The Owner and Contractor Agree as Set Forth Below:
ARTICLE 1. - CONTRACT SUM: The Owner shail authorize payment to the Contractor for the
pertormance of the construction work, subject to additions and deductions by approved change order, the
Contrac# Sum of One Hundred Twenty Nine Hundred and 00/100 Dollars ($120,900.00). The building to be
built on this lot is Dawn Marie.
ARTICLE 2. - THE WORK AND COMPLETION TIME: The Contractor agrees to fumish all labor,
materials, equipment, permits, licenses, and services for the proper completion of the above-identified property
in accordance with the Bid Documents and Specifications. The Contractor agrees that materials supplied are
guaranteed to be as specified. All work shall be satisfactorily completed within One Hundred Fifty (150)
calendar days after the Notice to Proceed is issued. All work shall be completed in a workmanlike manner
according to standard practices. If the workmanship does not comply with the Community Development
Division's

acceptable standards, work deemed to be below standards will be rebid and deducted from ftnal payment.
This action can prevent the contractor from bidding on any further work or p�o3ects. Substarrtial completion of
the rehab is determined when the job write-up is 100% completed. If the rehab is no# subs#antia(ly compfeted
by the due date, the contractor can not submit a bid for new projects. Upon substantial comple#ion, the
contractor has seven {7) days to complete the punch list items and fourteen (14) days to submit Final Release
of Liens from the contractors, sub-contractors, the notice to owner and afl constructian required documents. If
the contractor does not meet the fourteen (14} days for the Lien Release, he will not be able to bid on any new
projects until frnal paperwork has been submitted. The premises will be unoccupied during the course of
construction.
ARTICLE 3, - TIME OF COMMENCEMENT: Na work shall be commenced by the Contractor until
he/she has received a written Notice to Proceed from the Community Development Division. The Notice to
Proceed wi(I be issued within thirty(30) days fram the date of the Contraotor signing this Contract, or if it is not,
the Contractor has the option of withdrawing his bid and proposal. Work shall begin no later than seven (7)
days after the Notice to Proceed is issued. If work is not substantially completed within agreed upon time
period, Contractor wil! be penalized One Hundred and 00/100 Dollars {$100.00) per day liquidated damages on
new construction and Fifty and 00/100 Doliars ($50.00) per day liquidated damages on rehabilitatian projects
for every working day until substantial completion, unless approval for extension has been agreed upon in
writing by Owner, Contractor, and the Community Development Division. This provision is contingent upon
strikes, accidents, Acts of God, or other events beyond the control of Pasco Counry and the Contractor.
ARTICLE 4. - PAYMENT: The Contractor w�ll be paid according to the draw scheduie as s#ated in the
Program Policy Statement for the Coun#y's rehabilitation program after the Community Development Divlsion
conducts an ins�ection certifying that the work has been completed according to the contract. Final payment
due the Contractor will be made after the Corrtractor has fumished the Owner, in care af the Community
Development Division, satisfactory release af liens or claims for liens by subcontractors, iaborers, and material
suppiiers for completed work or installed materials. All such releases must be notarized. Payments wil! be
made in accordance with Fiorida Sfatutes, Chapter 218.735-74, Local Govemment Prompt Payment Act
considered due upon recsipt by fhe Cierk of the Circuit Court and paid upon approval by the Board �f County
Commissioners. Within thirty (30) days of completion of the contract, the contractor shal( submit the request
for final payment alang with the required docurnentation for payment. The County shall not be responsible for
any requests after the 30 days.
ARTICLE 5. -CONTRACTOR'S 1NSURANCE: The Contractor shall be responsibfe for all damages to
person or property that occu� on the job site or adjacent thereto as a result of his fauff or negligence in
connection with this cont�act. The Contra�tor shall, prior to commencing work, furnish evidence of required
cornprehensive liability insurance. The Contractor shall also fumish evidence of coverage in accordance with
Florida Workers' Compensation Laws. Contractor shall fumish Community Development Division with new,
updafed or renewed insuranc:e coverage to continue to bid on projects.
