prevention and protection related activities such as inspections, plan review,administrative fees,and other
aosts related to the aforementioned.
Complete Plans, Specifications and Fee Must Accompany Application. Commencement of work without written approval of
the Fire Department's Fire Marshal or required permits or opening up for commercial activity without an approved final
inspection shall be charged double permit fee per day of operation or a minimum of$100.00, whichever is greater. All

NOTICE OF DEED RESTRICTIONS The undersigned understands that this permit may be subject to "deed" restrictions"
which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance with any
applicable deed restrictions.
UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES: If the owner has hi�ed a dontractor or
contractors to undertake work, they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work, they are advised to contact the Pasco County Building Inspection Division—Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the "contractor Block" of this application for which they will be responsible. If you, as the owner sign as the
contractor, that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco
County.
CONSTRUCTION LIEN LAW(Chapter 713, Florida Statutes, as amended): If valuation of work is $2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Lav�Homeowner's
Protection Guide" prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the "owner", I certify that I have obtained a copy of the above described document and promise in good faith to
deliver it to the"owner" prior to commencement.
- CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the information in this application is accurate and
that all work will be done in compliance with all applicable laws regulating construction, zoning and land
development. Application is hereby made to obtain a permit to do work and installation as indicated. I certify
that no work or installation has commenced prior to issuance of a permit and that all work will be performed to
meet standards of all laws regulating construction, County and City codes, zoning regulations, and land
development regulations in the jurisdiction. I also certify that I understand that the regulations of other
government agencies may apply to the intended work, and that it is my responsibility to identify what actions I
must take to be in compliance.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans, construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six(6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
just�able cause for the extension. If work ceases for ninety(90)consecutive days, the job is considered abandoned.

This Officer has reviewed the revised plans for the Fire Alarm Modifications for
Angiocath Lab, Florida Hospital, 7050 Gall Blvd. Following the review, a canditional
approval to proceed is given. Payment for permit acknowledges acceptance and
compliance of the conditions noted herein. The following items shall be considered:
1. The work shall be performed by a Florida Licensed Fire Protection contractor,
one has been identified.
2. Unless verification of an exception or exemption prohibiting the installation of
a strobe in the angio room (101) can be provided the installation af a strobe in
that room is required. In the absence of an exemption letter, installation will
be verified during inspection and Contractor may proceed with work without
revising plans.
Inspections Required:
1. Acceptance test after work has been completed.
Review and approval of the submitted plans does not relieve the contractor from the
responsibility of correcting any deficiencies noted during inspection.

SCOPE OF WORK
; SimplexGrinnell is renovating the existing fire alarm system.System includes a new smoke detectors,v �
duct detector,and strobes.
; Installation of peripheral devices shall be per NFPA 72 and the following guidelines.

Ceilin�Smoke and Heat Detectors: A Smoke detectors shall be as shown on the shop drawings, no
further than 21 feet from any wall and,if wall-mounted,between 4-10 inches from top of device to the
�: ceiling.
Duct Smoke Detectors: Duct detectors shall be 4-wire devices and provided for units that gener�ate
over 2,000 cubic feet per minute(c.f.m.)airflow. One duct detector shall be provided and mounted on
the supply duct per NFPA-90A.
Notification Appliance Devices: Chime/strobes, and strobes will be provided. The chime/strobes and
strobes shall be mounted in the wall and in locations as shown on the shop drawings.All strobe candela ,
shall be sized per NFPA-72.

Upon activation of a manual pull station or automatic detector the system shall:
activate an audible alarm tone and alarm LED on the FACP ,
� activate all designated chime and strobe circuits(on until silence)
' shutdown all AHU's via the 4-wire duct detectors �
� automatically dial the Owner-contracted UL-listed central monitoring station '
! Should a circuit go out of service (i.e.short-or open-circuit,ground,etc.),the system shall:

� (TIS)

,� TIS is designed to make the expertise of a Simplex technician available at key,scheduled times
' during the project. This service is available during normal Simplex working hours,if scheduled
five working days in advance.

i TIS includes the following activities by a factory-trained technician,when determined by Simplex ,
' to be appropriate.

' Pre-construction review of submittals and drawings with the installer.
ITechnical advice during initial start up of control panels(including transponders) before installer .
wiring is connected.
' Review of panel wiring,and preparation of a list of any items to be corrected by the installer.

Initial programming of contro)equipment.
Program editing to correct minor errors and omissions.

Assistance with one complete,functional test of the system.
� One training session for the owner's representative.
� Additional installation support may be purchased from Simplex.
' TIS contributes significantly to the installation of Simplex equipment but does not alleviate the
installer's responsibility to:
Provide skilled labor and supervision to complete the project.
Review and understand Simplex instructions for installing and testing Simplex equipment.
; Potential problems can be avoided by adhering to the following:
Do not initially start up the system,except in the presence of a Simplex technician. Simplex
, assumes no liability for damaged equipment,and warranty may be voided it this procedure is
not followed.
; Do not install smoke detectors(unless protected by plastic bags)until final construction clean up
has occurred. This prevents damage caused by dust,dirt,and paint. Detectors installed prior to
clean up may requite disassemble,cleaning,or replacement which is not covered by warranty.
Care must be taken to protect equipment during the installation and warranty period. Failures
, due to external causes(lighting surges,construction dust,water damage,etc.)will be repaired by ,

i THE SimplexGrinnell WARRANTY—HARDWARE
iSIMPLEXGRINNELLWARANTYSTATEMENT:
, SimplexGrinneil warrants to the purchaser of new SimplexGrinnell building systems product(s)that during the effective
period of the warranty such product(s)shall be free of defects in workmanship and material.
� WARRANTY PERIOD.
The warranty period will terminate irnmediately following the earliest occurrence of either of the following:
18 months have elapsed following shipment of any system or subsystem from SimplexGrinnell to the custorner site,or
; 12 months have elapsed following the final connection operation and beneficial use of all or any part of the system.
SIMPLEXGRINNELL OBLIGATION UNDER THE TERMS OF THE WARRANTY:
' SimplexGrinnell sole responsibility shall be to repair,adjust or replace at its option any SimplexGrinnell product which
fails during this period,provided purchaser has promptly reported such failure to SimplexGrinnell in
writing. Replacement parts provided under warranty of products may contain used parts which are equivalent to new
i parts when used. Replacement parts will be warranted only for the balance of the equipment warranty.
� SimplexGrinnell agrees to continue to honor all of the unexpired express warranties specified above on defe�ctive
, equipment after transfer of the equipment to purchaser's customer,provided purchaser's customer assumes the
� purchaser's obligations specified below.
� Exceptforthe express warranties stated herein SimplexGrinnell disclaims all warranties on products furnished
� hereunder, including without limitation,all implied warranties of inerchantability and fitness for a particular purpose,
I and the stated warranties are in lieu of all obligations or liabilities on the part of SimplexGrinnell arising out of or in
� connection with the performance of the products. The seller shall not be liable for a direct,incidental of consequential
loss or damage to the purchaser or user of the equipment arising out of the failure of the equipment to operate in
excess of the purchase price of said equipment. SimplexGrinnell make no warranty and now warranty shall be deemed
', to exist,that purchaser holds the goods free of claim of any third person by way of patent infringement for the like.
; PURCHASER'S OBLIGATION UNDER THE TERMS OF THE WARRANTY:
