TRANSPORTATION IMPACT/UTILITIES IMPACT AND RESOURCE RECOVERY FEES The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07, as amended The undersigned also understands, that such fees, as may be due, will be identified at the time of

will be done in compliance with all applicable laws regulating construction, zoning and land development. Application is
hereby made to obtain a permit to do work and installation as indicated I certify that no work or installation has
commenced prior to issuance of a permit and that all work will be performed to meet standards of all laws regulating
construction, County and City codes, zoning regulations, and land development regulations in the jurisdiction I also

I understand that the following restrictions apply to the use of fill
- Use of fill is not allowed in Flood Zone"V" unless expressly permitted
- If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
- If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall
- If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is required
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel, alter, or
set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building Official from thereafter
requiring a correction of errors in plans, construction or violations of any codes Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
the permit is suspended or abandoned for a period of six(6) months after the time the work is commenced. An extension
may be requested, in writing, from the Building Official for a period not to exceed ninety (90) days and will demonstrate
justifiable cause for the extension If work ceases for ninety(90) consecutive days, the job is considered abandoned

Neumann Roofing, LLC proposes to furnish labor, material, supervision, equipm�nt, fees and other
costs related to the completion of the subject property; specifications listed below:
1. Record Notice of Commencement and obtain all required permitting.
2. Tear-off existing roof system down to deck substrate and re-nail roof deck to meet State Of
Florida Building Code.
3. Provide and install Peel & Stick Underlayment
4. Provide and install new drip edge around entire perimeter, matching the e:xisting face
dimension. Color: White
5. Strike lines to ensure a uniform shingle installation.
6. Provide and install new GAF ProStart Starter Strip at drip edge system.
7. Provide and install new GAF Timberline HD Shingles with stain guard.
Color: Slate Squares: 25 Pitch: 6/12
8. Provide and install GAF Cobra Rigid Vent 3 on ridges. Vent will be covered with ridge cap
shingle to match roof system.
9. Provide and install GAF SeaI-A-Ridge Ridge Cap Shingles.
10. Provide and install new 16"valley metal in all valleys.
11. Provide and install new lead boots at plumbing vents. Boots will be painted to match
specified roof color.
12. Provide and install new gooseneck vents. Vents will be painted to match specified roof

15. Our liability insurance coverage is based on completed operations.
16. Provide a GAF Weather Stopper System Plus Limited Warranty with upgraded wind
warranty up to 130 MPH (Free of charge).
17. All estimates/contracts are subject to Building Department approval in the appropriate
county/city.
18. Payment schedule: Full amount at job completion.
Unit Pricin� Schedule for Unforeseen Conditions:
1. Replace any rotted or damaged decking $45.00 per sheet. (Neumann Roofing provides 1-
sheet of 4'x8' plywood; included in contract price).
2. Replace any rotted fascia or perlins $4.50 per lineal ft.
Neumann Roofing's Commitment to Oualitv:
• All work will be supervised by project managers to ensure quality work is preformed
according to our proposed contract.
• All work will be completed in a timely manner, excluding acts of nature b�;yond our control.
• All carpentry work will be performed in conjunction with roofing work, in a timely manner
following general workmanship quality.
• All work will be completed by skilled and knowledgeable craftsman to ensure production of
high quality of work.
• The job-site will be kept free of debris throughout the duration of the project.
• The perimeter of the project area will be swept with magnets to pick up an,y loose nails,
staples, or other various scrap sheet metal components.
• Any existing gutters will be cleaned of all organic and roof related debris prior to project
close-out.
� Neumann Roofing, LLC. is fully insured with warkman compensation and general liability
insurance.
It is understood and agreed the Neumann Roofing,LLC will be held harmless for alleged or actual damages/claims as a
result of algae, fungus,or mold. It is understood the roofmg contractor and the roofing contractor's insurance will
exclude all coverage, including defense,damages,related to bodily injury,properiy damage and clean up directly or
indirectly in whole or in part for any action brought by mold, including fungus and mildew regardless of the cost,event,
product or warkmanship that may have contributed concurrently ar in any sequence to the injury or damage occws.

Roofing,LLC shall be entitled to reimbursement for all costs of collection of unpaid
amounts by Owner/Agent and for reasonable attorney's fees and costs for the breach,
or enforcement of any terms of this entire service agreement. A service charge of 1.5%
per month will be due on all invoices thirty (30) days past due.