ARTICLE 6. - SUBCONTRACTS: All subcontractors shall be baund by the terms and conditions of
this con#ract insofar as it appiies to their work, but this shall not relieve the Contractor from the responsibility ta
the �wner for the proper completion of all work to be executed under this Contract, and he/she shall not be
released from this responsibility by any subcontractual agreement he may make with others. The Contractor
shall be required to furnish to the Owner and Community Development Divisian in writing, a list of#he names of
the subcontractors proposed for the principal portions of the work at this contract signing. The Contractor shall
not employ any subcontractor to whom the Owner or the Community Development Division may have a
reasonable objection. A current form for listing subcontractors is available on the Community Development
Division website. The notice to proceed will not be issued until the required sub list has been completed for this
contract and submitted to the Community Development Divis'son.

ARTICLE 7. -GENERAL RESPONSIBILITIES OF THE CONTRACTOR:
7.1 The Cantractor shall be licensed and supervise and direct the wflrk using his best skiils
and attention.
7.2 The Contractor shail provide the Owner and Community Development Division with
evidence that he/she has secured and paid for al! licenses and permits necessary#or the
proper execution of the work; and, upon completion of the job, shall provide evidence
that all work has been inspected and approved by the appropriate building inspectors.
7.3 The Contractor shall not employ on the work any un�t person or anyone not skilled or
licensed, if appropriate, in the#ask assigned to him.
7.4 The Contractor shalf be responsibie for the acts and omissions of al! his employees, and
all subcontractors, their agent and employees, and all other persans performing any of
the work. -
7.5 The contractor shal( seek approval from Community Development Division prior to
substituting any materials or products deemed "EquaP' to the specific materials or
products (isted in the most recent Bid Specifications or Work Write-Up.
7.6 The Contractor shall not assign #his contract without written consent of the owner and
the Community Development Division. Any request for assignment must be addressed
to the Comm�nity Development Division.
7.7 The Contractor at all times shail keep the premises free from accumulation af was#e
materials or rubbish caused by this work.
7.8 The Contractor shall permit the Community Devetopment Division to examine and
inspect the construction work under this contract a#any reasonable time.
7.9 The Contractor shall fumish the Owner and the Cammunity Development Divisian with
the names, addresses, and#elephone numbers of the firms to be contacted for service to
the various appliances or other equipment that was installed or repaired under this
contract.
7.10 The Contractor shall indemnify and save harmless the County, its agents and
employees, and the Owner from fiability for any injury or damages#o persons or property
resulting from the Contractor's prosecution of work pursuant to the provisions af this
contract.
7.11 The contractor shall be responsible to submit all requests for payments, change orders,
release of liens, subcontractor list, etc., on the most cu�ent forms obtained from the
Community Development Divisian website for contractors.
7.12 All products and materials used in the rehabilitation of Community Development Division
projects shall be made by American manufacturers, if available, and meet the latest
energy and water conservation requirements.

ARTICLE 8. -GENERAL RESPONSIBII.ITIES OF THE OWNER:
8.1 The Owner shall permit the Contractor to use, at no extra cost, existing utilities such as
light, heat, power, and water necessary for the proper execution and completion of the
work.
8.2 The Owner and occupants shaU cooperafe with the Contractor to facilitate the pertor-
mance of the wo�lc and are responsible for the remova( ar protec#ion of rugs, fumiture,
and clothing, if necessary.
8.3 The Owner and occupants shall be responsible for the proper use and care of the
property including all equipment and appliances.
8.4 The Owner shall provide the Contractor with access to the pro#�erty during normal
working hours on normal working days.
ARTICLE 9. -GENERAL RESPONSIBtLIT1ES OF THE COMMUNtTY DEVELOPMENT DIVtSION:
9.1 The Community Development Division sha(I provide general administration of the
contract, but it is understood by all parties that the County is not a par#y to this Contract.
9.2 The Community Development Division shall make frequent inspections of the work while
it is in progress to determine if it is proceeding in accordance with the contract docu-
ments.
9.3 The Community Development Division shall have the authority to reject work which does
not conform to#he contract documents.
ARTICLE 10. - CONTRACT DOCUMENTS: This contract consists of the Bid Proposal, the Work
Write-Up, including any drawings or plans, the General Bid Specifications, and any insurance policies speci#ied
in Article 5 above.
ARTICLE 41. - CHANGES IN THE WORK: Except in an emergency endangering life or property, no
change in the work shall be made by the Contractor unless he has received a prior writ�en order signed by the
Owner and authorized representatives of the Community Development Oivision. The contract sum and the
contract fime may be changed only by Change Order. No extra costs will be paid ta the Contractor when he
has neglected to properly evaluate the extent of the cons#ruction work. Change orders need to be in line with
bidding amounts or pricing.
