This O�cer has reviewed the submitted plans for the build out of Humana Guidance
Center, 7920 Gall Blvd Zephyrhills. Plans are approved as submitted with the following
conditions:
1. Emergency Lights are required inside each restroom.
2. Any existing fire separation shall remain as per plans, should any
modification(s)be required they shall require plan submission and inspection.
3. Places of Assembly Permit shall be required for training area.
4. Combination Emergency/Exit light units are allowed.
5. Fire Alarm plans from a Florida licensed fire alarm contractor must be
submitted for review, approval and permitting.
6. Fire Sprinkler plans from a Florida licensed fire sprinkler contractor must be
submitted for review, approval and permitting.
Payment of permit fee indicates acceptance of the requirements of these review
comments.
Inspections Required:
1. FinalInspection.
2. Firewall inspection, onlv if any modifications occurred durin work.
3. Final Fire Alarm acceptance test/inspection, responsibility of Fire Alarm
Contractor.
4. Sprinkler rough-in inspection and final inspection, responsibility of Fire
Sprinkler Contractor.

�R�►NSPORTATION IMPACTIUTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
hat Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of
ise in existing buildings, or expansion of existing buiidings, as specifed in Pasco County Ordinance number 89-07 and
�0-07, as amended. The undersigned also unde�stands, that such fees, as may be due, will be identified at the time of
�ermitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
eceiving a "certificate of occupancy" or final power release. If the project does not involve a certificate of occupancy or
inal power release, the fees must be paid prior to permit issuance. Furthermore, if Pasco County Water/Sewer Impact
ees are due, they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.

- If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume" will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
- If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction, I certify that fill will be used only to fill the area within the stem wall.
- If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties, the owne� may be cited for violaiing
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill, an engineered drainage plan is requlred.

2. The Wmic shall be decmod complete upon obtaining an approval at the final building inspoction by
Humans and any otha pn�p�r aut6orities if applicable,obtaining any necessary Cutificate of Occupaacy,
and receipt by Humana of all s�boontrsctor lien waivers.Pos,�ession of the Premises shsll be delivaed to
Humana or its designa after issuance of a Final Permanent Certificate of Occupancy where applicable,and
written punch list has boen developod,agr+eed to,signed amd dated by Humana and Contractor.
3. The time of completion nnay be extendod if delay in the}n+ogress of the Work is caused diroctly and
solely by an act of negligence by Humam,material changes in the Work sppmved by Humana in writing,
force majeure events,or a cause not within Coutractor's c+easonable conh+ol.Lack of materials or workforce
on the part of Contractor shall not be considered a force maje�u�e event.
Artlde 3.LUMP SUM CONTRACT:

Artide 4.CHANGES IN WORK:
Changes in the Work may bc accomplishod after the exocution of the Contract by a Chaage Oider.A Change

r�►taave for written appmval.Chaage Orders shall u�clude: description of the chsnge in the Work,

Artide 6.APPLICATION FOR PAYMENT:
1. At the completion of the Work,Contractor sball submit a payment roquesVinvoice for the lump sum
oost of the Work set forth in Article 3,which roqu�t sb�all ideatify any retainage previously agreod.
2. Whene applicable,Contrsctor shall svbmit to Humana releasos or lien waiveas of all liens(including
�bvontrsctor,sub-subcontractor,msterial meq and laborers)(partial or final)showing that all payments and
or indebtedness from d�e previous period requisition 6ave ba,�t duly satisfied.
3. Humana shall review Contrecto�s roquisition for funds due as set out in this Article 6,and upon
apprvval,shall pay Contractor within 4S calendar days of such approval.
4. Payment(s)otherwise due may be widiheld on account of defective Wak that preveats the use of the
space, for damsge by Contractor or its ageats to others,or failure to make payments properly to
subcontiactors for materials or labor(whether or not a lien has been filod). Fiml payment wiU bc made no
later than 45 days after completian.On construction projocts,before final payment is msde to Contractor,
Contra�ctor shall furnish final lien waivers from all sub-conhactors end material suppliers clearing all
"Notices to Owner"or a bond satisfactory to Humana indemnifying against any liea,and a Contracta's Final
Affidavit.
Ardck 7.DISBURSEMENTS:
Should Contractor ncglect or refuse to pay,within thirry(30)days after ft falls due,any bill legitimately
incucred by Contr�tor heirunder in relation to the Work,Humans,after giving the Contractor twenty-four
(24)hours written notice of intention so to do,shall have.the right(but not the obligadon)to pay such bill
directly and deduct the amount fi+om the Contract ainouat.

