which may be more restrictive than County regulations. The undersigned assumes responsibility for compliance with any
appiicable deed restrictions.
UNLICENSED CONTRACTORS APID CONTRACTOR RESPONSIBILITIES: If the owner has hired a contractor or
contractors to undertake work,they may be required to be licensed in accordance with state and local regulations. If the
contractor is not licensed as required by law, both the owner and contractor may be cited for a misdemeanor violation
under state law. If the owner or intended contractor are uncertain as to what licensing requirements may apply for the
intended work,they are advised to contact the Pasco County Building Inspection Division—Licensing Section at 727-847-
8009. Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(s) sign
portions of the"contractor Block"of this application for which they will be responsible. If you, as the owner sign as the
contractor,that may be an indication that he is not properly licensed and is not entitled to permitting privileges in Pasco

TRANSPORTATION IMPACTIUTILITIES IMPACT AND RESOURCE RECOVERY FEES: The undersigned understands
that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings,change of
use in existing buildings,or expansion of existing buildings, as specified in Pasco County Ordinance number 89-07 and
90-07,as amended. The undersigned also understands,that such fees,as may be due,will be identified at the time of
permitting. It is further understood that Transportation Impact Fees and Resource Recovery Fees must be paid prior to
receiving a"certificate of occupancy"or final power release. If the project does not involve a certificate of occupancy or
final power release,the fees must be paid prior to permit issuance. Furthermore, if Pasco County WateNSewer Impact
fees are due,they must be paid prior to permit issuance in accordance with applicable Pasco County ordinances.
CONSTRUCTION LIEN LAW(Chapter 713,Florida Statutes,as amended): If valuation of work is$2,500.00 or more,I
certify that I, the applicant, have been provided with a copy of the "Florida Construction Lien Law—Homeowner's
Protection Guide"prepared by the Florida Department of Agriculture and Consumer Affairs. If the applicant is someone
other than the"owner",I certify that I have obtained a copy of the above described document and promise in good faith to

I understand that the following restridions apply to the use of fill:
- Use of fill is not allowed in Flood Zone"V"unless expressly permitted.
- If the fill material is to be used in Flood Zone "A", it is understood that a drainage plan addressing a
"compensating volume"will be submitted at time of permitting which is prepared by a professional engineer
licensed by the State of�lorida.
- If the fill material is to be used in Flood Zone "A" in connection with a permitted building using stem wall
construction,I certify that fill will be used only to fill the area within the stem wall.
- If fill material is to be used in any area, I certify that use of such fill will not adversely affect adjacent
properties. If use of fill is found to adversely affect adjacent properties,the owner may be cited for violating
the conditions of the buitding permit issued under the attached permit application,for lots less than one(1)
acre which are elevated by fill,an engineered drainage plan is required.
If I am the AGENT FOR THE OWNER,I promise in good faith to inform the owner of the permitting conditions set forth in
this a�davit prior to commencing construction. I understand that a separate permit may be required for electrical work,
plumbing, signs,wells, pools, air conditioning, gas, or other installations not specifically inciuded in the application. A

set aside any provisions of the technical codes,nor shall issuance of a permit prevent the Building Official from thereafter

unless the work authorized by such permit is commenced within six months of permit issuance,or if work authorized by
the permit is suspended or abandoned for a period of six(6)months after the time the work is commenced. An extension
may be requested,in writing,from the Building Official for a period not to exceed ninety(90)days and will demonstrate
justifiable cause for the extension. If work ceases for ninety(90)consecutive days,the job is considered abandoned.

clate, the contractor can not submit a bid�for new projec s. Upon substantial completian, the cont�actor has
seven (7} days to complete the punch list items and fou een (14) days to submit Final Refease of Liens from
the contractors, sub-contractors, the notice to owner and Ii construction required documents. If the contractor
� does not meet the fourteen (14) days for the Lien Release, he wi(( not be able to bid an any new projects until
; final paperwork has been submitted. The premises will b �unoccupied during the course of construction.
ARTtCL.E 3. - TtME OF C4MNfENCEMENT: N wark shall be commenced by the Contractor until
j he/she has received a written Notice to Proceed from t e Community Development Divisian. The Notice ta
' Proceed wiil be issued within thirty (30} days ft'am the da�e of the Contractor signing this Carttract, or if it is not,

