DISCLOSIIRE STATEMENT FOR OWNER <br /> CITY OF ZEPFIYRHILLS BIIILDING DSPARTN�NT <br /> I, have read and fully uaderstand aad <br /> agree to the provisions of this iastrumeat. <br /> The undersigned states and affirms that he or she is desirous of constructing, <br /> reaovatiag, addiag to or reroofiag his or her own domicile, that he or ahe <br /> actually occupies, or will occupy by � said domicile, aad same is not for <br /> reat, lease or sale. That he or she shall comply with the followiag conditions: <br /> 1. That the owaer aad he or ahe aloae shall act as the builder for all phases of <br /> coastruction. <br /> 2. That the owner will comply with all provisioas of the City of Zephyrhills <br /> ordinaaces aad codes pertinent to the buildiag. � <br /> 3. That ia the eveat various phases of construction are subcoatracted, he will <br /> engage only properly liceased subcoatractors and will persoaally supervise <br /> such work. <br /> 4. That ia the event the Building Iaspec�tor shall require corrections to be made, <br /> the owner will assume full responsibility to iasure they are made, aad upon <br /> completion will call for a reinspection before proceediag with the building. <br /> 5. That the owaer shall assume full respoasibility for the conatructioa aad will <br /> aot Expect superviaioa of his work �rom the City of Zephyrhilla Building <br /> Departmeat. <br /> 6. That prior to fiaal inspection any additioaal fees, iacluding reinspection <br /> fees, must be paid ia full. A written request from this office shall <br /> constitute an official aotice to pay additioaal fees. <br /> 7. That the owner shall comply with al�l City, State aad Federal laws in regard to <br /> social security, workman.'s,compensa�tion, lien laws, etc. , where applicable. <br /> 8. That the owner shall comply with al�l the safety codes issued by the Florida <br /> Industrial Commiasioa. <br /> 9. State law requires coastructioa to e doae by licensed contractors. You have <br /> applied for a, permit uader aa exemp�tioa to that law. The exemption allows <br /> you, as the owner of your property, to act as your own coatractor with certain <br /> restrictions evem though you do aot�I have a license.. You must provide direct <br /> onsite supexvision of the coastruct�ioa you`rself. You may build or improve a <br /> one-family or two-family residence or a farm outbuilding. You may also build <br /> or improve a commercial ,building, provided your coats do aot exceed $75,000. <br /> The building or residence must be for�your own use or occupancy. It may aot <br /> be built or substaatially improved for sale or lease. If you sell or lease a <br /> buildiag you have built or substaa ially improved yourself withia 1 year after <br /> the coastructioa is complete, the �aw will presume that you built or <br /> substantially improved if for sale�or lease, which is a violation of this <br /> exemption. You may aot hire aa ualicensed persoa to act as your coatractor or <br /> to supervise people workiag on you� buildiag. It is your responsibility to <br /> make sure that people employed by you have licenses required by state law and <br /> by couaty or muaicipal licensiag or�da.aaaces. You may aot delegate the <br /> responsibility for aupervising work to a licensed coatractor who is uot <br /> liceased to perform the work beiag done.