Artlde 8.TERMINATION OF CONTRACT:
1. Should Contractor fail to execute the Work pmpecly,or fail to pea�'am any provision of the Contnet,
Humana may,after written notice to Contiactor, immediately termiaate the Contract,in whole or in patt,and
tak�possession of the site,tnaterials,and tools to finish the Worlc,without further obligation or liability to
Contr�ctor. Humana shall have all ranedics available to it w�dv this Coatract,at law or in oquity.
2. Should Humaaa fail to pay Contractor any undisputod payment when due,Contractor upon sevw(7)
days written notice,may stop Work undl such time as paymdrt is received.
3. Notwithstanding anythiag in this qrticle to the oontrary,Humana may,at any time upon s�wen(7)
days written nodce,tuminate this Contract. In such evrnt,Humam wiU pay Contractor nd amounts,if any
ar�due,for�ompleted Work by Contractor up to the effoctive date of teiminstion.
4. Contrsctor shall,as a conditio�of roceiving the paymeats memioned in this Article,exer,ute and
deliver all such papers and talce all such steps,including the legal assignment of his contracwal rights, as
Huma��a may require for the�rpose of fully vesting the rights and benefits of Convactor undtr such
obligations aad oommitrnents to Humana,

Artide 9.INDEMNITY AND IIVSURANCE:
1. Connactor shall maintain such insurance as will pr�tect fmm claims under workers compensadon
acts and otha employee benefits acts as required by the law of the state wha�e the Work is Pert'orn�ed, fi+om
claims for damages because of bodily injury, including death, and from clsims for damages because of
bodilY mlurY+ including death, and fcvm claims for damages co pro�erty which may arise out of snd during
operations under this Contract. Contractor shall maintain a minimum of S 1,000,000.00 for general liability
per occumeace, 52,000,000 in the agg�+egate. Contractor shall maintain employoe t6eft and dishonesty
ooverege in an anrount not less than 51,000,000, Contrsctor shatl requine its subcontrsctors to have the same
amount of insurance or provide evidence that Contractor's insurance cov�cs its subcontractors, Contractor's
insurance shall be primary and pay beforc aay insuiance of Humana and aU policies of insuraace shall
contaia an duiorsement waiving any right of subrogation against Hwnsna with regard to genp�al liability
coverage. Evidence of alI ins�uance ha�der shall be pmvided by CertiScate of Insurance acceptable to
Humana prior to starting Work. Certificates shall contaia a pmvision prohibiting msterial modification or
cancellation of the coverage except upon at least 10 days prior writtea notice to Humana. No inswance
catriad her�undtr shall be constiued to limit the liability of Contractor or it agents.
2. To the fWl�st extent pennittod by law, Cont�actor shall indannify and hold hamrniless Hmnana� its
officers,directors,agants and empby�ees 5+om and again4t claims,damages, los.ses and expa�ses,including but
not limited to artton�s fa�,arising out of or resulting from performance of the Woric, for any claim,damage,
loss or expeose awibutable to bodily injury,siclrness,disease or death,or to iujury to or des�uction of taz�ible
prapaty inchiding loss of use raulting therefram, to the extent caused in w►twle or in pact by aogligent or
willful scts or omissions of Coa��, its suboonvsctors, anY�one directly or indiratly empbyod by tlxm ar
any�ne for whose acts they may be liable, regardless of whether or not such claim, damsge, loss�expa�se is
caused in part by a party indemniSed heramder.

applicable federal, stste, aad local laws and a�+dinu�s �nnd all orders, niles, and regulations iasued
therau�der(including any provisions, rcpr�atadons or agra�ants, or coatrsctual clauses required thereby
to be inclnded or incorporated by refd�eacx or operation of law in this Agroemeat), inch�ding but not limited

child lsbor pr�visions set fo�in 5ection 12(a)di«+eof,and in c�apliance with the United States Citiunship