� ARTICI.E 4. - PAYMENT: The Contractar wi(I b paid according to the draw schedule as stated in the
Program Policy Statement for the County`s rehab�litatio� program after the Community Deve(opment Division
conducts an inspection certifying that the work has bee completed according ta the cantract. Final payment
due the Contractor will be made after the Contractor has furnished the eiwner, in care of the Community
Deveiapment Division, satisfac#ory release of fiens ar c1 ims#or liens �y subco�tractors, laborers, and materiaf
suppiiers far compieted worEc or installed materiafs. AI such releases must be notarized. Payments will be
rnade in accordance wi#h Fiorida Sta#utes, Chapter 18.735-74, Laca! Gavernment Prampt Payment Act
considered due upon receipt by the Clerk of the Circuit Coutt and paid upon approva! by#he Board of County
Commissianers. Within thir#y {30) days of campletion f the contract, the cantractor shall submit the request
for fina! payment along with the required documentatia � for payment. The County shall not be responsible for
any requests after the 30 days.
ARTICLE 5. - CONTRACTOR'S INSURANCE: The Cantractor shail be responsibie for ali damages to
person or property that occur on the job site or adj cent thereto as a result of his fauit or neg}igence in
connection with this contract. The Contractar shall, p ior to commencing wock, furnish evidence ofi required
comprehensive liability insurance. The Contractor sha I alsa furnish evidence af coverage in accordance with
Fiorida Woefcers` Compensation Laws. Contractor sh 11 furnish Community Development Division with new,
updafed or renewed insurance cauerage to continue ta ic! on projects.
ARTlC�E 6, - SUBGt}NTRACTS: Al! subcon# actors shall be bound by the terms and conditions of
#his con#ract insofar as it applies ta their work, but this hall not relieve the Contractor from the responsibility to
the flwner for the proper completion of a!! work to be executed under this Contract, and he/she shall riot be
released fram this responsibility by any subcontractua agreement he may make with others. The Cantractor
shall be required to furnish to the Owner and Commun'ty Development Division in writing, a list of the names of
the subcantractors proposed for the principal portions f the work at this contract signing. The Contractor shall
not empfoy any subcontractor to whom #he Owner r the Community Development Division may have a
reasonable objection. A current form for listing subc ntractars is available on the Community Development
Divisian website. The notice to proceed wii( not be iss ed until the required sub list has been compfeted for this
contract and submitted to the Community Developmen Division.

7.1 The Contractor shall be l4censed ancf supervise and direct the work using his best skills
artd attention.
7.2 The Contractor shall provide the wner and Community Development Division with
evidence that he/she has secured a d paid far all licenses and permits necessary for the
proper execution of the work; and, upon comp(etion of the job, shall provide evidence
that all work has been inspected an� approved by the appropriate building inspectors.
7.3 The Contractor shali not employ o� the wark any unfit persan ar anyone not skiiled ar
(icensed, if appropriate, in the task assTgned ta him.
7.4 The Contractor shall be respar►sibl for the acts and omissions of all his emplayees, and
afi subcontractors, their agent and employees, and al! other persons perForming any of
the wark.
7.5 The cantractor shall seek appro al from Community Development Division prior to
substituting any materials or pro�ucts deemed "EquaP' to the specific materials or
products listed in the mast recent �id Speci�cations or Work Write-Up,

neglects to carry out the work in accordance with the car�tract documents or faiEs to perfarm any,provisions of
the contract, the Community Development Division or th� �wner acting through the Gammuni#y Devebpment
Division may, after seven (7} days' written notice, term nate #his contract and finish the work by whatever
method he cteems expedient. If the unpaid balance of#�e contract sum exceeds the expense of finishing the
