficial from thereafter
requtrinp a correction of errors in plans, consVuction or viola6ons of any codes. Every permk issued shall beoome invalid
unlesa the work authorized by such pertnit is commenced within six months of permit issuance, or ff worlc authorized by
the pertnft is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extensron
may be requested, in writing, irom the Building Official for a period not to exoeed ninety (90) days and will demonstrate
jusdflable cause fot the exbe�sion. ff work ceases for ninety (90) consecutive days, the job is considered abandoned.