ARTICLE 12. - GUARANTEES AND WARRANTiES: The Con#ractor shall guarantee to correct any
work that fails to conform to #he contract documents and shall correct such defects due to faulty materials,
equipment, or workmanship which appear during the pragress of the work or within a period of one year from
the date of finaf inspection and acceptance or such longer periods of time as may be specified by law or by '
terms of any special guarantees required by the contract documents. The provisions of this Article apply to
woric done by subcontractors as well as work done by the Contractor. Furthermore, the Contractor shall
furnish the Owner, in care of the Community Development Divisian, with all manufacturers' and suppiiers'
written guarantees and warranties covering materials and equipment furnished under the contract. These
documents are to be neatly organized and bound in a �!e type folder and submitted with the ffnal pay request.
If a warranty item malfunctians or breaks, the Contractor shall contact the Owner within 24 hours to schedule
the repair or replacement of the item. The warranty item should be repaired or replaced within seven (7) days.

ARTtCLE 13. - TERMINA'TION OF CONTRACT BY THE OWNER: If the Contractor defaults or
neglects to carry out the work in ac�ordance with the contract documents or faifs to pertorm any provisions of
the contract, the Community Development Division or the Owner acting through the Community Development
Division may, after seven {7) days' written noNce, terminate this contract and finish the work by whatever
method he deems expedient. If the unpaid balance of the contract sum exceeds the expense of finishing the
work, such excess shafl be paid to the Contractor; but if such expense exceeds such unpaid balance,the
Contractor shall pay the difference to the Owner or the Community Development Division. Failure to pay the
unpaid balance will result in disbarmenf from bidding on future Community Development Division projects.
ARTICLE 14. - TERMINATION OF CONTRACT BY THE CONTRACTOR: If the Owner fails to
perform any applicable provisions of the contract, the Contractor may, upon seven (7) days' written notice to
the Owner and the Community Develapment Division, terminate the contract, and recover from the Owner
payment for all work satisfactorily completed and all other such remedies as are otherwise specified by law
ARTICLE 15. -GENERAL CONDITIONS:
15.1 The Wor{c Write-Up shall take precedence over the basic specification artd, when in
conflic#, the material, equipment, or workmanship called for in the"Work Write-Up"wifl
be required.
15.2 The Drawinas of floor plans are diagrammatic on1y, illustrating the general intention of
the Owner; they do not show al! the work required, exact dimensions, or construction
details.
15.3 The 2a10 Bid Specifications are detailed in a separa#e booklet availabie to the Owner
and the Contractor from the Community Deveiopment Division.
95.4 Workmanship shall be done in accordance with the standards of the several trades
known as a "workmanlike manner."
15.5 Mate�ials shall be new, in good condition, and of standard grade unless otherwise
agreed in writing b�fore their delivery to the job.
15.6 Repairs shall be made by the Contractor fo all surfaces damaged by the Contractor
resulting from his or his subcontrac#or's work under this contract at no additional cos#
to the Owner. Where "repair of existing work" is called for by the cantract, the feature
is to be placed in "equal to new condition" either by patching or replacement; all
damaged, loose, or rotted parts shall be removed and replaced, and the finished work
shall match adjacent work in design and dimension.
15.7 Bids or Proaosals will be submitted at Bidder`s risk and the Owner ar the Community
Development Division reserves the right ta reject any or all bids or proposals. All bids
or proposals are to be typed-not hand written.
15.8 Buildinq Codes-All work to be done shall be subject to the regulations of local building
codes as interpreted by the applicable City or County building department/divisian
whether or no# covered by the specifications and drawings for fhe work.
15.9 Cleanup and removal from the site of all debris and waste materials resulting from his
work shall be the responsibility of�he Contractor. Materials and equi�ment that have
been removed and replaced as part of the work shall belong to the Contractor and
shail be removed from the site.

1�.10 Trade Names are used in #he basic specification to establish quali#y and type of
rtiaterial required; exact material to be used on a specific property wfil be described in
the"Work Wrfte-Up"for the particufar property.
15.11 Adiacent Propertv- When adjacent property is affec#ed or endangered by any work
done under this confract, it shall be the responsibility of the Contractor to take
whatever steps are necessary for the protection of the adjacent property and to notify
the Owner thereof of such hazards.
