This agreement is made on 11/15/2014 , by and between Carr
Contracting Services Inc. (Contractor), wh se place of business is located at 3310 Drum
Rd., Zephyrhills, F133541, and Kathleen O s Property Owner, whose residence is
located at the following address: 39026 lOT AVE Ze h hills FL 33542 (Location of
Work)
This contract is for construction of: Replace ent of sliding glass doors by owner
supplied French Doors.
Carr Contracting Services Inc. and Kathleen Oaks hereinafter referred to as_Owner,
entered into a certain contract in writing for project known as:
Door Replacement at 39026 lOth AVE, Zep yrhills, FL 33542.
Carr Contracting Services Inc. is required to furnish certain labor and materials,
and to perform certain construction in accor ance with the contract for the project which
collectively are hereinafter referred to as the " Construction Contract ", and
listed in detail in attachment A-(Contracto s Proposal) , attachment
B- (Insurance Certificates/Contractor Lice ses.
All work listed as part of this contract shall e completed for the sum of
$ 450.00

Now,therefore, it is agreed by and between t e parties herein, as follows:
1. Carr Contracting Services Inc. shall provid all material, equipment, labor services,
and any other items necessary to fully compl te the work set forth in Attachxnent A- (
Contractors Proposal). Carr Contracting Se ices Inc. shall at it's own expense, secure all
insurances, furnish all labor, material, tools, quipment, machinery, scaffolding, cartage,
and provide all other things and personnel ne essary for the full and diligent prosecution
thereof, all in compliance with applicable sta utes, ordinances and regulations, and in a
first class workmanlike manner, and in strict ccordance with the requirements of the
Construction Contract, as well as the directio of the Owner, and to the satisfaction of the
Owner,
(Note): Any change directives by Owner ab ve and beyond the outlined scope of work,
that increases the value of original contract s all be agreed upon in writing by both
� parties, and shall be considered a " Change rder ", and original contract sha11 be
modified to reflect as such.
2. Carr Contracting Services Inc. acknowled es that no representation as to the work
have been made by the Owner, or anyone on it's behalf, except as in this contract
, expressly set forth, and the Contractor warr ts and represents that it has familiarized
itself with existing conditions and the chaxac er of the operations to be carried on under
the Construction Contract, and that it fully u derstands the facilities, difficulties and
restrictions attending the execution of the w rk under this Contract;the Contractor fully
understands the requirements of the various overnmental agencies, departments, and
bureaus having jurisdiction, and from it's o investigations, has satisfied itself as to the
nature and location of the work, all local co itions, and all matters which may affect the
work or it's performance.
3. Carr Contracting Services Inc. shall be re onsible for supervision, and coordination of
it's trades, and all actions reasonably require to complete the work, even if not shown on
the plans, but can be reasonably inferred.
4. Carr Contracting Services Inc. shall prov'de Engineered drawings and submittal letters
by the Engineer of Record which may be re uired to fully complete the scope of work.
5. Carr Contracting Services Inc. shall be lia le for any damages to adjacent surfaces
caused by it's work, either accidentally or t ough negligence, Contractor is not
' responsible for damages caused by others.
6. Carr Contracting Services Inc. agrees to a ide by all OSHA safety guidelines for the
type of work being performed.

7. A: Owner shall pay Carr Contracting Servi es Inc. as follows:
Draw schedule payments shall be made withi 3 Davs of a receipt of Request for
Payment from the Owner for 100% of moni s due as outlined in this contract, after all
work has passed municipal inspections, and ork is done to the satisfaction of the owner.
B: Carr Contracting Services Inc. upon rec ipt of full and final payment sha11 produce
to Owner a fully executed, notarized " Final elease of Lien " for a11 monies owed as
outlined in this Contract.
8. Upon acceptance by Owner of final const ction, Carr Contracting Services hereby
shall warranty the material and craftsmanshi , for a period of one year from date of
substantial completion.
9. This Contract constitutes the entire unders anding of the parties, and no representations
or agreements of any kind have been made e cept as specifically herein contained and
incorporated. This Contract may be cancell d at any time by Owner, or Contractor, for
breach of any terms contained herein. Carr C ntracting Services Inc. shall be given seven
days written notice to cure any construction lated issues, Owner shall be given seven
days written notice for non payment if no res onse or compliance, then this contract will
be cancelled, and all outstanding matters sha be litigated in the court having jurisdiction
of the county where work commenced.
By signing below, both parties agree to be b nd to the terms of this agreement. .
Any litigation that may arise shall be govern d by AIA Construction Contract Documents
for general construction disputes.
