TRANSPORTATION IMPACTNTILITIES IMPACT AND RESOURCE RECOVERY FEES. The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings,change of
use in existing buildings,or expansion of existing buildings,as specified in Pasco County Ordinance number 89-07 and
90-07, as amended The undersigned also understands,that such fees,as may be due,will be identified at the time of
permitting It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to

- If the fill material is to be used in Fbod Zone "A", it is understood that a drainage plan addressing a
"compensating volume"wi►I be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of Florida.
- If the fill material is to be used in Flood Zone "A" in conr�ction with a permitted building using stem wall
construction,I certity that fill will be used only to fill the area within the stem wall.
- If fill rrmterial is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent propeRies,the owner may be cited for violating
the conditions of the building permit issued under the attached permit application, for lots less than one (1)
acre which are elevated by fill,an engineered drainage pfan is required.
If I am the AGENT FOR THE OWNER, I promise in good faith to inform the ovmer of the permitting conditions set forth in
this affidavit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs, wells, pools, air conditioning, gas, or other installations not specifically included in the application. A
permit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel,atter,or
set aside any provisions of the technical codes,nor shall issuance of a pertnit prevent the Building Official from thereafter
requiring a correction of errors in plans,construction or violations of any codes. Every permit issued shall become invalid
unless the work authorized by such permit is commenced within six months of permit issuance,or if work authorized by
the permit is suspended or abandoned for a period of six(6)months after the time the work is commenced An extension
may be requested, in writing,from the Building Official for a period not to exceed ninety (90)days and will demonstrate
justifiable cause for the extension. If work ceases for ninety(�)consecutive days,the job is considered abandoned

SCOPE:
This NOA is being issued under the applicable rules and regulations governmg the use of construction materials. The
documentation submitted has been reviewed and accepted by Miami-Dade County RER-Product Control Section to be
used in Miami Dade County and other areas where allowed by the Authority-Having Jurisdiction(AHJ).
This NOA shall not be valid after the e�uation date stated below The Miami-Dade County Product Control Section
(In Mianu Dade Counry) and/or the AHJ(in areas other than Miami Dade County)reserve the right to have this product
or material tested for quality assurance purposes. If this product or material fails to perform m the accepted manner.the
manufacturer will incur the e�cpense of such testing and the AHJ may immediately revoke,modify,or suspend the use
of such product or material within their jurisdichon. RER reserves the nght to revoke tlus acceptance,if it is
determmed by Miami-Dade County Product Control Section that this product or material fails to meet the reqwrements
of the applicable building code.

RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been no change
in the applicable building code negatively affect�ng the performance of tlus product.
TERMINATION of tlus NOA will occur after the e�piration date or if there has been a revision or change m the
matenals,use,and/or manufacture of the product or process. Misuse of this NOA as an endorsement of any product,for
sales, advertising or any other purposes shall automatically ternunate this NOA. Failure to comply with any section of
this NOA shall be cause for terminarion and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami-Dade Counry, Florida, and followed by the
e�pirahon date may be displayed in advertising lrterature If any portion of the NOA is displayed, then it shall be done

WOUD DECK SYSTEM LIMITATIONS:
1 A slip sheet is required with Ply 4 and Ply 6 when used as a mechanically fastened base or anchor sheet.
GENERAL LIMITATIONS:
1 Fire classification is not part of this acceptance, refer to a current Approved Roofing Matenals Director�% for fire
ratings of this product.
2 Insulation may be installed in multiple layers. The first layer shall be attached in compliance with Product Control
Approval guidelines. All other lay�ers shall be adhered m a full mopping of approved asphalt applied within the
EVT range and at a rate of 20-401bs./sq.,or mechanically attached usmg the fastening pattern of the top layer
3 All standard panel sizes are acceptable for mechanical attachment. When applied in approved asphalt, panel size
shall be 4'x 4'm�imum.
4 An overlay andlor recovery board msulation panel is requu-ed on all applications over closed cell foam msulations
when the base sheet is fully mopped. If no recovery boazd is used the base sheet shall be apphed usmg spot
mopping with approved asphalt, 12" diameter circles,24" o.c.,or stnp mopped 8"ribbons m three rows,one at each
sidelap and one down the center of the sheet allowing a continuous area of ventilation. Encucling of the stnps is
not acceptable. A 6" break shall be placed every 12' in each ribbon to allow cross venUlat�on. Asphalt application
of erther system shall be at a mimmum rate of 12 lbs./sq.
Note: Spot attached systems shall be limited to a mazimum design pressure of-45 psf.
5 Fastener spacing for insularion attachment is based on a Minimum Characteristic Force(F')value of 27� lbf.,as

prepared,signed and sealed by a Florida Registered Engineer,Architect,or Registered Roof Consultant ma��be
submitted. Said revised fastener spacmg shall utilize the withdrawal resistance value talcen from Test�ng
Application Standards TAS 105 and calculations in comphance with Roofing Application Standard RAS 117
7 Peruneter and corner areas shall comply with the enhanced uplift pressure requirements of these areas. Fastener
densrties shall be increased for both insulation and base sheet as calculated in compliance with Roofing Applicataon
Standard RAS 117 and/or RAS 137 Calculations prepared, signed and sealed by a Flonda registered Professional
Engineer, Registered Arctutect, or Registered Roof Consultant (When this limitation is specitically referred
within this NOA,General Limitation#9 will not be applicable.)
8 All attachment and sizing of perimeter nailers,metal profile,and/or flashmg ternunation designs shall conform with
Roofing Application Standard RAS ll 1 and applicable wind load requirements.
9 The maximum designed pressure hrmtation listed shall be applicable to all roof pressure zones(i.e. field,penmeters,
and corners).Neither rational analysis,nor e�ctrapolation shall be permitted for enhanced fastemng at enhanced
pressure zones(i.e.perimeters,etrtended corners and corners). (When this limitation is specifically referred