15.12 Pavroll Records of employee's hours worked and wages paid on construc#ion jobs
shall be made available to the Community Development Division, if requested.
15.13 The Use of Lead-Based Paint on applicable surFaces of any residential structure
undergoing Federally assisted construction or construction under any program with
federal assistance is prohibited. Any surface which contains lead-based paint shall be
considered an immediate health hazard.
15.14 The recognized amount of the contract is the Praposal page of the Bid, which is signed
by the Contractor. The prices fhat are detailed in the work write-up are for information
only.
ARTICLE 16. - ARBITRATION: The Contrador and Owner agree, that in cases of apparent
irreconcilable differences arising in connection with contract performance or terms of the contract during the
progress of #he construction, to submit the matter to the Community Development Divisivn for binding
arbitration.
ARTfCLE 17. - LIEN WAIVERS: Contractor agrees to protect, defend, and indemnify the Community
Development Division and the Owner from any claims for unpaid work, labor, or materials with respect to
Contractor's performanc�. Final payment shal! not be due until the Contractor has delivered to the Owner, in
care of Community Devefopmen# Division, camplete release of all liens for work completed arising ou# of
Contractor's performance or a receipt in fuli covering all labor and materials for which a lien could be filed or a
bond satisfactory to the Owner indemnifying him against any lien. All notice to owners must have a release of
lien for final payment. (Form Enclosed in Contract}.
ARTICLE 18. - PROHIBITION OF KICKBACKS: The Contractor nor any of its officers, partners,
owners, agents, representatives, employees, or parties in interest has in any way colluded, conspired,
connived, or agreed, directly or indirectly with any other Bidder, firm, or person to submit a collusive or shall
Bid in connection with the Contractor for which the attached Bid has been submi#ted or to refrain from bidding
in connection wi#h such contract, or has in any manner, directly or indirectly, sought by contrac#or collusion or
communication or conference with any other Bidder, firm, or person to fix any ove�head, profit, or cost element
of the Bid price or the Bid price of any other Bidder, or to secure through any collusion, conspiracy,
connivance, or uniawful contract, any advantage against the Pasco County Community Development Division,
or any person interested in the proposed contract; and,
The price or prices quoted are fair and prvper and are not tainted by any collusion, conspiracy,
connivance, ar unlawful contract on the part of the Bidder or any of its agents, representatives, owners,
employees, or parties in interest, including this affiant.

� ARTICLE 19. - iNTEREST OF MEMBERS. OFFlCERS, EMPLOYEES OF PUBLIC BODY MEMBERS
OF LOCAL GOVERNING B�DY. OR OTHER PUBLIC OFFICIALS: No member, officer, or employee of
Pasco County, or its designees or agents, no member of the governing body of the locality in which the
program is situated, and no other public official of such locality or localities who exercises any functions ar
responsibilities with respect to the program during his tenure or for one(1) year thereafter, shall have any
interest, direct or indirect, in any contract or subcon#ract, or the proceeds thereof, for woric to be pertormed in
connec#ion with the program assisted under the Contract.
ARTICLE 20. - PROH1BiTION OF BONUS OR COMMISStON: The assistance provided under this
Contract shall not be used in payment of any bonus or commission fo� the purpose of obtaining County
approval of the applicatian for such assistance.
ARTICLE 29. -LOCAL EMPLOYMENT: To the greatest extent possible, the Contractor agrees to hire,
train, and subcantract to individusts and firms that live or are focated in Pasco County and the Tampa Bay
Area.
ARTICLE 22. - MINORtTY AND WOMEN BUSINESS ENTERPRISE: To #he greatest extent possible,
the Contractor agrees that he wil1, if it is necessary, subcontract work to minoritar- and women-owned
businesses.
