UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSlBILITIES: If the owner has hired a contractor or
corrtradors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the

under state law If the owner or intended contrador are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Divisiort—Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or conVactors, he is advised to have the conVactor(s) sign
portions of the "contractor Blodc' of this application for which they will be responsible. If you, as the owner sign as the
contrador, that may be an indication that he is not properly licensed and is not entitled to pertnitting privileges in Pasco
County.
TRANSPORTATION IMPACTlUTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportafion Impact Fees and Recourse Recovery Fees may apply to the consVudion of new buildings, change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended. The undersigned also understands, that such fees, as may be due, will be identified at the tirrie of
permitting. It is further understood that Transportatlon Impact Fees and Resource Reoovery Fees must be paid prior to
recelving a"certificate of occupancy" or final power release. If the project dces not involve a certificate of occupancy or
final power release, the fees must be paid prior to pertnit issuance. Furthermore, 'rf Pasco County WateNSewer Impact
fees are due, they must be paid prior to pertnk issuance in accorciance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW (Chapter 713, Fbrtda Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the °Florida Construdion Lien Law—Homeowners
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Aftairs. If the applicant is someone
other than the "owne�', I certiiy that I have obtained a copy of the above described document and promise in good faith to
deliver ft to the "owne�' prior to commencement.
CONTRACTOR'3/OWNER'S AFFIDAVIT I certify that all the infortnation in this application is accurate and that all work
will be done in compliance with all applicable laws regulating construction, zoning and land development. Appliption is
hereby made to obtain a pertnit to do work and installation as indicated. I certi(y that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to rt�et standaMs of all laws regulating
conshuction, County and City codes, zoning regulations, and land devebpment regulations in the jurisdiction. I also
certify that I understand that the regulations of other govemmenf agencies may apply to the intended work, and that it is
my responsibility to identify what actions I must take to be in canpliance. Such agenaes indude but are not limited to:
- Department of Environmental Protectan-Cypress Bayheads, Wetland Areas and Environmentalty Sensitive
Lands, Water/Wastewater Treatment.
- Southwest Florida Water Management District-Wells, Cypress Bayheads, Wetland Areas, Altering
Watercourses.
- Army Corps of Engineers-Seawalls, Dodcs, Navigable Watervvays.
- Department of Heatth 8 Rehabilitative Services/Environmental Health Unit-Wells, Wastewater Treatrnent,
Septic Tanks.
- US EnvironmenTal Protedion Agency-Asbestos abatement.
- Federal Aviation Authority-Runways.
I understand that the following restrictions apply to the use of fill:
- Use of fill is not allowed in Flood Zone "V' unless expressy permitted.
- If the fill material is to be used in Flood Zone 'A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by Ure State of Fbrida.
- If the fill material is to be used in Flood Zone "A° in connec�ion with a permitted buildir►g using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
- If fill material is to be used in any area, 1 certity that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversey affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit appliqtion, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the pertnitting conditions set forth in
lfiis affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wetls, pools, air cond'Rioning, gas, or other installaHons not speafically included in ttie application. A
pertnit issued shall be construed to be a license to proceed with the work and not as authority to violate, qncel, alter, or
set aside any provisions of the technipl codes, rior shall issuance of a pertnit prevent the Building Official from thereafter
requiring a coRection of errors in plans, construc�ion or violations of any codes. Every pertnit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if wo�fc authorized by
the pertnit is suspended or abandoned for a period of six (6) months after the time the work is �mmenced. M extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the extension. If work ceases for ninety (90) consecutive days, the job is considered abandoned.

I have received and reviewed the plans for the fireworks tent sales located at 5953 Gall Bivd
and will allow the project to move forward. Please note that this review does not eliminate
any further requirements as the project continues moving forward. By receiving permit,
applicant acknowledges to comply with the items listed below. Should anyone have any
questions, please do not hesitate to contact the Fire Marshal's office.
1. Site shall be inspected after setup and prior to any sales.
2. Copies of all sales shall be given to this office every couple of days.
3. Attached guideline sheet shall be followed.
4. In addition to the certified ABC fire extinguisher, a minimum of 1 pressurized
water extinguisher shall be present.

This Lease Agreement is entered into as of the 10` day of March , 20 11 by and between the undersigned Lessor and Gala�cy
Fireworks, Inc. a Florida �:oFpc�atipn, having its principal place of business at 204 East Dr Martin Luther King, Jr. Boulevard, Tampa Florida 33603
("Lessee(s)") for the�purpose of establiSl�jng the terms and conditions by which Lessor leases certain property to Lessee.

1 Premises Location: The property that i5 the subject of this Lease Agreement (the "Premises") is located at the following address

2. Occupancy: Lessor represents and warrants that it owns the Premises, and Lessor hereby grants Lessee the exclusive right to occupy and use the
\Premises undisturbed du,[ing each Term (as hereinafter defined), access thereto (including the right to cross surrounding property belonging to Lessor, if
anyj; aleag_r�ith a}Yother rights provided to a tenant under the laws of the state of Florida.
3 Term:. The periods during which Lessee will be entitled to occupy the Premises ("Terms") are as follows:
Term 1 is from June lSth 2011 until Julv 7th, 2011. Term 2 is from Dec. ISth 2011 until January7'�2012.("Initial[.easePeriod")2011-
2016. Thereafter occupancy will be determined by Lessee's election pursuant to Section 11 This Agreement shall continue in full force and effect until
the Option Periods identified in Section 11 below expire;
4 Use: Lessee may use the Premises to sell seasonal products, including but not limited to fireworks and items incidental thereto.
5 Renh Lessee will pay Lessor the sum of $2.000.00 for Term I and $1,500.00 for term 2 which will be due and payable on the first day of the
applicable Term.
6. Permits: Lessee agrees to acquire any and all permits required by governing authorities for Lessee's intended use ofthe Premises. If the permits are
not approved for any reason, this Agreement will be null and void, and all deposits will be refunded to Lessee. If, after any necessary permits have been
issued to Lessee, Lessee is prevented from using the Premises for the intended purpose of selling fireworks during any part ofa Tenn because ofan act of
God, war, executive order, ordinance, change in law, or other reasun beyond Lessor's or Lessec's control, Lessee will be relieved of its obligation to pay
rent for that T'erm, and any rent paid for that Term will either be refunded to Lessee or applied to the next Term, as Lessor chooses. Lessor shall make
that choice and notify Lessee within ten days.of being informed by Lessee that it has been prevented from using the Premises.
7 Indemnification: Lessee shall indemnify and hold Lessor harmless from any and all actions and claims resulting directly and solely from L,essee's use
of the Premises, including but not limited to claims by Lessee's licensees, or invitee, and trespassers.
8. Fixtures: All structures (including tents) shall be placed on the Premises under the approval of Lessor Any placement shall not interfere with
Lessor's use of property surrounding the Premises. Any installation shall not damage any of the Lessor's improvements on the Premises.
9 lnsurance: Lessee shall provide Lessor with proof of liability insurance before the Term begins. Liability insurance shall be in an amount ofnot less
than $1,000,000 for each occurrence.
10 Maintenance: Lessee shall maintain the Premises in an orderly manner and �vill haul away any and all trash made by Lessee, its customers, and
vendors prior to vacating the Premises.
I i Option: Fol(owing the [nitial Lease Period, this Agreement will remain in effect for an additional five years ("Option Period"). However, if Lessee
notifies Lessor 30 days in advance of a paRicular year during the Option Period that Lessee elects to not exercise its option for that year, Lessee will not
be required to pay rent for that year, and Lessor will be free to rent the Premises to someone else for that year, subject to Lessee's right of first refusal set
forth below Such election wi11 not affect subsequent years. [n the event that Lessee elects to not exercise its option for a particular year, Lessee has the
exclusive right ofrefusal to match any and all offers to lease the Premises during that year Lessor shall notify Lessee in writing ofany such offer and the
terms thereof, and provide Lessee thirty days to elect to exercise or waive the �ight of first refusal.
12. Non-compete: Lessor agrees to not sell or permit the sale of fireworks by anyone other than Lessee on any property owned or leased by Lessor
within 2 miles of the Premises.
13 Cancellation: If the Premises is sold, master leased on a long term basis other than for the sale of fireworks, or the law is changed to prohibit the
sale of fireworks for all remaining Terms, this Agreement may be canceled by either party providing thirty days written notice to the other party
