See fee sheet attachment for cost of single family home
Again as I mention over the phone I only sent one fee sheet for each sq ft of home there are
(3) 1793 sq and (3) 1819 sq ft.
If you look at the top of fee sheet where address is it will show you the sq ft you would use
that fee sheet for that particular sq ft home whether 1819 or 1793. So three will cost
$14,027.28 and three will cost $13,995.78.
The Transportation impact fee is the only fee where our ordinance would allow for 25 o be
paid at permit pick up and the 75 aid before permanent power would be released.---- or you
could pay entire impact fee amount.
All other impact fees total amount would have to be paid along with the cost of permit.
Also need the Notice of commencement upon pick up. Give notice in advance when you are ready
to pick up so I may have the permits ready.
7ust need correct address for the last home soon as you get that to me I would be able to
process.

The Owner and Contractor Agree as Set Forth Below:
ARTICLE 1. - CONTRACT SUM: The Owner shall authorize payment to the Contractor for the
performance of the construction work, subject to additions and deductions by approved change order, the
Contract Sum of One Hundred Eleven Thousand Five Hundred Eighty and 00/100 Dollars ($111,580.00). The
building to be built on this lot is Leah I!.
ARTICLE 2. - THE WORK AND COMPLETION TIME: The Contractor agrees to fumish all labor,
materials, equipment, permits, licenses, and services for the proper completion of the above-identified property
in accordance with the Bid Documents and Specifications. The Contractor agrees that materials supplied are
guaranteed to be as specified. All work shall be satisfactorily completed within One Hundred Fifty (150)
calendar days after the Notice to Proceed is issued. All work shall be completed in a workmanlike manner
according to standard practices. If the workmanship does not comply with the Community Development
Division's

acceptabls standards, work deemed to be below standards wil! be rebid and deducted from final payment.
This action can prevent the contractor from bidding on any further work or projects. Substantial completion of
the rehab is determined when the job write-up is 100% completed. If#he rehab is not substantially completed
by the due date, the contractor can not subrnit a bid for new projects. Upon substantial completion, #he
cantractar i�as seven {7} days to complete the punch list items and fourteen (14) days to submit Fina( Release
of Llens #rom the contractors, sub-contractors, the notice to owner and al! consfruction required documents. If
#he contractor does not meet the fourteen (14) days for the Lien Release, he will not be able to bid on any new
projects until final paperwork has been submitted. The premises will be unoccupied during the course of
construction.
ARTICLE 3. - TIME OF COMMENCEMENT: No work shall be commenced by the Contractor until
he/she has received a written Notice to Proceed from the Community Development Division. The Notice #o
Proceed wil( be issued within thirty (30) days from#he date of the Contractor signing this Contract, or if it is not,
the Contractor has the option of withd�awing his bid and proposal. Work shall begin no later than seven {7)
days after the Notice to Proceed is issued. If work is not subsfantially completed within agreed upon fime
period, Contractor will be penalized One Hundred and 00/100 Dollars ($10Q.00) per day liquidated damages on
new construction and Fifty and 00/100 Dollars ($50.OQ) per day liquidated damages on rehabilitativn projects
for every working day until substantial completion, unfess approval for extension has been agreed upon in
writing by Owner, Contractor, and the Community Development.Division. This provision is contingent upon
strikes, accidents, Acts of God, or other events beyond the control of Pasco County and the Contractor.
ARTtCLE 4. - PAYMENT: The Contractor will be paid according fo.the draw schedule as stated in the
Program Policy Sta#ement for the County's rehabilitation program after the Communi#y Development Division
conducts an inspection certifying that the work has been completed according to the contract. Final payment
due the Contractor wift be rrtade after the Cantractor has fumished the Owner, in care of the Community
Development Division, satisfactory release of liens or claims for liens by subcontractors, lai�orers, and material
suppliers for completed work ar instalfed materials. All such releases must be notarized. Payments will be
made in accordance with Florida Statutes, Chapter 218.735-74, Local Govemment Prompt PaymeRt Act
considered due upon receip# by the Clerk of the Circuit Court and paid upon approval by the Board of County
Commissioners. Within thirty (30) days of completion of the contract, the contractor shall submit the request
for final payment alang with the required documentation for payment. The County sMall not be responsible for
any requests after the 30 days.
ARTICLE 5. -CONTRACTOR'S INSURANCE: The Contracto�shall be responsible for all damages to
person or properEy that occur on the job site or adjacent thereto as a result of his fault or negligence in
connection with this contract. The Contractor shall, prior to commencing work, fumish evidence of required
comprehensive liability insurance. The Contractor shall also furnish evidence of cove�-age in accordance with
Florida Workers' Compensation Laws. Contractor shall furnish Community Development Division wifh new,
updated or renewed insurance coverage to continue to bid on projects.
ARTICLE 6. - SUBCONTRACTS: A!! subcontractors shali be bound by the tenns and conditions of
this contract insofar as it applies to their work, but this shall not relieve#he Contractor from the responsibility to
the Owner for the proper completion of all work to be executed under this Contract, and he/she shall not be
released from this responsibility by any subcontractual agreement he may make with others. The Contractor
shalf be required#o fumish to the Owner and Community Development Division in writing, a list of the�ames of
the subcontractors p�oposed for the principal portions flf the work at this contract signing. The Contractor shall
not employ any subcontractor to whom the Owner or the Community Development Division may have a
reasonable objection. A current form for listing subcontractors is available on the Community Development
Division website. The notice to proceed will not be issued until#he required scab list has been completed for this
contract and submitted to the Community Development Division.

ARTICLE 7. •GENERAL RESPONSIBILITIES OF THE CONTRACTOR:
7.1 The Contractor shall be licensed and supervise and direct the work using his best skills
and attention.
7.2 The Contractor shall provide the Owner and Community Development Division with
evidence that he/she has secured and paid for all licenses and permits necessary for the
praper execution of the work; and, upon completion of the job, shall provide evidence
that all work has been inspected and approved by the apprapriate building inspectors.
7.3 The Contractor shall not employ on the work any unfit person or anyone not skilled or
licensed, if appropriate, in the task assigned to him.
• 7.4 The Contractor shall be responsible far the acts and omissions of al1 his employess, and
all subcontractors, their agent and empfoyees, and all other persons pertorming any of
the woi-ic.
7.5 The contractor shail seek approval from Community Development Division prior to
substituting any materials or products deemed °EquaP' to the specific materials or
products listed in the most recent Bid Specifica#ions ar Work Write-Up.
7.6 The Contractor shall not assign this contract without written consent of the Owner and
the Community Development Division. Any request for assignment must be addressed
to the Community Development Division.
7.7 The Contractor at all times shafl keep the premises free from accumulation of waste
materials or rubbish caused by thEs work.
7.8 The Contractor shall permit the Community Development Division to examine and
inspect the construction work under this contract at any reasonable time.
7.9 The Contractor shall fumish the Owner and the Community Development Divisian with
the names, addresses, and te(ephone numbers of the firms to be contacted for service to
the various appliances or other equipment that was installed or repaired under this
contract.
7.10 The Contractar shall indemnify and save harmless the County, its agents and
employees, and the Owner from liability for any injury or damages to persons or property
resulting from #he Contractor's prosecution of work pursuant to the provisions of this
contract.
7.91 The contractor shall be responsible to submit all requests for payments, change orders,
release af liens, subcontractor iist, etc., on the most current forms obtained from the
Community Deveiopment Division website for contractors.
7.12 Ail products and materiais used in the rehabilitation of Community Development Division
projects shall be made by American manufacturers, if available, and meet the iatest
energy and water conservation requirements.

ARTICLE 8. -GENERAI. RESPONSIBIl.ITIES OF THE OWNER:
8.1 The Owner shail permit the Contractor ta use, at no extra cost, existing utilities such as
light, heat, power, and water necessary for the proper execution and comple#ion of the
work.
8.2 The Owner and occupants shall cooperate with the Contractor to facilitate the perfor-
mance of the work and are responsible for the removal or protection of rugs, furniture,
and clothing, if necessary.
8.3 The Owner and occupants shall be responsible for the proper use and care of the
property including all equipment and appliances.
8.4 The Owner shall provide the Contractor wifh access to the property during normal
working hours on normal working days.
ARTICLE 9. -GENERAL RESPONSIBILITIES OF THE COMMUNITY DEVELOPMENT DIV131 N:
9.1 The Community Development Division shall provide general administration of the
contract, but it is understood by all parties that the County is not a party to this Contract.
9.2 The Community Development Division shall make frequent inspections of the work while
it is in progress to determine if it is proceeding in accordance with the contract dacu-
ments.
9.3 The Community Developmen# Division shall have the authority to reject work which does
not conform to th� contract documents.
ARTICLE 10. - CONTRACT DOCUMENTS: This contract consists of the Bid Proposal, the Work
Write-Up, including any drawings or plans, the General Bid Specifications, and any insurance policies specified
in Article 5 above.
ARTICLE 17. - CHANGES IN THE WORK: Except in an emergency endangering life or property, no
change in the work shall be made by the Contractor unless he has received a prior written order signed by the
Owner and authorized representatives of the Community Development Division. The contract sum and the
contract time may be changed only by Change Order. No extra costs will be paid to the Contractor when he
has neglected to properly eva(uate the extent of the construc#ion work. Change orders need to be in line with
bidding amounts or pricing.
ARTlCLE 12. - GUARANTEES AND WARRANTIES: The Contractor shall guarantee to coRect any
wvrk that fails to conform to the contract documents and shall correct such defects due to faulty materials,
equipment, or workmanship which appear during the progress of the wo�lc or within a period of one year from
the date of final inspection and acceptance or such longer periods of time as may be speafied by law or by
terms of any special guarantees required by the contract documents. The provisions of this Article app[y to
work done by subcontractors as well as work done by the Contractor. Furthermore, the Contractor shall
furnish the Owner, in ca�e of the Community Development Division, with all manufacturers' and suppliers'
written guarantees and warranties covering materiafs and equipment furnished under the contract. These
dacuments are ta be neatiy organized and bound in a �le type folder and submitted with the final pay request.
1f a warran#y item malfunctions or breaks, the Contractor shall contact the Owner within 24 hours to schedule
the repair ar replacement of the item. The warranty item should be repaired or replaced within seven (7)days.

ARTICLE 13. - TERMINATION OF CONTRACT BY THE OWNER: If the Contractor defaults or
neglects to carry out the work in accordance with the contract documents or fails to perform any provisions of
the contract, the Community Development Division or the Owner acting through the Community Development
Drvision may, after seven (7) days' written notice, terminate this contract and finish the work by whatever
method he deems expedient. If the unpaid balance of the contract sum exceeds the expense of finishing the
work, such excess shall be paid to the Cont�actor; but if such expense exceeds such unpaid balance, the
Contractor shall pay the difference to the Owner or#he Corrtmunity Development Division. Failure to pay the
unpaid baiance wilf result in disbarmenf from bidding on future Community Development Division projects.
ARTICLE '14. - TERMINATION OF C�NTRACT BY THE CONTRACTOR: If the Owner faiis to
perForm any applicable p�avisions of the contract, the Contractor may, upon seven (7) days' written notice to
the Owner and the Community Development Division, terminate the contract, and recover from the Owner
payment for all work satisfactorily completed and all other such remedies as are otherwise specifed by iaw
ARTlCLE 15. -GENERAL CONDITtONS:
15.1 The Work Write-Ua shall take precedence ove� the basic specification and, when i�
conflict, the material, equipment, or workmanship called for in the "Woric Write-Up"wi11
be required.
15.2 The Drawinqs of floor plans are diagrammatic only, illustrating the gene�al intention of
the Owner; they do not show all the work required, exact dlmensions, or construc#ion
details.
15.3 The 2090 Bid Speci�cations are detailed in a separate booklet available to the Owner
and the Contractor from the Community Development Division.
95.4 Workmanship shall be done in accordance with the standards of the several trades
known as a "workmanlike manner."
15.5 Materials shall be new, in good condition, and of standard grade unlsss otherwise
agreed in writing before their delivery to the job.
15.6 Repairs shalf be made by the Contractor to all surfaces damaged by the Contractar
resulting from his or his subcontractor's work under this contract at no additional cost
to the Owner. Where "repair of existing work" is called for by the contract, the feature
is to be placed in "equal to new candition" either by patching or replacement; all
damaged, loose, or rotted parts shal! be removed and replaced, and the finished work
sha(I match adjacent work in design and dimension.
15.7 Bids or Proqosals will be submitted at 8idders risk and the Owner or the Community
Devefopment Division reserves the right to reject any or all bids or proposals. All bids
or proposals are to be typed -nof hand wriften.
15.8 Building Codes -All work to be done shall be subject to the regulations of local building
cfldes as interpreted by the applicable City or Caunty buifding department/division
whether or not covered by#he specifications and drawings for the work.
15.9 Cleanup and removal from the site of all debris and waste materials resulting from his
work shall be the responsibility of the Contractor. Materials and equipment that have
been removed and replaced as part of the work shall belong to the Contracto� and
shall be removed from the site.

95.10 Trade Names are used in the basic specification to estabfish quality and type of
material required; exact material to be used on a specific property will be described in
the "Work Write-Up"for the particular property.
15.11 AdiacQnt Propertv- When adjacent property is affected or endangered by any work
done under this contract, it shali be the responsibility of the Contractor to take
whatever steps are necessary for the protection of the adjacent property and to notify
the Owner thereof of such hazards.
15.12 Pavroil Records of empioyes's hours worked and wages paid on construcfion jobs
sha(I be made available to the Community Developrnent Division, if requested.
� 15.13 The Use of Lead-Based Paint on applicabie surfaces of any residential s#ructure
undergoing Federally assisted construction or construction under any program with
federal assistance is prohibited. Any surface which contains lead-based paint shatl be
considered an immediate health hazartf.
15.14 The recognized amount of the contract is the Proposal page of the Bid, which is signed
by the Contractor. The prices that a�e detailed in the work write-up are for information
only.
ARTICLE 16. - ARBITRATION: The Contractor and Owner agree, that in cases of apparent
irreconcilable differences arising in connection with contract performance or terms of the contract during the
progress of the construction, to submit the matter #o the Community Development Division for binding
arbitration.
ARTICLE 17. - LIEN WAIVERS: Contractor agrees to protect, defend, and indemnify the Community
Development Division and the Owner from any claims for unpaid work, labor, or materials with respect to
Cantracfor's performance. Final payment shal! not be due unti! the Contractor has delivered to the Owner, in
care of Community Development Division, complete release of all (iens for work completed arising out of
Cantracfor's performance or a receipt in full covering all (abor and materia(s for which a lien could be filed or a
bond satisfactory to the Owner indemnifying him against any lien. All nottce to owners must have a release af
lien far�nai payment. (Form Enclosed in Con#�act).
ARTICLE 18. - PROHIBITION OF KICKBACKS: The Contractor nor any of its officers, partners,
owners, agents, representatives, employees, or parties in interest has in any way colluded, conspirecl,
connived, or agreed, directly or indirectly with any other Bidder, firm, or person to submit a collusive ar shall
Bid in connection with the Contractor for which the attached Bid has been submitted or to refrain from bidding
in connection with such contract, or has in any manner, directly or indirectly, sought by contract or collusion or
communication or conference with any other Bidder, firm, or person to fix any overhead, profit, or cost element
of the Bid price or the Bid price of any other Bidder, or to secure through any collusion, conspiracy,
connivance, or unlawfui contract, any advantage against the Pasco County Community Development Divisioh,
or any person interested in the proposed cont�act; and,
The price or prices quoted are fair and proper and are not tainted by any collusion, cQnspiracy,
connivance, or unlawful contract on the par# of the Bidder or any of its agents, representatives, owners,