CONSTRUCTION LIEN LAW
Florida State Statutes 713.015: Mandatory provisions for direct contracts. (1) Any direct contract greater
than $2,500 between an owner and a contractor, related to improvements to real property consisting of
single or multiple family dwellings up to and including four units, must contain the following notice provision
printed in no less than 12-point, capitalized, boldfaced type on the front page of the contract or on a
separate page, signed by the owner and dated:

4. PRODUCT DESCRIPTION:
4.1 Royal Sovereign is a fiberglass reinforced, 3-tab asphalt roof shingles.
4.2 Camelot, Camelot II / Camelot 30, Capstone, Country Mansion, Country Mansion II, Grand
Canyon, Grand Sequoia, Grand Slate, Grand Slate II, Timberline ArmorShield II, Timberline
Natural Shadow, Timberline HD / Timberiine Prestique 30, Timberline Prestique 40 / Timberline
Cool Series, Timberline Ultra HD / Timberline Prestique Lifetime, and Timberline Majestic /
Timberline Majestic 30 are fiberglass reinforced, laminated asphalt roof shingles.
4.3 Slateline is a fiberglass reinforced, 5-tab asphalt roof shingle.
4.4 SeaI-A-Ridge, SeaI-A-Ridge Armorshield, Ridglass 8, Ridglass 10, Ridglass 12 and Timbertex Hip
and Ridge are fiberglass reinforced, hip and ridge asphalt roof shingles.
4.5 ProStart Starter is a starter strip for asphalt roof shingles.
5. LIMITATIONS:
5.1 This Evaluation Report is not for use in the HVHZ.
5.2 Fire Classification is not part of this Evaluation Report; refer to current Approved Roofing
Materials Directory for fire ratings of this product.
5.3 Wind Classification:
5.3.1 All GAF shingles noted herein are Classified in accordance with FBC 'Tables 1507.2.10 and
R905.2.6.1 to ASTM D3161, Class F or ASTM D7158, Class H, indicating the shingles are
acceptable for us in all wind zones up to 150 mph. Refer to Seckion 6 for installation
requirements to meet this wind rating.
5.3.2 Classification by this method applies to buildings less than 60 feet high and with Wind Exposure
B and C only in an Occupancy Category of I or II.
5.3.3 For buildings falling outside the constraints in 5.3.1 or 5.3.2, calculations by a qualified design
professional in accordance with UL2390 / ASTM D7158 / ASCE 7 may be conducted for the
specific project with data for the specific shingle. Contact the shingle manufacturer for data
specific to each shingle.
5.3.4 All GAF hip & ridge shingles noted herein have been evaluated in accordance with ASTM D3161,
Class F. Refer to Section 6 for installation requirements to meet this wind rating.
5.5 All products in the roof assembly shall have quality assurance audit in accordance with the
Florida Building Code and F.A.C. Rule 9N-3.
6. INSTALLATION:
6.1 Underlayment:
6.1.1 Underlayment shall be acceptable to GAF Materials Corporation and shall hold current FBC
Approval, or be Locally Approved per Rule 9N-3, per FBC Sections 1507.2,3 and/or 1507.2.4.

6.2 Asphalt Shingles:
6.2.1 Installation of asphalt shingles shall comply with the manufacturer's current published
instructions, but not less than four (4) fasteners per shingle in accordance with the
requirements of FBC Section 1507.2 or FBC Residential Volume Section R905.2, with the
following exceptions:
➢ Camelot, Camelot II / Camelot 30, Grand Slate II and Country Mansion II require minimum
five (5) fasteners per shingle
6.2.2 Fasteners and fastener placement shall be in accordance with manufacturer's current published
installation instructions.
6.2.3 Where the roof slope exceeds 20 units vertical in 12 units horizontal (166-percent slope),
special methods of fastening are required. Contact GAF Materials Corporation for details.
6.3 Hip & Ridge Shingles:
6.3.1 Installation of Ridglass 8, Ridglass 10 and Ridglass 12 asphalt shingles shall comply with the
manufacturer's current published instructions with a minimum two (2) fasteners per side and
asphalt roofing cement.
6.3.2 Installation of SeaI-A-Ridge and SeaI-A-Ridge Armorshield asphalt shingles shall comply with
the manufacturer's current published instructions with a minimum two (2) fasteners per
shingle.
6.3.3 Installation of Timbertex Hip and Ridge asphalt shingles shall comply with the manufacturer's
current published instructions with a minimum two (2) fasteners per shingle and beads of
Sonneborn NP1 Gun Grade Polyurethane Sealant or Henkel PL Roofing and Flashing Sealant.
7. LABELING:
Each unit shall bear a permanent label with the manufacturer's name, logo, city, state and logo of the
Accredited Quality Assurance Agency noted herein.
8. BUILDING PERMIT REQUIREMENTS:
As required by the Building Official or Authority Having Jurisdiction in order to properly evaluate the
installation of this product.
9. MANUFACTURING PLANTS:
Contact the named QA entity for information on which plants produce products covered by Florida Rule
9N-3 QA requirements.
