TRANSPORTATION IMPACTlUTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the constniction of new buildings,change of
use in existing buildings, or expansion of existing buildings, as specified in Pasco Courriy Ordinance number 89-07 and
90-07,as amended. The undersigned also understands,that such fees,as may be due,will be identified at the time of
permitting. It is Turther understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a"certificate of occupancy"or final power release. If the project does not involve a cerdficate of occupancy or
final power release,the fees must be paid prior to permit issuance. Furthermore,if Pasco County Water/Se�r Impact
fees are due,they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW(Chapter 713,Florida Statutes,as amended): If valuation of work is$2,500.00 or more, I
certify that I, the applicant, have been provided with a copy of the `Florida Construcbon Lien Lawr—Homeovmer's
Protection Guide"prepared by the Florida Department of Agriculture and Consumer Afhairs. If the applicant is someone
other than the"owner°,I oertity that I have obtained a copy of the above described documeM and promise in good faith to
deliver it to the"owne�'prior to commencement.
CONTRACTOR'S/OWNER'S AFFIDAVIT: I certify that all the infortnation in this application is accurate and that all work
wilt be done in compliance with all applicable laws regulating construction,zoning and land development. Application is
hereby made to obtain a pertnit to do wo�1c and installaGon as indicated. I certify that no �nrork or installation has
commenced prior to issuance of a pertnit and that all work will be perfornied to meet standards of all laws regulating
construction, County and Ciry codes, zoning regulations, and land development regulabons in the jurisdiction. I also
certify that I understand that the regulations of other govemment agencies may apply to the intended work, and that it is
my responsibility to identiiy what actions I must take to be in compliance. Such agencies inGude but are not limited to:

- Use of fill is not allowed in Flood Zone"�//"unless expressly permitted.
- If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume"will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
- If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
consWction,I certify that fill will be used only to fill the area within the stem wall.
- If fill material is to be used in arry area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely aifed adjacent properties,the owner may be ated for violating
the conditions of the building pertnit issued under Uie attac�ed permit application,for lots less than one(1)
acre which are elevated by fill,an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER,I promise in good faith to inform the owner of the pertnitting conditions set forth in
this affidavit prior to commencing co�truction. 1 u�derstarxl that a separate pertnit may be required for electrical work,
plumbing, signs,wells, pools, air conditloning, gas, or other installations not specifically inGuded in the application. A
permit issued shall be const�ued to be a lioense to proceed with the work and not as authority to violate,cancel,after,or
set aside any provisions of the technical codes,nor shall issuance of a permit prevent the Building Official from thereafter
requiring a coRection of errors in plans,construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six moMhs of permit issuance,or if work authorized by
the permit is suspended or abandoned for a period of six(6)months after the time the work is commenced. An extension
may be�quested, in writing,from the Building Official for a period not to exceed ninety(90)days and will demonstrate
justifiable cause for the extension. If work ceases for ninety(90)consecutive days,the job is considered abandoned.

AGREEMENT made this �' day of M!��G�� ,2013,by and between BIG DCX'FENCE INC.hereina$er
referred to as"BDF"and,hereinafter referned to as"Customer."
In consideration of the mutual covenants contained herein and the consideration to be paid by Customer to Big Dog Fence
Inc.,Yt is agre�ed as follows:
1.Castomer shatl pay to Big Dog Fence Inc.the sum of$ 3�/b'6.`'`' for the work described herein.Payment shall be
made to BDF by the Customer upon completion,unless otherwise specified.
2.BDF agrees to furnish all necessary labor and materials to construct the improvements described
herein as per the specifications and drawings,if any,shown on the reverse side.BDF further warrants a11 materials will be as
specified and that all work will be completed in a workmanlike manner according to standard practices.
3.If this is a residen6al job,Customer represents and warrants that he/she is the owner of the property or Renter described
herein.If this is a commercial job,Customer represents that he/she is the owner of the property described herein,or an
Authorized Representative,or Agent representing the following Business(es)or Org,anization,titial as,
or renter/contractor who has a contract to
improve property owned by
and that part of the improvements contracted for by the owner include the work described herein.
4.Customer agrees to locate,relocate,stake and clear fence lines,and assumes full responsibility for same.Customer
agrees to indemnify and hold BDF hazmless for any liability or damages that may result from Customer's failure to properly
locate,stake and clear fence lines.Customer further agrees to advise BDF of any deed,subdivision,or other restrictions which
may affect const�vction of the fence or other improvements to be constructed by BDF,and agrees to indemnify and hold BDF
harmless from any liability or damages that may result because of the failure to comply with deed,subdivisioq or other
restrictions.
5.BDF will call the local Utility Locating Service for public utilities only.BDF dces not assume any responsibility or
liability for any damage of any nature to underground obstructions not marked or flagged by customer.Customer agrees to
clearly"mark"any underground obsttuctions other than public utilities in the area of the fence line or the improvements to be
constructed by BDF.Customer assumes responsibility for any damage to underground obstructions caused by Customer's
failwe to locate and mark accurately with paint or flags,their existence.For purposes of this Agreement,underground
obstructions inciude but are not limited to all improvements constructed underground including utility pipes,sprinkler pipes,
cables,wires,foundations,and the like.
6.It is agreed by and between the parties,�that any alteraxion or deviation from the above specifications involving extra cost
is to be performed by BDF and will become an extra charge over and above the price stated herein.
7.Customer agrees to pay in fuil the price stated herein upon completion of work.Any failwe to pay within tlu�ee(3)days
after completion of work,excluding Sundays and Holidays shall be considered a default and BDF shall be entided to recover
the full amount of the invoice plus interest(finance charge)at the rate of 1 1/2%per month(18%per annum)until the contract
price is paid in full.In the event it becomes necessary for BDF to take collection action,Customer agrees to pay all costs of
collecrions including reasonable attomey's fees and costs.
8.It is agreed between the parties that BDF will retain S 125.00 from deposit if said contract is cancelled by customer and
customer further agrees to a 25%restocking fce plus retum freight cost for ail special orders.No return for custom orders.
9.It is further agreed between the parties that the fencing materials shall remain personal property until the contract price is
paid in fu1L In the event the Customer fails to pay the contract price,BDF shall have the right to peaceably repossess and
remove all materials instatled by it on the premises.Customer fiuther agrees that BDF shall have all of the rights of a secured
party,as the same is defined under the Florida Uniform Commercial Code,and that said rights shall be cumulative and in
addirion to any other rights which BDF inay have in accordance with Florida Law.
10. If Credit Cards are used to pay totnl,or any amount toward total,the amount charged through terminat will be assessed
an extra 3.5%processing fee.Credit cazd checks will not be assessed the extra.fee.
I 1.BDF is not responsible for warping or cracking of wood products.
12.BDF will warranty labor for installation of residentiaUcommercial for a period of 1 year.
13.BDF will warranty labor for installation,(residentiaUcommercisi gate operator)for period of 90 days.
14.'This agreement is the entire agreement of the parties and cannot be changed except in writing signed by both parties.
There are no warranties or representaxions except as are expressed herein.BDF specifically disclaims all EXPRESS and
IMPLIED WARRANTIES,including any WARRANI'y OF MERCHANT ABILITY or FITNESS FOR A PARTICULAR
PURPOSE.