ARTICLE 23. — SECTION 3: HUD ACT OF 1968: The Section 3 preference requirement applies tn
any cont�act in excess of $'Id0,000.00. The purpose of Section 3 is to ensure that emp(oyment opportunities
generated by HUD financial assistance, shall to the greates# extent feasible, be directed to law- and very low-
income persons, particularly those who are recipients of govemment assistance for housing and to business
concems that provide economic opportunities to low- and very (ow-income persons. If this contract is in
excess of$100,000.00 t�e following language is applicable, and shall be included in any subcontracts:
Section 3 Clause: All section 3 contracts shaq include the following clause (referred to as the Section 3
Clause):
23.1 The work ta be performed under this contract is subject to the requirements of Section 3 of the
Housing and Urban Development Act of 1968, as amended, 12 U.S.C. 1701u. Section 3
requires that, to the greatest extent feasible, opportunities for training and employment be given
lower-income residents of the project area, and cantracts for work in connection with the project
be awarded to business concerns, which are located in or owned in substantial par� by persons
residing in the area of the praject.
23.2 The parties to this contract agree to comply with HUD's regulations in 24 Part CFR 135, and al!
applicable rules and orders of the Department issued thereunder prior to the execution of this
contrac#. As evidenced by their execution of this contract, the parties to this contract certify that
they are under no contractual or other impediment, which would prevent them from complying
with the Part 135 �egulations.
23.3 The contractor agrees to send to each labor organization or representative of workers with
which the contractor has a collective bargaining agreement or otlier cantract or understanding, if
any, a notice advising the labor organization or workers' representative of the contractors
commitments under this Section 3 Clause and shall post copies of the notice in conspicuous
places a# the work site where both employees and applicants for training and employment
positions can see the notice. The notice shafl describe the Section 3 preference, sha(I set forth
minimurn number and job titles subject to hire, availability of apprenticeship and training
positions, #he quali�cations for each; and the name and location of the person{s) taking
applications for each of the positions; and the anticipated date the work shal! begin.
23.4 The contractor agrees to include this Section 3 Clause in every subcontract for work in
connection with the project and agrees to take appropriate action, as provided in an applicable

provisio.n of the subcontract or in this Section 3 Clause, upon a fnding that the subcor�tractor is
in violation of regulations in 24 Part CFR 135. The confractor will not subcontract witf� any
subcontractor where the contractor has notice or knowledge that the subcontractor has been
found in violation of the regulations in 24 Part CFR 135.
23.5 The contractor will certify that any vacant emplayment positions, including training positions,
that are filled {1) after the contractor is selected but before the contract is executed, and (2)with
persons other than those to whom the regulations of 24 CFR Part 135 require employment
opportunities to be directed, were not filled to circumvent the contractor's obligations under 24
CFR Part 135.
23.6 Noncompliance with HUD's reguiations in 24 CFR part 135 may result in sanctions, terminatian
of this contract fo�defautt, and debarmen#or suspension fram future HUD assisted contracts.
ARTICLE 24. - USE OF DISBARRED CONTRACTORS: The Contractor agrees not to hire, employ, or
award subcontracts to, or otherwise engage the services of, any subcontractor during any period of debarment,
suspension, ar placement in ineligibility status of 24 C.F.R., Part 24. Use of such subcontractors may require
the Contractor to repay the County any funds paid to such persons andlor face disbarment from bidding on
future Community Development Division projects.
ARTlCLE 25. - HOLD HARMLESS: The Contractor and Owner covenant and agree to indemnify

FOR GENERAL CONTRACTORS
HOME INVESTIUIENT PARTNERSHtPS PR�GRAM
(Only if appiicable if assistance funded with HOME Funds)
THiS AGREEMENT is entered into this 21 day of November 2013, by and between the Pasco County Board
of County Commissioners (Community Development Division), (hereinafter referred to as the"County"}, and
Covenant Communities. Inc. (hereinafter referred to as the"Contractor").
THIS AGREEMENT 1S ENTERED INTO BASED ON THE FOLLOWING FACTS:
A. WHEREAS, Pasco County has been designateti by the United States Department of Housing and Urban
Development as a participating jurisdiction for the receipt and use of funds as provided by the HOME
Investment Partnerships Program ("HOME Program") as provided in 24 CFR Part 92; and
B. WHEREAS, the County has agreed to use the HOME funds to finance a Homeowner program that meets
the requirement as set forth in 24 CFR Part 92 as amendecf by the United States Housing and Urban
Development Department(HUD).
C. WHEREAS, the Contractor has agreed to rehabilitate the follawing property:

NOW, THEREFORE, the County and the Contractor do mutually agree as follows:
{1) Housing Quality Standards. Once rehabilitation is completed, the property will meet the property
standards (HQS} as outlined in 24 CFR Part 92.251 and the County will ensure that the property will meet the
requirements of its written standards, applicable local codes, rehabilitation standards, ordinances and zoning
ordinances. Homes that are non-conforming but have been a(lowed to continue by the County or applicab(e
city shall be aflowed to participate in the program.