ARTlCLE 19. -lNTEREST OF MEMBERS. OFFICERS. EMPLOYEES OF PUBLIC BODY MEMBERS
OF L,QCAL GOYERNING BODY. OR OTHER PUBLtC OF�ICIALS: No member, offioer, or employee of
Pasco County, or its designees or agents, no member of the governing body of the locaiity in which the
program is situated, and no other public official of such locality or localities who exercises any functions or
responsibilities with respect to the program during his tenure or for one (1) year thereafter, shall have any
interest, direct or indirect, in any contract or subcontract, or the proceeds thereof, for work to be perFormed in
connection with the program assisted under the Contract.
ARTICLE 20. - PROHlBITION OF BONUS OR COMMISSION: The assistance provided under this
Contract shall not be used in payment of any bonus or comrrtission for the purpose of obtaining County
approval of the application for such assistance.
ARTICLE 21. - LOCAL EMPLOYMENT: To the greatest extent possible, the Contractor agrees fo hire,
train, and subcontract to individuals and firms tha# live or are located in Pasco County and the Tampa Bay
Area.
ARTICLE 22. - MINORITY A D WOMEN BUSINESS ENTERPRISE: To the greatest extent possible,
the Contractor agrees that he will, if it is necessary, subcontract work to minority- and women-owned
businesses.
ARTICLE 23. — SECTlON 3: HU� ACT OF 1968: The Section 3 preference requirement applies to
any contract in excess of $100,000.00. The purpose of Section 3 is to ensure fhat employment opportunities
generated by HUD financial assistance, shall to the greatest extent feasible, be directed to low- and very low-
income persons, particularly those who are recipients of government assistance for housing and to business
concerns that provide economic apportunities to fow- and very low-income persons. If this contract is in
excess of$100,000.00 the following language is applicabfe, and shall be inciuded in any subcontracts:

contract. As evidenced by their execution of this contract, the parties to this contract certify that
they are under no contractual or ather impediment, which woufd p�event them from complying
with the Part 135 regulations.
23.3 The contractor agrees to send to each labor o�ganization or representative of workers with
which the contractor has a coliective bargaining agreement or o#her conttact or understanding, if
any, a notice advising the labor organization or workers' representative of the contractor's
commitments under this Section 3 Ciause and sha(( post copies of the notice in conspicuous
places at the work site where both employees and applicants for training and employment
positions can see the notice. The notice shall describe the Section 3 preference, shell set forth
minimum number and job titles subject to hire, availability af appren#iceship and training
positions, the qualifications for each; and the name and location of the person(s) taking
applications for each of the positions; and the anticipated date the work shall begin.
23.4 The contracfior agrees to include this Section 3 Clause in every subcontract for woric in
connection with the project and agrees to take appropriate action, as provided in an applicable

provision of the subcor�tract or in this Sectian 3 Clause, upoR a finding that the subcontractor is
in violation of regulations in 24 Part CFR 135. The contractor will not subcontrack with any
subcontractor where the contractor has notice or knowiedge that the subcontractor has been
found in violation of the regulations in 24 Part CFR 135.
23.5 The contractor will certify that any vacant employment positions, including training positions,
that are filled (1} after the contractor is selec#ed but before the contract is executed, and (2)with
persons other than those to whom the regu(a#ions of 24 CFR Part 135 require employment
oppartunities to be directed, were not filled to circumvent the contractor's obligations under 24
CFR Part 135.
23.6 Noncompfiance with HUD's regulations in 24 CFR part 935 may result in sanctions, termination
of this contract for default, and debarmenf or suspension from future HUD assisted contracts.
A TICLE 24. -USE OF DISBARRED CONTRACTORS: The Contractor agrees not#o hire, employ, or
award subcontracts to, or otherwise engage the senrices of, any su6contractor during any period of debarment,
suspension, or placement in ineligibility status of 24 C.F.R., Part 24. Use of such subcontractors may require
the Contractor to repay the County any funds paid ta such persons and/or face disbarment from bidding on
future Community Development Division projects.
ARTICLE 25. - HOLD HARMLESS: The Contractor and Owner covenant and agree to indemnify
Pasco County and all of its officers and agents from any and all liability, claims, actions, suits, or demands by
any person, firm, or corpora#ion for injuries, death, or property damage arising out of or in connection with the
work to be performed by the Contractor.
ARTECLE 26. - ADDITiONAL WORK: The Owner agrees not to have any other Contractor do any
other work on the property during the period of this contract. This does not include the Owner doing work on
their own behalf.