(2) Rehabllitation Work. The work that will be done on the house shall be in accorclance with the proposal,
attached as an exhibit to this agreement. Additional work shall be approved by using a change order fonn.
Change order forms must be signed by the contractor and the Homeowner, and approved by the County.
It is estirnated that the work will be completed according to the following schedule:
Confract Signing: November 21. 2013
50°/a Completion 03/05/14
100% Compietion 05/19/14
The budget for this project is estimated as fallows:
Titie Costs 500.00
Rehab Work $120.900.00
Total $121,400.00
The original contract calls for the work to be completed in 150 days.
(3) Other Program Requirements.The Contractor and the County must comply with all applicable federal
laws and regulations as described in the HUD guidelines (24 CFR Part 92}.
a. Equal Opportunity and Fair Housing. No person in the United State shall on the grounds of race, color,
national origin, religion, handicap or sex be excluded from participation in, be denied the benefits of, or
be subjected to discrimination under any program or activity funded in whale or in part with HOME funds,
This shalf be enforced by Pasco County.
b. Affirmative Marketing. Not applicab(e due to the Contractor not being a subrecipient or sta#e recipient.

c. Environmentai Review. The environmental effects shall be assessed in accordance with the provisions
af the National Environmental Policy Act of 1969 (NEPA) and the related authoritles Iisted in HUD's
implementation regulations at 24 CFR Parts 50 and 58. An environmental revisw shall be completed by
Pasco County t�e#ore any funds are obligated.
d. Disp(acement, Relocation and Acquisition. Not applicable due to the Contractor not being a
subrecipient or state recipient.
e. Labor. Not applicable due to HOME funds being used to rehabilitate one single family house.
f. Lead-based paint. If the house was built by and before 1978, a lead based paint assessment was
compfeted, and that is attached to this agreement .If lead paint is present abatemenU and/or removal
shal! be part of the propasal.
g. Conflict of Interest. In accordance with 24 CFR Part 92.356, the Contractor certifies that he/she has not
been in a position (as an employee, agent, consuitant, officer, or an elected or appointed afficial of a
particEpating jurisdiction, state recipient, or subrecipient which received HOME funds)to exercise any
functions or responsibilities with respect to activities assisfed with HOME funds or has not been in a
position tfl participate in the decision making process or gain inside information with regard to the
disbursement of HOME funds. The County certifies that it has no knowledge of the Contractor having any
conflict.
h. Debarment and Suspension. fn accordance with 24 CFR Part 92.357,the Contractor certifies that
he/she is not presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily
excluded from participation in receiving any HOME or other Federal Funds. The County certifies that it
shalf check on every vendor that it is associated with this project that it is not presently debarred,
suspended, proposed for debarment, declared ineligible, or voluntarily exciuded from participation in
receiving any HOME or other Federal Funds.
I. Flood Insurance. All Federa! Emergency Management Agency rules shall be follawed
j. Executive Order 12372. Nat appiicable since the HOME funds are not competitive.
(4j Records and Reports. The County agrees to complete aN applicable reports and maintain documentation
in accordance with HUD guidelines for a period of five years after the loan has been paid-off.
(5)Term.The term of this agreement shall be for one year after the final building inspection, which is
schedufed for 05/19/15.
(6) Lobbying. The Contractor and#he County certify that no Federal appropriated funds have been paid
by or on behalf of it to any person for influencing ar attempting to influence an officer or employee of any
agency, a member of Congress, an officer or employee of Congress, or an emplayee of a member of Congress
in connection with the awarding of this ar any contract; the making of any Federal grant; the making of any
Federal loan; the entering into of any cooperative agreement; and the extension, continuation, renewal,
amendment, or modificatian of any Federal contract, grant, loan, ar cooperative agreement.
If any funds other than Federally appropriated funds have been paid or will be paid to any person for
influencing or attem�ting to influence an officer or employee of any agency, a member of Congress, an officer
or employee of Congress, or an employee of a member of Congress in connection with this agreement, the
Cantractor shall complete and submit Standard Form LLL, Discfosure Form to Report Labbying, in accordance
with its instructions.