A.WHEREAS, Pasco Caunty has been designated by the United States Department of Housing
and Urban Development as a participating jurisdiction for the receipt and use of funds as provided
by the MOME Investment Partnerships Program("HOME Program")as provided in 24 CFR Part
92;and
B.WHEREAS, the Coun#y has agreed to use the HOME funds to finance a Homeowner program
that meets the requirement as set forth in 24 CFR Part 92 as amended by the United 5tates
Housing and Urban Development Department(HUD).
C.WHEREAS, the Contractor has agreed to rehabilitate the following propet�ty:

standards, ordinances and zoning ordinances. Homes that are non-conforming but have been
ailowed to continue by#he County or applicable city shaEl be allowed to partiapate in the program.
(2) Rehabilitation Work. The work that will be done on the house shall be in accordance with the
proposai, attached as an exhibit to this agreement. Additional work shall be approved by using a
change order form. Change order forms musf be signed by the con#ractor and the Homeowner,
and approved by the County.

authorities listed in HUD's implementation regulations at 24 CFR Parts 50 and 58. An
environrrtenta! review shall be completed by Pasco Counry before any funds are obligated.
d. Displacement, Relocation and Acquisition. Not applicable due to the Contractor not being
a sub recipient or state recipient.
e. Labor. Not applicable due to HOME funds being used to rehabilitate one single family
house.
f. Lead-based paint If the house was built by and before 1978, a lead based paint
assessment was compfeted, and that is attached to this agreement.If lead paint is present
abatement/and/or removal shal( be pa�t af the proposal.
g. Conflict of interest In accordance with 24 CFR Part 92.356, the Contractor certifies that
he/she has not been in a position(as an employee, agent, consultant, officer, or an elected
or appainted offiaal of a pardcipating jurisdiction, state recipient, or subrecipient which
received HOME#unds)to exercise any functions or responsibilities with respect to activities
assisted with WOME funds or has not been in a position to participate in the decision
making process or gain inside information with regard to the disbursement of HOME funds.
The County certifies that it has no knowledge of#he Contractor having any conflict.
h. Debarment and Suspension. In accordance with 24 CFR Part 92.357, the Contracfor
certifies that he/she is not presently debarred,suspended, proposed for debarment,
declared ineligible, or voluntarily excluded from participation in receiving any HOME or
other Federa!Funds.The County certifies that it shall check on every vendor that it is
associafed wi#h this project that it is not presentEy debaRed, suspended, proposed for
debarment, declared ir�efigible, or voluntarily excluded from pa�ticipation in receiving any
HOME or other Federai Funds.
I. Flood Insurance. All Federal Emergency Management/�qency rules shall be followed

(6) Lobbying. The Contractor and the County certify that no Federal appropriated funds

afficer or employee of any agency, a member of Congress, an officer or employee of Congress,
or an employee of a member of Congress in connection w(th the awarding of this or any contrac�;
the making of any Federal grant; the making of any Federal loan; the entering into of any
cooperative agreement; and the extension, continuation, renewal, amendment, or modification of
any Federal contract, grant, loan, or cooperative agreement.
If any#unds other than Federally appropriated funds have been paid or will be paid to any person
for influencing or attemp#ing to influence an officer or employee of any agency, a rnember of
